Bio Data

Penyakit Kusta merupakan penyakit menular disebabkan oleh
Mycobacterium leprae yang bersifat intraseluler obligat dan
dapat menyerang ke saraf perifer sebagai afinitas pertama,
kulit dan mukosa traktus respiratorius bagian atas, kemudian
dapat ke jaringan tubuh lainnya kecuali susunan saraf pusat

Th 1873: M. Leprae ditemukan oleh
Hansen dari Norwegia
Th 1879: Albert Neisser (Jerman)
mewarnai kuman ini dg gentian violet
dan fuchsin
Robert menemukan bahwa kuman
ini BTA

Mycobacterium Leprae ini merupakan
kuman aerob, berbentuk batang
di kelilingi oleh membran sel lilin,
berukuran panjang 1-8 micro,
lebar 0,2-0,5 mikron, waktu pembelahannya
lama yaitu 2 – 3 minggu dan masa tunasnya rata –
rata 2- 5 tahun, diluar sel tubuh manusia dalam
kondisi tropis bakteri dari sekret nasal dapat
bertahan sampai 9 hari serta bersifat tahan asam
(BTA) atau gram positif

Skin Smear atau sediaan apusan kulit
adalah pemeriksaan sediaan yang diperoleh
lewat irisan dan kerokan kecil pada kulit
yang kemudian diberi pewarnaan tahan
asam untuk melihat Mycobacterium leprae
dibawah mikroskop

→ mempercepat penegakan diagnosis dan klasifikasi.
→ 7-10 % kasus klasifikasi PB merupakan kasus MB
dini.
→ Diagnosis dan klasifikasi yang meragukan harus
dilakukan pemeriksaan skin smear
Menurut hasil penelitian,
skin smear berguna untuk :

TUJUAN PEMERIKSAAN
BACTERIOLOGIS
 Membantu menentukan diagnosis penyakit
kusta, terutama pada kasus tersangka
(suspek) kusta
 Membantu menentukan klasifikasi
penyakit kusta pada pasien baru
 Membantu diagnosis pasien relaps dari
pasien yang sebelumnya pernah mendapat
pengobatan
 Membantu menilai hasil pengobatan
(pemeriksaan dilakukan minimal 2 kali,

 Ambillah skin smear pada cuping telinga kanan dan kiri
ditambah 2 atau 3 bercak pada bagian tubuh lain.
Sediaan diambil dari kelainan kulit paling aktif (lesi
yang meninggi dan berwarna kemerahan). Jika tidak
ada lesi kulit yang sesuai, ambil skin smear dari lokasi
yang sebelumnya diketahui aktif atau lokasi dimana
skin smear sebelumnya positif
Beberapa ketentuan dalam pengambilan skin smear:

Beberapa ketentuan dalam pengambilan skin smear:
 Kulit muka sebaiknya dihindarkan karena alasan kosmetik
kecuali tidak ditemukan kelainan kulit di tempat lain.
 Pada pemeriksaan ulang dilakukan di tempat kelainan kulit
yang sama dan bila perlu di tambah dengan lesi kulit yang
baru timbul.
 Sebaiknya petugas yang mengambil dan yang memeriksa skin
smear dilakukan oleh orang yang berbeda. Hal ini untuk
menjaga pengaruh gambaran klinis terhadap hasil
pemeriksaan bakteriologis

Ketentuan dalam pengambilan skin smear:
Lokasi pengambilan ;
• Cuping telinga kanan / kiri
• 2-3 bercak yg aktif, Pilihlah
lokasi tempat lesi kulit yang
paling aktif
• kulit wajah sebaiknya dihindar

Persiapan pengambilan skin smear
1. Persiapan peralatan
a. Kaca obyek yang baru dan kotak kaca obyek (slide box)
b. Skalpel (Tangkai pisau ukuran No. 3 dan pisau No. 15)
c. Lampu spiritus (bunsen)
d. Kapas alkohol
e. Kapas kering
f. Korek api
g. Pensil kaca atau spidol
h. Penjepit kaca obyek
i. Sarung tangan

Persiapan pengambilan skin smear
2. Berikan penjelasan kepada pasien
Mintalah pada pasien untuk duduk dengan tenang. Jelaskan apa
yang akan anda lakukan dan mengapa hal tersebut harus
dilakukan. Jawablah tiap pertanyaaan yang muncul dari pasien.
Catat permintaan pada formulir permintaan pemeriksaan
laboratorium.
Ingatlah bahwa prosedur pengambila skin smear merupakan
tindakan invasif. Sebelumnya cucilah tangan, kenakan sarung
tangan dan gunakan peralatan yang sudah disterilkan, serta mata
pisau scalpel baru bagi tiap pasien. Jangan mengambil skin
smear jika tidak ada indikasi

Tempatkan semua material dan
perlengkapan yang anda butuhkan di
atas meja yang bersih. Juga formulir
permintaan pemeriksaan kaca obyek
dan penanda (marker) atau label
penanda kaca obyek.

Cara Pengambilan skin smear
Cucilah tangan lalu kenakan sarung tangan
✓ Ambil kaca obyek sediaan yang baru, bersih dan tidak tergores.
Beri tanda atau nomor pada bagian bawah kaca obyek atau label
kaca obyek sesuai nomor identitas pasien
✓ Bersihkan lokasi kulit tempat pengambilan skin smear dengan
kapas alkohol. Biarkan mengering.
✓ Nyalakan api spiritus , pasanglah bisturi (mata pisau skalpel)
pada gagangnya.
✓ Jepitlah kulit dengan erat menggunakan jempol dan telunjuk;
tetap jepit dengan kuat agar darah tidak ikut keluar.

✓ Buatlah insisi (irisan) pada kulit dengan
panjang sekitar 5 mm dan dalam 2 mm (3).
Kulit tetap dijepit agar tidak ada darah yang
keluar. Jika berdarah, bersihkan darah
tersebut dengan kapas alkohol.
✓ Putar pisau skalpel 90° dan pertahankan
pada
sudut yang tepat pada irisan. Keroklah irisan
tersebut menggunakan skalpel guna
mengumpulkan cairan dan bubur jaringan.
✓ Lepas jepitan dan buat apusan diatas
objek glass

✓ Ulangi langkah di atas untuk lokasi apusan lain.
✓ Lepas pisau skalpel dengan hati-hati. Tutup
luka dan ucapkan terima kasih pada pasien.
✓ Biarkan kaca obyek tersebut mengering.
Fiksasi apusan dengan melewatkannya di atas
nyala api bunsen 3 kali. Taruh kaca obyek di
kotak kaca obyek/Box slide dan kirimkan ke
laboratorium disertai dengan form permintaan
pemeriksaan

PEWARNAAN SKIN SMEAR
1. Peralatan dan Reagensia
a. Reagen ziehl nelsen yang terdiri dari : Larutan Carbol Fuchsin 0,3 %, asam
alkohol 3 % dan methylen blue 0,3 %.
b. Lampu spiritus (bunsen)
c. Wadah dengan air mengalir
d. Pipet tetes e. Besi penyangga
f. Timer
g. Rak kaca obyek
h. Sarung tangan
Catatan :Buat register kaca obyek di register laboratorium.Letakkan kaca obyek di
rak pewarnaan dengan sisi apusan menghadap ke atas. 10 kaca obyek atau lebih
dapat diwarnai bersamaan. Pastikan bahwa kaca obyek tersebut tidak saling
bersentuhan satu dan yang lain

2. Melakukan Pewarnaan dengan menggunakan
metode Ziehl-Nelsen.
1. Tuang carbol fuchsin 0,3 %, pada sediaan skinsmear yang telah
difiksaasi, panaskan dengan api sampai keluar uap, biarkan mendingin
selama 5 menit
2. Cuci dengan air mengalir.
3. Bilas Asam alkohol 3 % untuk menghilangkan semua warna ( kurang
lebih 10 menit)
4. Cuci dengan air yang mengalir.
5. Tuangkan methyllen blue 0,3 % selamat untuk latar belakang.
6. Bilas dengan air yang mengalir. Basil kusta akan terlihat seperti batang-
batang merah dengan latar belakang berwarna biru.

Cara melakukan pembacaan skin smear
1. Letakkan kaca obyek di bawah mikroskop dengan apusan
menghadap ke atas dan nomor identitas terletak dikiri
2. Focuskan gambar menggunakan objektif 10 kali
3. Tetesi hapusan dengan setetes oil immersi
4. Ubah objektif menjadi pembesaran 100 kali. Ini akan
membuat lensa objektif menyentuh oil immersi (jika perlu,
pindahkan sekrup makrometer untuk memastikan bahwa
minyak immersi menyentuh lensa)
5. Buka diafragma seluruhnya dan naikkan kondensor ke posisi
tertinggi
6. Fokuskan dengan tepat menggunakan mikrometer

Bentuk – bentuk Kuman Kusta yang dapat ditemukan
dalam lapangan pandang mikroskop
1. Bentuk Utuh (Solid)
a. Dinding sel tidak putus
b. Mengambil zat warna secara merata
c. Panjang kuman 4 kali lebarnya
2. Bentuk pecah-pecah (fragmented)
a. Dinding sel terputus mungkin sebagaian atau seluruhnya
b. Pengambilan zat warna tidak merata

Bentuk – bentuk Kuman Kusta yang dapat ditemukan
dalam lapangan pandang mikroskop
3. Bentuk granular (ganulated) : Kelihatan seperti titik-titik tersusun
garis lurus atau berkelompok
4. Bentuk globus : Beberapa BTA utuh atau
fragmented/granulated mengadakan ikatan atau kelompok.
Kelompok kecil 40-60 BTA. Kelompok besar 200-300 BTA
5. Bentuk Clumps : Beberapa bentuk granular membentuk pulau-
pulau tersendiri (lebih dari 500 BTA)

Cara menghitung BTA
Sebaiknya dicari lapangan penglihatan yang paling baik, artinya
tidak/paling sedikit mengandung globus atau clumps. Apabila ditemukan
globus atau clumps jangan dihitung. Kita dapat menggunakan cara :
1. Cara zig zag (Zig Zag Method)
2. Cara huruf Z ( Z Method)
3. Cara setengah lingkaran (Half Circle Method)
4 .Cara serempat lingkaran (Quarter Circle Method)

Identifikasi kuman
Setelah menemukan lapangan pandang pertama, pindahlah ke
lapangan pandang berikutnya. Periksalah tiap apusann sekitar 100
lapangan pandang. BTA akan nampak sebagai batang merah
dengan latar belakang biru. Bentuknya dapat lurus atau
melengkung, dan warna merah dapat merata atau homogen
(solid) atau tidak rata (fragmanted dan granular). Kelompok basil
disebut sebagai globi. Basil yang solid menandakan adanya
mikroorganisme yang hidup dan dapat dengan mudah terlihat
pada pasien baru yang belum diobati atau pasien relaps

- Hasil pembacaan sediaan apus cukup
dinyatakan negatif (-) atau positif (+) saja.
- Bagi petugas Puskesmas/Kabupaten/Provinsi
yang sudah mampu, dapat menghitung derajat
positifnya sesuai indeks bakteri

PEMERIKSAAN PENUNJANG LAIN
Pemeriksaan penunjang lain dapat dilakukan di rumah
sakit rujukan yang memiliki fasilitas terkait. Pemeriksaan
tersebut, antara lain :
1. Histopatologi
2. Serologis
3. Polimerase Chain Reaction (PCR
