4. Cochrane review Main results
We included 28 trials, involving 5855 participants, comparing
stroke unit care with an alternative service.
More-organised care was consistently associated with improved
outcomes.
Twenty-one trials (3994 participants) compared stroke unit care
with care provided in general wards.
5. Cochrane review Main results
Stroke unit care showed reductions:
1/ in the odds of death recorded at final (median one year)
follow-up
(odds ratio (OR) 0.81, 95% Confidence Interval (CI) 0.69 to 0.94; P = 0.005)
2/ the odds of death or institutionalised care
(OR 0.78, 95% CI 0.68 to 0.89; P = 0.0003)
3/ the odds of death or dependency
(OR 0.79, 95% CI 0.68 to 0.90; P = 0.0007).
6. Cochrane review Main results
Sensitivity analyses indicated that the observed benefits
remained when the analysis was restricted to securely
randomised trials that used unequivocally blinded outcome
assessment with a fixed period of follow-up.
Outcomes were independent of patient age, sex, initial stroke
severity or stroke type, and appeared to be better in stroke units
based in a discrete ward.
There was no indication that organised stroke unit care resulted
in a longer hospital stay.
7. Cochrane review Main results
Authors’ conclusions
Stroke patients who receive organised inpatient care in a stroke
unit are more likely to be alive, independent, and living at home
one year after the stroke.
The benefits were most apparent in units based in a discrete
ward.
We observed no systematic increase in the length of inpatient
stay.

Behandeling met tPA kan binnen de 4.5 u na het begin van de symptomen gestart
worden. Bij basilaristrombose met ernstige neurologische uitvalstekenen:
overweeg IVtPA tot 6u na onset (diagnose dient bevestigd ad hv. CT angio of MR
angio zo > 4.5u na onset).

Informed consent bekomen van patiënt of verwant

14 % meer kans op gunstig herstel (per 7 patiënten 1 overlijden of ernstige
handicap minder).
3 % kans op overlijden door een bloeding, ten gevolge van de behandeling
Minder kans op overlijden door het infarct.
Totale kans op overlijden is hetzelfde, met of zonder trombolyse.
26. Ischaemisch CVA – iv trombolyse
Absolute contraindicaties
Gebruik van vitamine K-antagonisten en INR >1.7 of PT >15s
Gebruik van dabigatran en aPTT boven normale bovengrens óf laatste inname <4h
Gebruik van rivaroxaban en INR/PT boven normale bovengrens óf laatste inname <4h
Gebruik van heparine in de voorbije 48u en verlengde aPTT, gebruik van
therapeutische dosis heparine (eg LMW heparine 80 mg SC) in voorbije 12u (indien
LMW heparine met gift van 2x per dag) of 24u (indien product met 1 gift per dag, eg
Fraxodi)
Gekende bloedplaatjesdaling < 100 000/mm3
Majeure chirurgie of majeur trauma in de voorbije 14 dagen

Arteriële punctie op niet comprimeerbare plaats of LP in de voorbije 7 dagen
27. Ischaemisch CVA – iv trombolyse
Relatieve contraindicaties
Systolische BD >185 mm Hg of diastolische BD >110 mm Hg en agressieve
therapie noodzakelijk om bloeddruk onder deze waarden te krijgen.
Nauwelijks invaliderende neurologische afwijkingen (geïsoleerde facialisparese,
geïsoleerde sensibiliteitsstoornis of geïsoleerde dysartrie, minimale zwakte)
Glucose < 50 mg/dL of > 400 mg/dL;
CVA of contusio cerebri in de voorbije 6 weken.
Recente TIA vormt geen contraindicatie (tenzij beeldvorming recent infarct toont).
Gastro-intestinale of urinaire bloeding in de voorbije 14 dagen
Recent acuut myocard infarct in de voorbije 3 weken (risico van tamponnade).
Bij simultaan hartinfarct en herseninfarct is tPA WEL toegelaten. Bij twijfel,
bespreek met cardioloog.
Epileptisch insult bij ontstaan van symptomen
Leeftijd < 18 jaar, bovengrens?

Trombolyse – parameterregistratie
Opname op IZA/ Stroke unit:
Frequente controle van pols, BD, bewustzijn:
Van 0 tot 2 uur: iedere 15 min.
Van 2 tot 8 uur: iedere 30 min.
Van 8 tot 24 uur: ieder uur.
Bedrust en niets per os gedurende 24 uur.
Controle CT hersenen na 24 uur.

Trombolyse – behandelprotocollen
Normaliseren van de bloeddruk vóór toediening IV tPA: ≤185/110mmHg
Indien bloeddruk systolisch ≥ 185mm Hg of diastolisch ≥110 mm (2 x gemeten):
Nicardipine (Rydene)
Contraindicatie: AMI
5 mg/u IV infusie, verhogen met 2.5 mg elke 5 minuten tot SBP <190 mmHg en
DBD <110, maximaal 15 mg/u; Indien BD niet onder controle binnen 30 minuten is
patient geen kandidaat tPA meer. Na bereiken van target BD, mag nicardipine
verlaagd worden naar 3 mg per uur.
Een arteriële lijn is NIET nodig voor nicardipine toediening.
30. Ischaemisch CVA – iv trombolyse
Alternatieven indien contraindicatie voor nicardipine (Rydene):
Labetalol (Trandate)
Contraindicaties: astma, hartfalen, bradycardie of belangrijke conductieblocks
Bolus 10 mg IV mag 2 maal herhaald worden met 20 minuten tussenpauze
Als Systolische BD ≥ 185 mm Hg of diastolische BD ≥110 mm Hg blijft ondanks 2
maal 10 mg IV is patiënt geen kandidaat voor IV tPA
Urapidil (Ebrantyl)
Bolus 10 mg IV mag 2 maal herhaald worden met 20 minuten tussenpauze.
Als Systolische BD ≥ 185 mm Hg of diastolische BD ≥110 mm Hg blijft ondanks 2
maal 10 mg IV is patiënt geen kandidaat voor IV tPA
31. Ischaemisch CVA – iv trombolyse
Beleid bij hypertensie tijdens en na infusie van tPA (>185/110mmHg)
CAVE: Een intracraniële bloeding kan zich presenteren met een plotse
bloeddrukstijging.
Bloeddrukmonitoring na start IV tPA:
0-2u: elke 15min
2-8u: elke 30 min
8-24u: elk uur
>185/110mmHg: Nicardipine (Rydene) 5 mg/h IV infusie met aanpassing met 2.5
mg/h elke 5 min tot een maximum van 15 mg/h. Bij bereiken van target BD,
nicardipine brengen naar 3 mg/u
Een arteriële lijn is niet nodig voor toediening nicardipine.
32. Ischaemisch CVA – iv trombolyse
Behandeling van anaphylaxis op het trombolyticum
Mild en transiënt orolinguaal angio-oedeem (in 5%), meestal contralateraal aan
ischemische hemisfeer: conservatief beleid

Behandeling bij hyperglycemie
Actrapid-drip IV wordt gestart bij een glycemie boven 180 mg/dl (cfr QASC-studie)

DVT/PE preventie
Steunkousen ter preventie van DVT.
Ischemisch CVA: fraxiparine 0.4ml/d
Anti-epileptica
Er is geen indicatie voor de profylactische toediening van anti-epileptica.
Klinische aanvallen worden behandeld (met bv Keppra, Depakine of Lamictal).
Overweeg afbouw van therapie na 30 dagen; bij recidief insult is chronische
behandeling geïndiceerd.
37. Ischemisch CVA – Trombectomie
Protocol UZA
Indien patiënt voldoet aan criteria voor IV rtPA: Eerst IV rtPA (standaarddosis) te
starten in verwijzend centrum + nadien verwijzing voor endovasculaire therapie.
Indien contra-indicatie voor IV rtPA: Onmiddellijke verwijzing voor endovasculaire
therapie.
Mogelijk indicaties endovasculaire therapie:
Voor alle patiënten: goede premorbide status (mRS < 3) en levensverwachting (> 6
maanden).

ernstig hartfalen, een aortadissectie of een hypertensieve encefalopathie. Een

and unbound apixaban concentration was reduced by 9.3 ng per milliliter versus
1.9 ng per milliliter (P<0.001); thrombin generation was fully restored in 100%
versus 11% of the participants (P<0.001) within 2 to 5 minutes. Among the
rivaroxaban-treated participants, anti–factor Xa activity was reduced by 92%
among those who received an andexanet bolus (27 participants), as compared with
18% among those who received placebo (14 participants) (P<0.001), and unbound
rivaroxaban concentration was reduced by 23.4 ng per milliliter versus 4.2 ng per
milliliter (P<0.001); thrombin generation was fully restored in 96% versus 7% of the
participants (P<0.001). These effects were sustained when andexanet was
administered as a bolus plus an infusion. In a subgroup of participants, transient
increases in levels of d-dimer and prothrombin fragments 1 and 2 were observed,
which resolved within 24 to 72 hours.

VKAs were a major advance in the field, offering much-needed anticoagulation protection for more than 60 years
VKAs are effective drugs for thromboprophylaxis, reducing the risk of stroke and thromboembolism in patients with AF by two-thirds.1,2
However, the use of VKAs is limited by their narrow therapeutic window (2.0–3.0 INR), slow onset and offset of action, unpredictable pharmacokinetics and pharmacodynamics (influenced by drug–drug interactions, diet, and genetic factors), and the need for regular monitoring and dose adjustment.1,2

The NOACs were developed not only to advance patient care beyond what was possible with warfarin, but also to provide anticoagulation therapy options that are both safer and more convenient.
When we look at the collective data from all of the NOAC trials, we see a 19% reduction in total stroke and SE, and more than halving in the risk of haemorrhagic stroke. There was also a 10% reduction in all-cause mortality, and again a more than halving in the risk of any type of intracranial bleeding. These benefits of NOACs over warfarin are accompanied by no overall difference in the risk of major bleeding.

Data from a randomized, double-blind, placebo-controlled trial of 145 healthy male volunteers.
Volunteers were pretreated with dabigatran followed by infusion of idarucizumab or placebo. The figure shows clotting times assessed by the dTT assay; dosing with dabigatran prolonged clotting times, as shown by the dark blue line on the graph.
Upon infusion of a dose of idarucizumab, we see immediate, complete, and sustained reversal of anticoagulation, as shown by the dramatic drop of the red line on the graph. Mean clotting times were reversed to baseline immediately after end of the injection and remained low for the duration of observation. This effect was consistent for both the 2 g and 4 g doses.
The same results were also demonstrated with a 5 g dose of idarucizumab in a second study involving middle-aged (age 45–64 years) and elderly (age 65–80 years) men and women, and in individuals with mild (CrCl 60–90 mL/min) and moderate (CrCl 30–60 mL/min) renal impairment.2
There were no reports of any serious adverse effects in any volunteers exposed to idarucizumab.

The comparable number of patients with ICH and gastrointestinal (GI) bleeds in RE-VERSE AD™ reflects the enrolment criteria that only patients who require urgent reversal of anticoagulation due to a critical or overt bleeding event are to be enrolled.
Every ICH is life-threatening and, therefore, likely to be included in REVERSE AD™. In contrast, not every GI bleed is critical enough to warrant idarucizumab treatment. Most GI bleeds can mainly be managed with standard care and local intervention.
In conclusion, the results of RE-VERSE AD™ have so far confirmed that idarucizumab provides immediate and complete reversal of dabigatran anticoagulation in clinical practice.
In this cohort of elderly dabigatran-treated patients with multiple underlying illnesses who presented with life-threatening emergencies, administration of idarucizumab 5 g provided immediate and complete pharmacological reversal of anticoagulation as demonstrated by the dTT or ECT assays in up to 98% of patients.
The secondary clinical endpoints confirmed the effective reversal of anticoagulation, with normal intraoperative haemostasis reported in 92% of patients undergoing procedures and cessation of bleeding within an average of 12 hours for patients presenting with uncontrolled bleeds.
Assessment of cessation of bleeding was subjective and based upon factors that could be observed or measured by the investigator. Sometimes tests (e.g. CT scans or MRIs) were not repeated for several days.

In the absence of a control group, such events are difficult to distinguish from prothrombotic events that might be a direct result of stimulation of the coagulation cascade by the reversal agent. The absence of a temporal proximity between treatment and event (earliest event 48 hours after idarucizumab) and the fact that none of these patients were anticoagulated at the time of event makes prothrombotic effects very unlikely.
