3. A : can talk, no c-spine tenderness
B : trachea in midline, clear equal breath sound both lungs,
CCT neg.
C : BP 130/77 mmHg, P 60 bpm, no active bleeding, PCT
neg.
D : E4V5M6, pupils 2 mm RTLBE
E : Rt. Shoulder flattened with marked tenderness, humeral
head prominent, Rt. Arm slightly abduction with external
rotation, limit ROM all direction due to pain

5. A : no Hx. of food and drug allergy
M : no current medication
P : no underlying disease
L : 20.00 น. (at ER 01.30 น.)
E : 3 ชั่วโมงก่อน ขับ MC ชนสุนัข ล้มไหล่ขวากระแทกพื้น ปวดมาก PS 7/10 ยก
แขนขวาไม่ขึ้น ไม่มีบาดแผล ไม่ชา ไม่เคยไหล่หลุดมาก่อน สวมหมวกกันน็อค ศีรษะไม่กระแทก
พื้น ไม่สลบ จาเหตุการณ์ได้

6. Rt. Shoulder flattened with marked tenderness,
can palpable head of humerus
Rt. Arm slightly abduction with external rotation,
can contract deltoid, limit ROM all direction due to
pain
Rt. Elbow and wrist can move, ulnar and radial
pulse 2+, capillary refill < 2 secs, intact pinprick
sensation

Closed reduction
On interlocking sling
F/U 2 wks

16. Mechanism of injury :
- direct blow from posterior aspect of shoulder
- combined shoulder abduction, extension and external rotation
!!! Tear ligament around shoulder
!!! Axillary Nerve injury
Anterior fullness
Flatten deltoid
Duga’s test
Ruler’s test
Imaging : AP, transcapular

24. Immobilization - interlocking sling
- age < 30-35 yrs : 3 wks
- age > 35 yrs : 1 wk
Isometric exercise
Early ROM exercise

25. Old dislocation or onset > 2-3 wks
Greater tuberosity fracture and fail after
reduction
Glenoid rim fracture
