11. Management (in this case)
• On posterior cylindrical slab
• Advice non weight bearing ambulation with axillary crutches
• D/C นัด 2wk F/U film knee AP, lateral
• Pain control: Paracetamol

14. Fracture in children
• More elasticity -> green stick fracture, torus fracture, plastic
deformity
• Thick and though periosteum -> help splinting around fracture site
• Epiphyseal plate -> more fragile, harder to detect fracture
• Mostly conservative
• Complication -> growth arrest, less non-union or malunion, less
stiffness
• Better remodeling
16. Salter-Harris Classification
• Type I
• More common in younger children
• Separation of epiphysis and metaphysis
• No growth disturbance
• Splinting / casting for 3 weeks
17. Salter-Harris Classification
• Type II
• Most common type
• Epiphysis separation + metaphysis fracture
• Thurston Holland’s sign
• Minimal growth disturbance
• Close reduction and splinting / casting for 3-4 weeks
18. Salter-Harris Classification
• Type III & IV
• Intra-articular fracture with (IV) or without (III) metaphysis
involvement
• likely to cause growth disturbance
• Close / open reduction (internal fixation if needed)
• Splinting / casting
19. Salter-Harris Classification
• Type V
• Axial crush injury
• Mostly missed
• Growth disturbance almost all cases
• Result in limb length discrepancy or angular deformity
