Power Case Study
Chris is the manager of the telemetry unit. On this particular unit, a problem exists with the staff and their
poor morale. The morale issue has a variety of reasons, including a very busy workload, demanding
physicians who are not as collegial as they could be, and a high turnover rate of new people who leave
within a year. Almost half of the staff has been employed on the telemetry unit for more than 10 years. The
new people who are leaving are not necessarily new to nursing, however, because a registered nurse must
have a minimum of 1 year of experience to apply for a position on the unit.

work environment in health care facilities. Nursing–sensitive indicators are a quality improvement measure
used to determine how the quality of nursing care is evaluated in hospitals and other healthcare facilities.
Nurse sensitive indicators allow healthcare facilities to evaluate how the care they are providing is following

My interviewee obtained her Master's in Informatics at the University of Phoenix in 2013. Transitioning into
the Master's program was seamless since this school was where she obtained her BSN. The online route was
most efficient and that is the route she pursued. The University of Phoenix Master of Science in nursing
degree program is accredited by the Commission of Collegiate Nursing Education (University of Phoenix,
2014). The goal of this program is to equip nurses to function in leadership roles in a variety of settings.
Additionally, nursing theories and evidence–based practices are fundamental guidelines in this program that
will help shape the future of nursing thus impacting the health care system (University of Phoenix, 2014).
More specifically, the informatics program afford students to gain knowledge in data management and
information technology in regard to the health care setting (University of Phoenix, 2014).
According to my interviewee, the Master's graduate program as compared to the undergraduate program was

The group was led by myself and included members of a multidisciplinary team of all units involved in the
telemetry process including the managers, nurses, monitor technicians and clinical engineers. This group
selection afforded us an opportunity to share successful approaches, discuss barriers and create specific
strategies acknowledging variation and where in the process steps it could be eliminated. Multiple barriers in
the process were readily identified and the team would meet on a bi–weekly for several months to discuss
each process step and decided on countermeasures. An example of one risk recognized was an interruption in
the ECG data recorded between the emergency department and the data recorded in the telemetry units. This
step revealed a possible loss of data and possible incorrect patient identification of data upon arrival to the
unit. The solution was to involve clinical engineering to create a seamless record for recording the cardiac
ECG rhythms from the emergency room to the inpatient bed. The team scored each FMEA step, agreed upon
the risk score threshold for each actionable items, and finalized the approach. Measurement of post FMEA
showed a reduction in events an important part of evaluating the success of the implemented steps. The

This week in Telemetry I mainly got to observe a patient on the first day and talk to a telemetry nurse the
second day. On the first day I was assigned to a nurse named Missy. She was not very busy at the time so I
got to observe a man on dialysis. The dialysis nurse's name was Chris, and he told me all about what dialysis
is. Dialysis is a machine used when someone's kidney is not working. Since your kidney is what cleans your
blood, that is one of the things that the machine has to do for you. The nurse also told me that the machine
removes fluids and can add electrolytes. He said that a patient is on average on dialysis for anywhere from
two to four and a half hours. The fistula is where the machine attaches to your body and is where a vein and
an artery are sewn together. Chris was explaining to me what the fistula feels like, he said that is feels like a
pulse, but much stronger. It is not that the rhythm is any different, the pulse just feels stronger. The patient
told me to grab a glove and feel his fistula but the nurse told me I don't really need to because I would have
to put a gown and gloves on just to feel the pulse. The next day we were in telemetry we got to talk to a
telemetry nurse, Matthew. He explained to us a lot about the different floors in the hospital and how
telemetry is mostly for people with heart or respiratory issues, which is why they have to be hooked up to the

Nurses are so inundated with various buzzes and beeps from numerous alarms that they sometimes are
desensitized, and nurses to alarms fail to respond to alarms in a timely manner if they respond at all. This is
called alarm fatigue. Nuisance alarms are often called false positive alarms and are one of the leading causes

The mystery of migration patterns of Mallard Ducks is fascinating. Understanding a migratory pattern and
reasons for migration, can help me learn the behavior of a certain species of a duck and help me in knowing
when and where to hunt. Mallard ducks migrate south every year from their breeding grounds. The reason
they do this is the shortage of food in the winter months. As they make their way south, they use many
different paths such as flyways.

This case is purely about the legal and ethical implications of nurse T.C and her nursing manager of
telemetry unit, nurse F.J. This nurse T.C has worked in a highly stressful environment before but her
behavior has only recently deteriorated and become emotional with the occasional outbursts and
uncontrollable crying. Her behavior put the legal issue of non–maleficence at risk for her patients as it would
cause emotional stress on them. However, the legal issue that really matters is her case is that of defamation
of her character as well as disclosure of her private and confidential mental state information by F.J to other
nurse managers not in the unit she worked and to even the subordinates. F.J was only justified to release the

The other ethical issue involved justice, specifically being fair to her being new to the profession, which
justifies her nervous behavior. This would have called for a lenient action being taken on her instead of the
conclusion made by F.J who was a nurse manager and who had experience with such cases in the past. The

He said that a patient is on average on dialysis for anywhere from two to four and a half hours. The fistula is where the machine attaches to your body and is where a vein and an artery are sewn together. Chris was explaining to me what the fistula feels like, he said that is feels like a pulse, but much stronger. It is not that the rhythm is any different, the pulse just feels stronger. The patient told me to grab a glove and feel his fistula but the nurse told me I don't really need to because I would have to put a gown and gloves on just to feel the pulse.
