The identified barriers are financial cost, physician and organizational resistance due to low
computer literacy skills and disturbance of workflow caused by CPOE systems. The resistance can
be overcomed by strategies such as strong leadership, providing trainings, addressing workflow

The Electronic Medical Records Is A Complex Entity
Healthcare is a complex entity that encompasses a variety of specialties necessary toward meeting
the needs of patient seeking clinical services. There are multiple communications necessary to
efficiently meet patient needs. For many years detailed documentation, progress notes, specialty
consults, and physician orders have been hand written. The legibility of this documentation was
often illegible, and difficult to decipher, which resulted in clarification orders and often delays. The
electronic medical record was introduced approximately 50 years ago with an ultimate goal of
compiling healthcare information for immediate and future reference (Keller, 2016). Since the

Successfully implementing the electronic medical record, requires a great deal of research to ensure

The medical or healthcare record has multiple purposes. First, it is repository of documentation of
the actions taken in patient management, the documentation of a patient's progress, and therefore a
mean's to augment one's memory and knowledge as well as to provide meaningful medical
information to other practitioners should the patient transfer to a new provider or should the original
provider be unavailable for some reason. The health electronic health record system that is currently
in use at my facility is Cerner. Cerner retained the top spot for community hospitals of 101–250 beds
for the same time period; it also came out the best for hospital systems. I will discuss the role of this
system as it relates to memory, computation decision support and collaboration. Although there are
some minor issues I have noticed throughout the years I find navigation through this system is
secure and easy to use. Cerner has the capability to display available history and demographics.
Each time a patient is admitted into the hospital their records are maintained and are able to be
viewed by any authorized staff member. Their charts are only allowed to be accessed if the patient
was admitted within the last 30 days at any Methodist Health Care hospital. All information
documented from other Methodist facilities will be visible to current providers. Chart Search

INTRODUCTION An inappropriate medication use that can harm a patient or no therapeutic effect
is considered as the pharmaceutical error, as according to the Malaysian National Patient Safety
Council. The pharmaceutical error can be caused by the mistakes done by physicians, pharmacists,
healthcare professionals, nurses or patients itself in either diagnosis or administration of drugs. The
pharmaceutical error is an event that can be prevented. Generally, every step in treating of a patient,
including diagnosis, dispensing of drugs and administration, should be done accurately without

basic to a more integrated concept of HI. In the 1990's when I started nursing, I associated nursing

The Importance Of Patient Care Technology In Nursing
Patient care technology is becoming increasing helpful for the nursing world. Throughout the years,
technology has transformed the way information is delivered through nursing care. (Hughes, 2014)
Most of these new technology machines help reduce the errors made through nursing and decrease
the amount of assessment mistakes made by nurses. Before pulse oximetry, nursing assessments had
to include temperature and color of the patient's skin and follow up with an ABG to confirm simple
results. (Powell–Cope, 2014) Since the invention of the pulse oximetry, nurses now can put a strap
on a finger and a number will appear reading the oxygen level in the patients blood. Barcode
administration is a strong example of how communication devices have help to increase patient
safety. (Powell–Cope, 2014) Before barcode scanning, nurses made duplicate patient arm bands and
one was left at the medication cart. The nurse would scan one bar code at the medication cart and
then one on the patients arm. (Powell–Cope, 2014) The concern with this was the face to face,
positive patent identification was bypassed and increased patient medication errors. With arm bands
being placed on the patients arm and medication administration consist of selecting the patient in the
Pyxis, then scanning the patient at the bedside before administrating the medication has decreased
our medication errors. Alarms are to believe have helped decreased patient falls, increase
satisfaction scores and maintain medication regimen. (Powell–Cope, 2014) Although they have
decreased, increased and maintained some research has proven "alarm fatigue" has increased.
(Powell–Cope, 2014) There are numerous alarms used in the medical field, from call bells to IV

Meadow Hills Hospital is a two hundred bed hospital with a large Medical Staff that serves a
population of three hundred thousand. It is one of four similar hospitals in the region. They do not
currently have an EHR, but they're financial systems are well developed. They have a good amount
of money in reserve to spend on any solution they feel is needed. Historically, their IT function has
