This study examined the association between periodontitis and rheumatoid arthritis. It found that periodontitis was more frequent, severe, and generalized in rheumatoid arthritis patients compared to controls. Several rheumatoid arthritis disease markers like CDAI, anti-CCP titer, and CRP levels were higher in rheumatoid arthritis patients with periodontitis compared to those without. The results suggest periodontitis is more common and severe in rheumatoid arthritis, and may be associated with rheumatoid arthritis disease activity and severity.

This study aimed to identify factors associated with the development of vitiligo in patients with halo nevus (HN). The researchers performed a retrospective study of 212 patients with HN, 101 of whom also had vitiligo (HNV group). This study examined the prevalence of microvascular and macrovascular complications in 1200 patients with type 2 diabetes in India. Retinopathy was found in 13.5% of patients, nephropathy in 26%, neuropathy in 31.5%, cardiovascular disease in 19.1%, and peripheral vascular disease in 9.75%. Logistic regression revealed that older age, longer diabetes duration, and hypertension were significantly associated with all complications. Poor glycemic control was associated with higher rates of nephropathy and retinopathy. The study highlights the high prevalence of complications, especially nephropathy and neuropathy, in this population.
This study analyzed burn wound swabs collected from 187 hospitalized patients over 3 years to identify aerobic bacterial pathogens and their antibiotic resistance patterns. The most common isolate was Pseudomonas aeruginosa (49.4% of isolates), followed by Staphylococcus aureus (22.2%) and various Enterobacteriaceae species. P. aeruginosa demonstrated high resistance to many commonly used antibiotics but was most susceptible to piperacillin/tazobactam and imipenem. 59% of S. aureus isolates were methicillin-resistant but all were susceptible to vancomycin and linezolid. Background : Sepsis is life-threatening organ dysfunction caused by a dysregulated host response to infection which is one of the most important cause of mortality & morbidity in critically ill patients. In this study clinical profiles of the sepsis patients admitted in ICU in this part of India have been evaluated. Methods & Materials: This prospective hospital based observational study was undertaken in the department of Emergency Medicine ICU of Gauhati Medical College & Hospital, over a period of one year from August 2014 to July 2015 after obtaining institutional ethical committee clearance.
RESULTS: Clinical profiles of 50sepsis patients, with male preponderance (56%) & mortality rate 36% were studied. Mean age was 48.36 years (SD ±17.16). fever & tachycardia were present in all patients. 30 patients (60%) required ventilatory support, 28 patients (56%) required inotropic support, 10 patients (20%) required dialysis. Gram negative bacteria were found to be the predominant pathogens associated with sepsis(73.4%) where most common organism responsible was Klebsiella (36.8%). Conclusion : assessment of clinical signs & initial serological & radiological investigations are of utmost importance to detect more critically ill patients as early as possible to intervene earlier for saving the life of the sepsis patients.

ABSTRACT : Despite the concerted effort to detect and treat TB, there are still poor treatment outcomes in a significant number of the patients. These poor treatment outcomes have been significantly linked to poor adherence to TB treatment. Therefore, a cross sectional descriptive study was conducted in Kisumu East District to establish the relationship between health care system factors and TB treatment adherence among patients aged above 18 years attending TB clinics in Kisumu East District, in Western Kenya. A total sample of 250 respondents was surveyed. An interviewer administered structured questionnaire was used to collect data from the respondents on the social, demographic aspects of the patients and structural aspects of TB care. The data was analyzed using descriptive statistics for socio-demographic variables and bivariate analysis to determine the health care system factors that significantly predicted treatment adherence. P values, Odds Ratios with 95% confidence interval (CI) were used to demonstrate significance of association between the health system related predictors and adherence. Significance was assumed at P value ≤0.05. Behaviour of the health care workers (OR: 3.6; 95% CI1.1-12.1; P=0.031) and waiting time (OR: 7; 95%CI: 3-18; P<0.001) were the significant determinants of adherence related to health care set up. Health care system setup has a number of immediate modifiable predictors of adherence like waiting time and staff behaviour. It is important to establish the key predictors of adherence that are linked to health care system for quality TB treatment and care services in every TB care setting.
Tuberculosis affected mankind for over 5000 yrs and continue to be a major public health problem. So to generate hypothesis about tuberculosis this study was conducted to study the profile of tuberculosis cases attending at DTC Jaipur. A record based cross-sectional study of tuberculosis patients attended at District Tuberculosis Centre, Jaipur were taken. Chi-square test was used to find out association. It was found in this study that Cure Rate significantly varied with type of case, age and sex of case. It was observed highest in cat.III and below 20 yrs of age. Defaulters were max. in Relapse retreated cases and in elderly (>60 yrs.) cases whereas it was not affected significantly with age . Failure Rate was highest in category I cases and significantly varied with age and sex. About 3/4th of total deaths were observed in Failure and Relapse retreated cases. Case Fatality Rate (CFR) showed not significant variation with sex. So it can be concluded with this study that maximally affected age group with tuberculosis is either children or 40-60 years with male predominance. Category III cases are more easier to cured and Category II cases are most difficult to cured. Cure rate was lesser in 40-60 years of age group and males. Defaulters and CFR were more in category II, 40-60 year of age group and males. And Failure Rate was more in category I, 40-60 year of age group and males

Abstract— Season seems to have its role in wound infection which is the second commonest nosocomial infection and most troublesome disorder of wound healing. This study was carried out on 100 post-operative cases of Surgical Unit 1st of General Surgery Department of Sawai Man Singh Hospital, Jaipur (Rajasthan) India in years 2014. This study aimed to find out the seasonal trend in Post-operative wound infections (PSI). After interview of these, swab from post-operative wound was taken and sent for culture and sensitivity test in Microbiology. Results were inferred by Chi-square test. In this study, post-operative wound infection rate was found 21%. In majority of cases, causative agent found in post-operative infected wound was Staphylococci (90.48%) followed with Streptococci, E. Coli, Klebsella and Pseudomonas. Maximum cases were found in April followed by March, January and none was found in other months but this variation was not found significant.

This study examined the prevalence of microvascular and macrovascular complications in 1200 patients with type 2 diabetes in India. Retinopathy was found in 13.5% of patients, nephropathy in 26%, neuropathy in 31.5%, cardiovascular disease in 19.1%, and peripheral vascular disease in 9.75%. Logistic regression revealed that older age, longer diabetes duration, and hypertension were significantly associated with all complications. Poor glycemic control was associated with higher rates of nephropathy and retinopathy. The study highlights the high prevalence of complications, especially nephropathy and neuropathy, in this population.
This study analyzed burn wound swabs collected from 187 hospitalized patients over 3 years to identify aerobic bacterial pathogens and their antibiotic resistance patterns. The most common isolate was Pseudomonas aeruginosa (49.4% of isolates), followed by Staphylococcus aureus (22.2%) and various Enterobacteriaceae species. P. aeruginosa demonstrated high resistance to many commonly used antibiotics but was most susceptible to piperacillin/tazobactam and imipenem. 59% of S. aureus isolates were methicillin-resistant but all were susceptible to vancomycin and linezolid. Background : Sepsis is life-threatening organ dysfunction caused by a dysregulated host response to infection which is one of the most important cause of mortality & morbidity in critically ill patients. In this study clinical profiles of the sepsis patients admitted in ICU in this part of India have been evaluated. Methods & Materials: This prospective hospital based observational study was undertaken in the department of Emergency Medicine ICU of Gauhati Medical College & Hospital, over a period of one year from August 2014 to July 2015 after obtaining institutional ethical committee clearance.
RESULTS: Clinical profiles of 50sepsis patients, with male preponderance (56%) & mortality rate 36% were studied. Mean age was 48.36 years (SD ±17.16). fever & tachycardia were present in all patients. 30 patients (60%) required ventilatory support, 28 patients (56%) required inotropic support, 10 patients (20%) required dialysis. Gram negative bacteria were found to be the predominant pathogens associated with sepsis(73.4%) where most common organism responsible was Klebsiella (36.8%). Conclusion : assessment of clinical signs & initial serological & radiological investigations are of utmost importance to detect more critically ill patients as early as possible to intervene earlier for saving the life of the sepsis patients.
Relationship between Health Care System Setup and Adherence To Tuberculosis T...QUESTJOURNAL
ABSTRACT : Despite the concerted effort to detect and treat TB, there are still poor treatment outcomes in a significant number of the patients. These poor treatment outcomes have been significantly linked to poor adherence to TB treatment. Therefore, a cross sectional descriptive study was conducted in Kisumu East District to establish the relationship between health care system factors and TB treatment adherence among patients aged above 18 years attending TB clinics in Kisumu East District, in Western Kenya. A total sample of 250 respondents was surveyed. An interviewer administered structured questionnaire was used to collect data from the respondents on the social, demographic aspects of the patients and structural aspects of TB care. The data was analyzed using descriptive statistics for socio-demographic variables and bivariate analysis to determine the health care system factors that significantly predicted treatment adherence. P values, Odds Ratios with 95% confidence interval (CI) were used to demonstrate significance of association between the health system related predictors and adherence. Significance was assumed at P value ≤0.05. Behaviour of the health care workers (OR: 3.6; 95% CI1.1-12.1; P=0.031) and waiting time (OR: 7; 95%CI: 3-18; P<0.001) were the significant determinants of adherence related to health care set up. Health care system setup has a number of immediate modifiable predictors of adherence like waiting time and staff behaviour. It is important to establish the key predictors of adherence that are linked to health care system for quality TB treatment and care services in every TB care setting.
Tuberculosis affected mankind for over 5000 yrs and continue to be a major public health problem. So to generate hypothesis about tuberculosis this study was conducted to study the profile of tuberculosis cases attending at DTC Jaipur. A record based cross-sectional study of tuberculosis patients attended at District Tuberculosis Centre, Jaipur were taken. Chi-square test was used to find out association. It was found in this study that Cure Rate significantly varied with type of case, age and sex of case. It was observed highest in cat.III and below 20 yrs of age. Defaulters were max. in Relapse retreated cases and in elderly (>60 yrs.) cases whereas it was not affected significantly with age . Failure Rate was highest in category I cases and significantly varied with age and sex. About 3/4th of total deaths were observed in Failure and Relapse retreated cases. Case Fatality Rate (CFR) showed not significant variation with sex. So it can be concluded with this study that maximally affected age group with tuberculosis is either children or 40-60 years with male predominance. Category III cases are more easier to cured and Category II cases are most difficult to cured. Cure rate was lesser in 40-60 years of age group and males. Defaulters and CFR were more in category II, 40-60 year of age group and males. And Failure Rate was more in category I, 40-60 year of age group and males

This study analyzed data from a clinical trial of once-weekly teriparatide therapy for osteoporosis. It found that two thresholds - the least significant change (LSC) criterion and achieving a bone mineral density above -2.5 standard deviations from the young adult mean - were useful indicators of treatment success. Using these thresholds, the study found treatment success rates of 79.2% for the lumbar spine and 44.1% for the femoral neck at 72 weeks of teriparatide therapy. Achieving the treatment success thresholds, especially at the femoral neck, was associated with a lower incidence of vertebral fractures.

Abstract— Season seems to have its role in wound infection which is the second commonest nosocomial infection and most troublesome disorder of wound healing. This study was carried out on 100 post-operative cases of Surgical Unit 1st of General Surgery Department of Sawai Man Singh Hospital, Jaipur (Rajasthan) India in years 2014. This study aimed to find out the seasonal trend in Post-operative wound infections (PSI). After interview of these, swab from post-operative wound was taken and sent for culture and sensitivity test in Microbiology. Results were inferred by Chi-square test. In this study, post-operative wound infection rate was found 21%. In majority of cases, causative agent found in post-operative infected wound was Staphylococci (90.48%) followed with Streptococci, E. Coli, Klebsella and Pseudomonas. Maximum cases were found in April followed by March, January and none was found in other months but this variation was not found significant.

Presentation highlights the needs to recognize gout as one of the rheumatic conditions that put patients at risk for developing CV disease.
Two-year outcomes were studied in 111 patients with infected total knee arthroplasty treated with debridement, antibiotics, and implant retention. Sixty-one percent of implants were retained at 2 years. Arthrotomy and polyethylene liner exchange were strongly associated with improved implant retention rates compared to arthroscopy without liner exchange. Staphylococcus aureus infections had higher failure rates than non-S. aureus infections, but outcomes improved with the addition of rifampin for staphylococcal infections. Culture-negative cases had a 78% implant retention rate. No differences in outcomes were seen based on the age of the implant at the time of debridement.

RHEUMATOID ARTHRITIS PATIENTS:

4. Rheumatoid arthritis (RA), a chronic inflammatory
autoimmune disease characterized by persistent synovitis,
systemic inflammation, production of autoantibodies, &
bone destruction; preferentially involving peripheral joints
( Lee et al., 2015).
A remarkable increase in the prevalence of moderate to
severe periodontitis (PD) in RA patients has been
mentioned ( Cochran, 2008).
PD is a chronic infectious disease caused by Gram-negative
anaerobic bacteria, affecting the tissues of protection and
support of the tooth as gums, periodontal ligament,
cementum & alveolar bone ( Abroa et al., 2016).
5. For periodontal tissues in a healthy state, a
steady equilibrium exists between tissue
destruction and repair.
Periodontal destruction is initiated and
progressed by specific periodontal
microorganism that colonize in plaque
biofilm.

13. Possible relationship sharing inflammatory
disease pathways
Sharing common risk factors such as:
smoking, oral hygienic difficulties from
peripheral joint dysfunction and tempro-
mandibular disease, and effects of certain
drugs prescribed
Xerostomia from Sjogren disease
14. The importance of peptide citrullination
as an etio-pathologic al event in RA might
play a role especially, when focusing on the
expression and function of PAD enzymes that
can lead to development of anticitrulinatted
peptide antibodies(Sokolove et al., 2012) .
The discovery of a major bacterial species
involved in the development and
propagation of periodontal disease having a
PAD capability of citrullination (McGraw et
al., 1999) (Bartold et al., 2010).
15. An interrelationship between RA and PD
has been suggested due to their common
pathogenic mechanisms: Protein
carbamylation & neutrophil extracellular traps
(NETs) formation have been shown to be
related to autoimmune conditions, including
RA and is associated with PD severity which is
influenced with periodontal treatment.
(Kaneko et al., 2018)
16. METHODS
The study included 60 RA patients & 30 controls. Full
history taken, complete general and musculoskeletal
examination was performed.
Clinical disease activity index (CDAI), modified health
assessment questionnaire (MHAQ), visual analogue scale
(VAS) of pain and modified Larsen score were assessed.
Rheumatoid factor (RF) positivity & anti-CCP titer were
measured. Periodontal examination was performed &
relevant indices calculated.
17. RESULTS
The mean age of the patients was 49.1±13 years and they were 52
females and 8 males. PD was present in 71.7% of patients versus
46.7% in control (p=0.02). PD was predominantly generalized
(p=0.004) and with a moderate-severe degree (p=0.01). Age
(p=0.007), disease duration (p<0.0001), morning stiffness (p=0.01),
CDAI (p<0.0001), MHAQ (p=0.02), CRP (p=0.02), anti-CCP titer
(p=0.01) and methotrexate treatment (p=0.005) were significantly
higher in RA-PD vs RA patients. However, gender, smoking, oral
hygiene, erythrocyte sedimentation rate, RF, anti-CCP positivity and
radiographic scoring were insignificantly different. PD positivity was
96.3%, generalized in 92.6%, moderate (40.7%) and severe (37%) in
early vs late (51.5% ,24.2%, 24.2%, 12.1% respectively) RA patients.
All PD indices were higher in early patients (p≤0.05) while teeth loss
(p=0.03) was higher in late cases. CDAI, VAS , MHAQ and anti-CCP
titer all significantly correlated with PD indices (p<0.05).
23. In the present work, the frequency of PD in control was 46.7%,
among which mild PD was represented in 23.3% and moderate
in 26.7%. This was in agreement with the National Health and
Nutrition Examination Survey (NHANES) in 2009-2010 that
demonstrated the overall rate of PD in the general population
over age of 30 was 47.2%, with moderate PD in 30% and
severe PD in 8.5% (Eke et al., 2012).
The frequency of PD was significantly higher in RA patients;
96.3% in early RA and 24% in late RA. PD degree was
predominantly moderate and severe in RA patients versus the
control (p=0.01).Generalized form of PD was more dominant
in RA patients (p=0.004), and all indices which reflect PD
severity. This was in agreement with a study which examined
anti-P.gingivalis (Pg) to evaluate the oral microbiome in groups
of individuals with early and established RA, compared with
healthy adults (Scher et al., 2012).
24. Another study recorded PD in 80% of patients with early RA
and in 85% of patients with chronic RA vs 40% of controls
documenting the poorer periodontal health in RA patients
(Äyräväinen et al., 2017) . This documentation could be
explained by weakened immune defense in the host due to
RA and increased systemic inflammation which may initiate
or enhance the severity of periodontitis ( Payne et al.,
2015).
Intensity of gingival inflammation varies widely between
individuals following plaque accumulation, suggesting that
susceptibility to disease varies between individuals due to
differences in the inflammatory host response , rather than
being entirely due to differences in the amount and/or
composition of the bacterial plaque ( Trombelli et al., 2004).
25. In the current work CRP positivity was significantly higher
in RA-PD vs RA patients, (p=0.02). These periodontal
lesions were suggested to be the origin for daily
bacteraemia and even raise systemic inflammation
reflected in markers as CRP( Loos ,2005).
A recent study showed that patients with chronic RA with
severe periodontitis had higher CRP values compared with
corresponding early RA participants (Äyräväinen et al.,
2017) .
26. Also in RA the main triggers of articular bone erosion are
synovitis, including the production of pro-inflammatory
cytokines and receptor activator of nuclear factor κB ligand
(RANKL), as well as antibodies directed against citrullinated
proteins, both cytokines and autoantibodies stimulate the
differentiation of bone-resorping osteoclasts, thereby
stimulating local bone resorption (Schett and Gravallese,
2012).
On evaluating the effect of anti-rheumatic medications on
periodontal parameters, low doses corticosteroids were
insignificant. This was in agreement with a recent study, where
no difference in periodontal parameters with corticosteroids
was documented (Äyräväinen et al., 2017).While, another
study reported an association between the use of
corticosteroids and higher levels of CAL and deepened
periodontal pocket depth( Beeraka et al., 2013).
27. The current study found that MTX was significantly
related. Similarly, an increased frequency of MTX-induced
oral mucosal ulcers was demonstrated (Mays et al.,
2012).On the other hand, it has been reported that MTX
and leflunamide had no effect on dental status ( Han and
Raynolds, 2012).
Furthermore, Äyräväinen et al 2017, reported that
biologic therapy do not worsen periodontitis in RA
patients. In fact, there was a trend for improvement in
pocket depths.
28. Smoking was not different in RA patients perhaps, due to
presence of pathway for citrullination process in RA
independent to smoking, although being one of important
extrinsic risk factors in RA ( Chang et al., 2014).
In 2014, Agnihotri R and Gaur S reviewed the link
between RA and PD in the elderly thorough under-
standing the link between the two chronic inflammatory
diseases might be beneficial in rendering better health
care protection in elderly individuals.
29. There is a mutual relation between the disease activity
and functional status in RA with the frequency and
severity of PD, being the first and most recent Egyptian
study done in rheumatoid arthritis patients’.
Dental assessment and attention to oral hygiene
assume an increasingly important part of the clinical
management of the RA patients. Furthermore, closer
attention to oral health in all patients will improve
quality of life and address what is now recognized as an
important RA co-morbidity. Routine periodontal
examination is recommended for all RA patients.
