Le occlusioni venose retiniche sono caratterizzate dall’ostruzione della circolazione di una vena retinica con formazione di emorragie retiniche e possibile formazione di aree ischemiche e/o di edema maculare. La diagnosi è basata sulla clinica sulla fluorangiografia e sull’OCT. Si deve sempre effettuare un inquadramento sistemico del paziente. La terapia oculistica si avvale dell’argon laser retinico e di farmaci intraviterali.
Le occlusioni arteriose retiniche sono caratterizzate dall’ostruzione della circolazione di una arteria retinica con ischemia retinica corrispondente all’area di mancata irrorazione sanguigna. La diagnosi è clinica, si può effettuare una fluorangiografia e un OCT. Si deve sempre effettuare un inquadramento sistemico del paziente. La terapia deve essere tempestiva.

sui campi sportivi tanti ragazzi (si stima che negli
ultimi 10 anni ne sono morti 516 solo in Italia)
cominciamo a dire quali sono le cause più frequenti
di queste morti. Si muore per:
-Cardiomiopatia ischemica acuta (vedi la morte del
calciatore del Livorno PierMario Morosini alcuni
anni fa)

Un altro caso riguarda un ragazzino di 12
anni che mentre giocava a calcio il quale
spesso sveniva. Dopo infiniti
accertamenti è stata effettuata una
scintigrafia del miocardio che ha messo
in evidenza una malattia coronarica
trivascolare. Il ragazzo è stato operatore
di bypass multiplo ed ha continuato
regolarmente la sua vita sportiva

Alcuni anni fa una rivista americana famosa nel
settore ha pubblicato un articolo sugli operai di
Brooklyn che lavorando sotto l’acqua andavano
incontro a queste morti per embolia

9. Tutte le cause di queste morti
sono diagnosticabili e
prevedibili in tempo!
10. LA CARDIOMIOPATIA ISCHEMICA ACUTA
La cardiomiopatia ischemica acuta si diagnostica
con certezza con una scintigrafia del miocardio.
Purtroppo a tutt’oggi si esegue al massimo un
elettrocardiogramma sotto sforzo, un esame che ha
una probabilità del 30-40% di falso positivo o falso
negativo, soprattutto quando la malattia coronarica
è monovascolare, cosa che nei giovani è una delle
più frequenti.
11. ARITMIA GRAVE WOLFF PARKINSON WHITE
L’aritmia grave, W P W, si diagnostica con uno
studio di elettrofisiologia cardiaca. Di tale aritmia,
nella mia carriera, ho visto morire un’infermiera di
24 anni che lavorava nella mia clinica, morta nella
sua casa di notte, perché non si era mai sottoposta
allo studio di elettrofisiologia cardiaca. Stessa fine è
capitata ad una collega anestesista di 40 anni il
giorno che lei entrava come capo servizio
all’ospedale Sant’Andrea di Roma.
12. GLI ANEURISMI CONGENITI
Gli aneurismi congeniti, in particolare quelli
cerebrali, si possono diagnosticare con un esame
angio-RM.
Molto frequenti sono gli aneurismi a livello delle
arterie cerebrali anteriori che sotto una pressione
eccessiva si rompono con conseguenti emorragie.
13. L’EMBOLIA
L’embolia si può prevenire con una semplice
ecocardiografia. L’embolia si verifica in seguito ad
una cardiomiopatia congenita: la più frequente è la
persistenza del forame ovale o la pervietà del setto
interateriale oppure la mancata saldatura di esso.
Tale setto è composto da due foglietti che si
saldano dopo la nascita, ma nel caso in cui manca
questa saldatura, e sotto l’aumento delle pressioni
cardiache a destra, avviene l’inversione del circolo
con le conseguenti embolie dagli esiti spesso
mortali.

GIUSTA CHE SI DOVREBBE FARE
Per concludere direi ad ogni sportivo che intenda
cominciare una qualsiasi attività sportiva agonistica
di cominciare con una visita cardiologica accurata
da uno specialista cardiologo, con tutto il rispetto
per i colleghi medici dello sport.
