Critical care involves highly complex decision making. It is by nature data-intense. Despite the growth of critical care, however, the basic approach of data collection and management has remained largely unchanged over the past 40 years. Large volumes of data are collected from disparate sources and reviewed usually retrospectively; and even that is difficult.
Understanding the dynamics of critical illness requires precisely time-stamped physiologic data (sampled frequently enough to accurately recreate the detail of physiologic waveforms) integrated with clinical context and processed with a wide array of linear and nonlinear analytical tools.

Defecation
Normal defecation begins with movement in the left colon, moving stool toward the anus. When stool reaches the rectum, the distention causes relaxation of the internal sphincter and an awareness of the need to defecate. At the time of defecation, the external sphincter relaxes, and abdominal muscles contract, increasing intrarectal pressure and forcing the stool out
The Valsalva maneuver exerts pressure to expel faeces through a voluntary contraction of the abdominal muscles while maintaining forced expiration against a closed airway. Patients with cardiovascular disease, glaucoma, increased intracranial pressure, or a new surgical wound are at greater risk for cardiac dysrhythmias and elevated blood pressure with the Valsalva maneuver and need to avoid straining to pass the stool.
Normal defecation is painless, resulting in passage of soft, formed stool
CONSTIPATION
Constipation is a symptom, not a disease. Improper diet, reduced fluid intake, lack of exercise, and certain medications can cause constipation. For example, patients receiving opiates for pain after surgery often require a stool softener or laxative to prevent constipation. The signs of constipation include infrequent bowel movements (less than every 3 days), difficulty passing stools, excessive straining, inability to defecate at will, and hard feaces
IMPACTION
Fecal impaction results from unrelieved constipation. It is a collection of hardened feces wedged in the rectum that a person cannot expel. In cases of severe impaction the mass extends up into the sigmoid colon.
DIARRHEA
Diarrhea is an increase in the number of stools and the passage of liquid, unformed feces. It is associated with disorders affecting digestion, absorption, and secretion in the GI tract. Intestinal contents pass through the small and large intestine too quickly to allow for the usual absorption of fluid and nutrients. Irritation within the colon results in increased mucus secretion. As a result, feces become watery, and the patient is unable to control the urge to defecate. Normally an anal bag is safe and effective in long-term treatment of patients with fecal incontinence at home, in hospice, or in the hospital. Fecal incontinence is expensive and a potentially dangerous condition in terms of contamination and risk of skin ulceration
HEMORRHOIDS
Hemorrhoids are dilated, engorged veins in the lining of the rectum. They are either external or internal.
FLATULENCE
As gas accumulates in the lumen of the intestines, the bowel wall stretches and distends (flatulence). It is a common cause of abdominal fullness, pain, and cramping. Normally intestinal gas escapes through the mouth (belching) or the anus (passing of flatus)
FECAL INCONTINENCE
Fecal incontinence is the inability to control passage of feces and gas from the anus. Incontinence harms a patient’s body image

Antibiotic Stewardship by Anushri Srivastava.pptxAnushriSrivastav
Stewardship is the act of taking good care of something.
Antimicrobial stewardship is a coordinated program that promotes the appropriate use of antimicrobials (including antibiotics), improves patient outcomes, reduces microbial resistance, and decreases the spread of infections caused by multidrug-resistant organisms.
WHO launched the Global Antimicrobial Resistance and Use Surveillance System (GLASS) in 2015 to fill knowledge gaps and inform strategies at all levels.
ACCORDING TO apic.org,
Antimicrobial stewardship is a coordinated program that promotes the appropriate use of antimicrobials (including antibiotics), improves patient outcomes, reduces microbial resistance, and decreases the spread of infections caused by multidrug-resistant organisms.
ACCORDING TO pewtrusts.org,
Antibiotic stewardship refers to efforts in doctors’ offices, hospitals, long term care facilities, and other health care settings to ensure that antibiotics are used only when necessary and appropriate
According to WHO,
Antimicrobial stewardship is a systematic approach to educate and support health care professionals to follow evidence-based guidelines for prescribing and administering antimicrobials
In 1996, John McGowan and Dale Gerding first applied the term antimicrobial stewardship, where they suggested a causal association between antimicrobial agent use and resistance. They also focused on the urgency of large-scale controlled trials of antimicrobial-use regulation employing sophisticated epidemiologic methods, molecular typing, and precise resistance mechanism analysis.
Antimicrobial Stewardship(AMS) refers to the optimal selection, dosing, and duration of antimicrobial treatment resulting in the best clinical outcome with minimal side effects to the patients and minimal impact on subsequent resistance.
According to the 2019 report, in the US, more than 2.8 million antibiotic-resistant infections occur each year, and more than 35000 people die. In addition to this, it also mentioned that 223,900 cases of Clostridoides difficile occurred in 2017, of which 12800 people died. The report did not include viruses or parasites

4. 2. Wearable's Support Bedside Care and Remote
Patient Monitoring
• Wearable's also offer great benefits for both nurses and their patients.
Small, Wi-Fi-enabled devices such as smart watches and monitors can
now track and transmit personal health data such as glucose levels,
blood pressure, body position and physical activity so nurses can keep
a better eye on patients — whether they’re at home or in a hospital.
• Doing work with theses equipments can improve care, save time,
prevent medical errors and minimize the potential for adverse effects.
Wearable's “provide numerous benefits to nurses, such as freeing up
time to provide better-targeted patient care,

Monika Devi NR M.Sc.Nursing
7. 5. Command Centers Deliver Detailed and
Actionable Insights
• Software-enabled command centers offer real-time updates,
improve patient experiences and help nurses stay more
actively aware of schedules, patient needs and room
availability.
• “All the information is in one location so we can make
decisions without having to go many different places,”
Murphy says. “We use that for staffing, scheduling,
discharge, plan of care and post-acute care.”

9. How Do Nursing Technologies Help
Patients
• The nursing profession finds many of these new
medical technologies help them with routine
processes, as well as decrease human mistakes and
errors that can come from too few nurses who are
working long hours with too many patients.
9
Monika Devi NR M.Sc.Nursing
10. 1. Automated IV Pumps
• Automated IV pumps control the dosages and drips given
to patients.
• Software and medical tech allows nurses to change the
drip amounts and medication doses so patients aren’t
waiting for changes.
• There are IV pumps for nutrition that give needed meals
at the right times.
• Additionally, there are self-pumps that allow patients to
increase a controlled amount of pain medication for
themselves.
10
Monika Devi NR M.Sc.Nursing
11. 2. Portable Monitors
• Portable monitor equipment allows nursing professionals to
check on patients, even if they’re on the move or busy
helping someone else.
• Portable devices monitor vital signs like ECG, respiratory
rates, and oxygen saturations while transmitting the
information back to a central monitor.
• This means that nurses will get an alarm notification if
there’s an emergency.
11
Monika Devi NR M.Sc.Nursing
12. 3. Smart Beds
• Smart bed technology can help nurses track
movement, weight, and even vitals.
• Smart beds also play a major role in keeping patients
safe and comfortable during a long hospital stay.
• With the number of falls and patient injuries inside
hospitals, smart beds are very important for patient
safety.
12
Monika Devi NR M.Sc.Nursing
13. 4. Wearable Devices
• Wearable devices and mobile apps are transforming
the healthcare industry.
• Devices that help track heart rates, exercise, sleep,
respiration, and more are helping people take their
health into their own hands.
13
Monika Devi NR M.Sc.Nursing
14. 5. Electronic Health Records
• Electronic Health Records (EHR) are replacing older
paper filing methods.
• Electronic Health Records allow nursing experts to
document care provided to patients and retrieve
information that can help prioritize care.
• Additionally, information entered into computer systems
can then be accessed by the care team, including doctors
and even patients themselves when necessary.
14
Monika Devi NR M.Sc.Nursing
15. 6. Centralized Command Centers
• One of the newest ideas for hospitals, centralized
command centers promise improved patient
experiences and better ways for RNs and doctors to
manage supplies, clinical technology, and capacity.
This is done through software applications such as
dashboards that provide real-time updates.
15
Monika Devi NR M.Sc.Nursing
16. 7. Tele Health and Apps
• Tele health is a valuable, newer element in healthcare.
• Hospitals and clinics allow patients to virtually video chat
with a doctor or nurse to describe their symptoms or show
doctors things like rashes or bumps.
• This helps patients with a quick diagnosis without leaving
the comfort of their own home.
• They can find out if they need to come in for further
testing or diagnosis, get a prescription for medicine, or get
medical advice.
