  • 2. DEFINITION  “DISCHARGING A PATIENT FROM ONE UNIT OR AGENCY & ADMIITING HIM OR HER TO ANOTHER WITHOUT GOUING HOME IN THE INTERIM.”
  • 3. TYPES OF TRANSFER  TRANSFER OF A PATIENT FROM ONE UNIT TO ANOTHER IN SAME HOSPITAL. TRANSFER OF A PATIENT FROM ONE HOSPITAL TO ANOTHER.
  • 4. STEPS OF TRANSFER FROM ONE UNIT TO ANOTHER IN SAME HOSPITAL. 1) WRITTEN ORDR BY PHYSICIAN. 2) INFORM PATIENT AND FAMILY. 3) INFORM WARD INCHARGE OF ANOTHER UNIT / WARD. 4) COMPLETE PATIENT’S DOCUMENTS AND DICHARGE SUMMARY. 5) ARRNGEMENT OF TRANSPORT. 6) INFORM OTHER DEPARTMENTS.
  • 5. 7) TRANSPORTATION OF THE PATIENT. 8) RECEIVING OF TRHE PATIENT IN NEW WARD. 9) HANDING OVER & TAKING OVER OF THE PERSONAL BELONGINGS. 10)DOCUMENTATION.
  • 6. STEPS OF TRANSFER FROM ONE HOSPITAL TO ANOTHER  PATIENT CAN TRANSFERRED TO:-  EXTENDE CARE FACILITIES. SKILLED NURSING FACILITIES.
  • 7. NURSES ROLE IN TRANSFER OF A PATIENT ACTION RATIONAL 1) BE SURE THAT PATIENT AND FAMILY INFORMED OF THE NEED TRANSFER. COMMUNICATION PROMOTES CO- OPERATION.
  • 8. ACTION RATIONAL 2)IF TIME PERMITS & IF THE PATIENT OR FAMILY HAVE SOME CHOICE , ENCOURAGE THE FAMILY TO INVESTIGATEVARIO US FACILITIES & COLLABERATION ON THEY PREFER. DECISION SHOULD BE ALWAYS MADE BY PERSONS WHO ARE GOING TO BE AFFECTED.
  • 9. ACTION RATIONAL 3) COMMUNICATE WITH THE AGENCY OR UNIT WHERE THYE PATIENT WILL BE TRANSFERRED. 4) MAKE PHOTOCOPY OF MEDICAL RECORDS. OTHER PERSONNEL NEEDS TIME TO PREAPARE FOR PATIENT’S ARRIVEL. A COPY OF THE RECORDS AIDS INB CONTINUTY OF CARE & AVOIDS DUPLICATION OF SERVICES.
  • 10. ACTION RATIONAL 5)PROVIDE WRITTEN TRANSFER SUMMARY. PERSONNEL AT THE NEW FACILITY MAY NOT HAVE TIME TO READ THE DOCUMENTS THOROUGHLY BEFORE ASSUMING RESPONSIBILITY OF PATIENT’S CARE.
  • 11. ACTION RATIONAL 6)COLLECT ALL THE PATIENT’S BELONGINGS CARELESSNESS CAN LEAD TO THE LOSS OF THE PATIENT’S VALUABLE BELONGINGS & CAUSE INCONVIENENCE IN RETURNING THEM.
  • 12. ACTION RATIONAL 7)HELP IN TRANSPORTATIO N OF THE PATIENT. 8)GIVE TRANSFER PERSONNEL A COPY OF THE MEDICAL RECORDS IN A FOLDER OR ENVELOP ASSISTANCE REDUCES DEMANDS ON THE PATIENT. TO AVOID LOSS OF ANY DOCUMENTS & PROTECT CONFIDENTIALITY
  • 13. ACTION RATIONAL 9)COMPLETE THE MEDICAL RECORDS BY ADDING TRANSFER SUMMARY. 10)DOCUMENTATION ALL RECORDS TO BE KEPT FOR FUTURE REFERENCE.
  • 14. ACTION RATIONAL 11)NOTFY OTHER DEPARTMENTS OF HOSPITAL REGARDING PATIENT’S TRANSFER. EACH DEPARTMENT HAS IT’S OWN RESONSIBILITY TOWARDS PATIENT.
