4. Work place which is of the following nature , shall

Which is large enough and so configured that an
employee can enter bodily and perform work .
Which is limited or restricted means of entry or
exit .

Tank Pipes

7. Hazards in Confined Space
Oxygen Deficiency
Flammable like – Methane , Hydrogen , Acetylene ,
Propane , Gasoline fumes etc.
Toxic like – Carbon monoxide, Hydrogen Sulphide ,
Welding fumes .
Electricity
Mechanicals Hazards like – Mixers , Crushers etc.
8. Confined Space Entry Procedure
Isolate the confined space from all hazards
Ventilate the space
Conduct a Tool Box meet
Fill entry permit form
Test the atmosphere
Enter the space and proceed with work
Close the permit when job is completed
10. Isolate the confined space from all hazards :-
Close valves – Double block , Blank Flange
Empty the space – De-pressurize the , vent. , Drain
Lock out / Tag out Equipments – Electrical sources ,
rotating / Hazardous materials .
Clean residues from the confined space .
Ventilate the space
11. Use fan or air movers to ventilate the confined
space . Make sure that the air supply is not
contaminated .
If welding / Gas cutting shall be carried out in side
the confined space , ventilate at the space.
Use air supplied respirators or self contained
breathing apparatus , if mechanical ventilation can
not be provided or it is not effective .
12. Conduct a Tool Box meet
Make sure that all the Crew members
(Attendants , entrants , entry supervisor , and all
workmen) attend the meeting .
Review hazards of entry and work
Review Requirement of PPE .
Review Procedure for Emergency rescue .
13. Fill entry permit form
Fill permit Form Completely .
No Entry Shall be allowed without a valid permit
.
Permits are Valid for one days and renew the
permit , if required .
14. Test the atmosphere
Test the atmosphere using calibrated tester prior to
the human entry inside the confined space
Stop the ventilation at least 15 minutes before the
test to allow time for the confined space atmosphere
to normalize . Records the results of the test on the
permit register .
Check the oxygen content . it should be at least
19.5% and less than 21 % .

16. Enter the space and proceed with work
Check for in-flammables . it should be less than 10%
of the lower explosive limit of the flammable material
contained in the confined space .
Check the toxic gases .
An attendant shall be posted near the entrance for
the duration of the work . He shall be in constant
communication with the entrants while the job is in
progress .
All entrants shall sign the sign in log when entering
the space and sign out when exiting .
17. The attendant shall maintain the permit and
sign in log for duration of the work .
Hot work in confined space can cause heat
stress concerns appropriate rest regimes shall be
followed .
Permit shall be posted at the entrance of the
confined space and at a location that is readily
visible by personnel entering the space .
18. Close the permit when job is completed
Remove all personnel tools and equipments .
Remove the debris and residues .
Sign off the entry register .
Close the confined space .
Close the permit and return permit to Control
room .
