9. 1964’de Terasaki ve arkadaşları sitotoksik antikorları kullanarak, mikrolenfositotoksisite yöntemi ile antijenlerin serolojik olarak tanımlanmasını sağlamışlardır. Deneysel cerrahinin babası Hunter , 18. yüzyılın sonlarında modern transplantasyon dönemini başlatmıştır… Alexis Carrel , 1905 yılında hayvanlar ile deneysel çalışmaları başlatmıştır. Bir köpek yavrusundan aldığı kalbi yetişkin bir köpeğin boynuna yerleştirmiştir . Heparin, 1915’de bir tıp öğrencisi olan Jay McLean tarafından bulunmuştur. Lindbergh , 1927 yılında New York – Paris seferi yaparak, atlantik okyanusunu geçmeyi başardı. Yıldızı paralayn başarılı mühendis, Carrel’ın labaratuarında fikirleri ile ortak çalışmalara katılıyor. Kalp akciğer makinesinin temelleri atılıyor… Carrel, zamanla vasküler anastomoz teknikleri geliştirerek, gelecekte alacağı nobel ödülünü ne kadar hak ettiğini gösteriyor. İnsanoğlunda ilk intratorasik heterotopik kalp nakli 1946 yılında Sovyet cerrah Vladamir Demikhov tarafından başarıyla gerçekleştirilmiştir. 1950 yıllarında kalp akciğer makinesi kullanıma girmeye başlıyor. İlk kan transfüzyonları 17. yy’da hayvanlar ve insanlar arasında denenmiş ve alınan korkunç sonuçlar nedeniyle bu konu 150 yıl boyunca bir daha gündeme gelememiştir. 1900 yılında Landsteiner ve Miller , insanları kanlarındaki aglutininlere göre gruplandırarak transfüzyonları tekrar gündeme getirirken, doku tiplendirmesinin de yolunu açmışlardır. 1937’de Gorer insanlarda ilk histokompatibilite antijenini tanımlamış ve self-nonself ayrımını izah etmiştir. 1958 yılında Golberg , Berman ve Akman ilk izole ortotopik kalp naklini kardiopulmoner bypass desteğinde bir köpek üzerinde gerçekleştirdiler. 1960’ lara doğru donör organa karşı alıcıda meydana gelen reaksiyonlar incelenerek zamanla rejeksiyon kavramı oluşturuldu. Lower , Dong ve Shumway ’ ın ortotopik kalp transplantasyonu yaptıkları köpekleri ilk uzun yaşayan alıcılar oldu. İmmunsupresif tedavi ( azatiopirin ve metilprednizolon ) uygulanmaya başlandı . 1963 yılında James Hardy insanda ilk akciğer naklini gerçekleştirdi. 1 yıl sonra da maymun kalbinden xenotransplantasyon gerçekleştirmiş ancak alıcı 2 saat içinde ölmüştür. 1952’de Dausset multiple kan transfüzyonu yapılanlarda lökoaglutininler oluştuğunu gözlemleyerek insanlarda HLA lokuslarının keşfine giden yolu açmıştır. İnsandan insana ilk başarılı kalp naklini Güney Afrikalı cerrah Christiaan Barnard 3 Aralık 1967 günü gerçekleşmiştir. Philip Caves ve arkadaşları transvenöz endomiyokardial biopsi alma tekniğini geliştirerek histolojik olarak greft rejeksiyonunu izleme olanağı tanıdılar. Türkiyede insandan insana ilk kalp naklini 1968 yılında Siyami Ersek ve Kemal Beyazıt birbirlerinden bağımsız olarak gerçekleşmişlerdir. 1981 yılında immunsupresif tedavide kullanılabilecek başta siklosporin olmak üzere spesifik ajanlar bulunmaya başlanmıştır. Aynı yıl Stanford’ da Shunway ve Bruce Reiztsis İlk başarılı kalp – akciğer naklini gerçekleştirmiştir . 1983 yılında Cooper , Torontoda ilk başarılı akciğer naklini gerçekleştirdi. Yine aynı ekip 1986’ da ilk çift akciğer naklini gerçekleştirdi . Eylül 1989’ da Koşuyolu Kalp ve Eğitim Araştırma Hastanesi’nde Cevat Yakut ve ekibi tarafından ilk başarılı kalp nakli gerçekleştirildi. İstanbul Üniversitesi Tıp Fakültesi’nden Necip Göksel Kalaycı ilk akciğer naklini gerçekleştiren isim oldu… 07/03/2009’ da Asım Kutlu ilk başarılı akciğer naklini Yeditepe Süreyyapaşa Eğitim ve Araştırma Hastanesi’ nde gerçekleştirdi. 08/04/2009’ da ise ilk başarılı çift taraflı akciğer nakli Ege Üniversitesi Tıp Fakültesi’ nde bronşitis obliterans tanılı çoçukta başarı ile gerçekleştirildi. 19/09/2009’ da S.B. 95. Normal teleskopik tekniğin coronal kesiti ve iç sınırda sütüre edilmemiş endolüminal flep Bronşial dehiscence, erken post-op dönemde en sık hava yolu komplikasyonudur. En iyi bronkoskopi ile görülebilir. Burada BT ile bronşial duvarındaki fokal defekt ve ekstraluminal hava görülüyor. Bronşial strüktür, geç post-op periyotta ~ %10 oranında gözlenir. Genellikle bronkoskopi ile saptanır. BT, özelliklemultiplanar rekonstrüksiyon yöntemiyle, stenozun uzunluğu ve stentin durumu hakkında yardımcı olabilir. Plevral boşluk komplikasyonları, ~ % 22 oranında gözlenir. Bilateral akciğer ve kalp + akciğer transplantasyonunda daha sık. Artmış kapiller permeabilite ve transplante edilen akciğerin yetersiz visseral lenfatik dolaşımı sorumlu tutulur. Çoğu kendi kendini sınırlar ve 2 hafta içinde resorbe olur. Israr eden efüzyonda, komplikasyon akla gelmeli. ( Örn. : Ampiyem, organize hematom, rejeksiyon, post transplant lenfoproliferatif hastalık gibi ) Ampiyem ve Hemotoraks : Mortalite artışı yapan tek plevral komplikasyondur. Ampiyem % 4 oranında görülür. Vasküler komplikasyonlar : Oldukça nadirdir. Özellikle arter anastomozlarında gelişir. Perioperatif dönemde pulmoner infarkt riski yüksektir. ( Transplante edilen akciğerin alternatif bronşial kanlanması yok ) Post-op perfüzyon sintigrafisi pulmoner arterdeki anastomoz defektini yada trombüsü saptamada oldukça yararlıdır. Kötü prognozu çağrıştırır. Ancak günümüzde etkin dilatasyon ve stent implantasyon tekniklerindeki gelişmeler umut verici…

97. Reperfüzyon Hasarı : % 95 hastada gözlenen bir çeşit non kardiojenik ödemdir. Çoğu hastada minimal semptom oluşur. Sol ventrikül yetmezliği, sıvı yüklenmesi, enfeksiyon ve akut rejeksiyon ekarte edildikten sonra tanı konabilir. Sebep : Cerrahi travma, denervasyon, surfaktan yapım azlığı, permeabilite artışı, lenfatik akım olmayışı İlk günlerde başlayıp 4-5. günlerde pik yapıp 10. güne doğru geriler. 7 gün sonrası şiddetli persistant durumlarda akut rejeksiyon yada enfeksiyonlar akla gelmeli.
98. Akut rejeksiyon : Tipik olarak 5. ve 10. günler arası olmak üzere ilk 3 haftada gelişir. Çoğu alıcı İlk 3 aylık periyotta 2 yada 3 rejeksiyon episodu geçirir. ( İlk 6 ayda % 60 -75 gözlenir ) Tekrarlayan akut rejeksiyon episodu Bronşiolitis obliterans riskinde artış ile ilişkilidir. Alıcılar asemptomatik olabileceği gibi dispne, ateş yüksekliği, lökositoz, azalmış egzersiz kapasitesi, arteryel oksijen saturasyonunda azalma FEV1 ve FVC’ de azalma gelişebilir. En önemli özelliği klinik ve radyolojik olarak kortikosteroidlere ve dozu artırılan immunsupresiflere dramatik cevap vermesidir. Radyografide ısrar eden perihiler ve bazal opasiteler, plevral efüzyon, septal kalınlaşmalar gözlenir. Transbronşial biopsi tanı ve enfeksiyonu ekarte etmede önemli. Perivasküler lenfatik infiltrasyon izlenir.
99. Enfeksiyon : En sık komplikasyon. Major morbidite ve mortalite sebebi. Akciğer denervasyonu sonucu öksürük refleksi yokluğu, immunsupresyon, yetersiz mukosilier klirens ve lenfatik drenaj zemin hazırlayan faktörlerdir. Rutin TMP/SMZ profilaksisi nedeniyle pneumocystis carini pnömonisine artık sık rastlanılmıyor. % 50’ si bakteriyeldir. En sık ilk 1 ay içinde gözlenir. En sık etkenler gr (-) basiller Klebsiella, Pseudomonas aeruginosa ve Enterobacter cloacae; gram (+) Staf aureus ve Hemophilus influenzadır. ( Burkholderia cepacis öz. kistif fibrozislilerde oldukça ciddi enf. ) Radyolojik bulguları normal bireylerdeki gibi konsolidasyon, kavitasyon ..
100. Fungal enfeksiyon : İlk 10 – 60 gün arasında en sık. Yüksek mortalite ile birlikte. Viral enfeksiyonlardan daha az saptanır. En çok kandidiazis ve aspergillosiz tanımlanır. Özellikle Akciğer transplant hastaları, diğer immunsuprese kişilere göre aspergillus enfeksiyonuna daha yatkındır. Aspergillus enfeksiyonu alıcıların %5’ inde gelişir. Ancak takip bronkoskopilerde saptanan sessiz pnömoni yada anjioinvaziv sistemik disseminasyona uğrayan enfeksiyon yapabilir. Konsolide alan, kavite, buzlu cam alanları yapar. Radyolojik olarak saptanamayan ülseratif trakeabronşit yapabilir. Bu da anastomoz dehiscence ve ardında bronkomalazi ile stenoza yol açabilir.
101. Viral enfeksiyon : En sık CMV , diğerleri HSV, RSV, Adenovirus İlk 1 yıl içinde özellikle ilk 4 ay içinde gelişir. Akciğer transplantlılarda gelişen pnömonilerin en sık 2. nedeni ve oportunisitk enfeksiyonlar arasında 1. sıra CMV dir. Bronşiolitis obliterans gelişiminde risk faktörü ve bakteriyel yada mantar enfeksiyonları için uygun ortamı oluşturduğu düşünülür. Hasta asemptomatik yada fulminan tabloda olabilir. Primer enfeksiyon : Seronegatif alıcıya seropozitif donörden nakil olunca gelişir. (%90 ) % 50 ciddi seyreder. Bu yüzden donör uyumu önemli. Sekonder enfeksiyon : Latent virusun immunsupresyon nedeniyle reaktivasyonu ile olur. Primere göre daha iyi seyirli. Bronkoalveolar lavaj yada tranbronşial biopsi ile tanı konur. Grafi normal olabilir, parankimal bulanıklık, retikülonodüler opasite ve minimal efüzyon izlenebilir.
102. Post transplant lenfoproliferatif hastalık ( PTLD ) : Histolojik olarak bening poliklonal lenfoid proliferasyon ile agressif B hücreli yüksek grade lenfoma arasında değişen bir spektrumdur. Diğer solid organ transplantasyonu yapılan hastalardan daha sık saptanır. % 1,8 – 20 ( Ortalama % 5 ) oranında saptanır. Bu oran EBV, CMV profilaksisi, yaş, immunsupresyon durumu ile değişebilmekte. İlk bir yıl içinde özellikle 3. ve 4. aylarda tanı konulur. Asemptomatik yada kilo kaybı, ateşlenmeler, letarji ile seyredebilir. Grafide soliter yada multipl nodül izlenebilir. Ekstrapulmoner ilerleme nadirdir. ( Plevral efüzyon, perikard, timus, mediastinal LAP ) BT eşliğinde TTİİAB yapılabilir.
103. Bronşitis Obliterans : En sık 6. ve 18. aylar arası gözlenir. Hastaların morbidite ve mortalitelerinin en büyük sebebidir. Transplant yapılanların ~ % 50’ si etkilenir. % 50 si de bu yüzden kaybedilir. Kronik immunolojik rejeksiyonun manifestasyonu olarak kabul edilir. Risk faktörleri : Enfeksiyon ( Öz. CMV ), daha önceki akut rejeksiyon atakları, GÖRS HLA uyumsuzluğu Klinik olarak nefes darlığı kötüleşir, öksürük ve akciğer fonksiyonlarında açıklanamayan gerileme izlenir. Patoloji küçük hava yollarını dağınık bir şekilde tutar. Erken dönemde transbronşial biopsi diagnostik olmayabilir. Konnektif doku trichrome boya ile bonabilir. Grafide normal bulgular, akciğerlerde volüm artışı, vasküler işaretlerde azalma, subsegmental atelektaziler izlenebilir. BT’ de bronşlarda duvar kalınlaşması, dilatasyon, özellikle alt loblarda mozaik attenuasyon izlenir. Erken dönemde inspiriumda hava yakalanması saptanması gözlenebilir.
105. Rekürrent hastalık : Nadiren görülür ( ~ %1) Radyolojik bulgular normal hastalık seyrindeki gibidir. Ancak enfeksiyon, rejeksiyon ya da PTLD ile karışabilir. Sarkoidoz( *), Lenfanjioleiomyomatozis (LAM), Bronkoalveolar ca, Langerhan’s hücreli Histiozis rapor edilen vakalardır. Sarkoidoz : 2 hafta ile 2 yıl arasında geniş bir zaman diliminde nüksedebilir. Transbronşial biopsinin insidental bulgusu olabilir. Non – kazeifiye granülom rapor edilir. ( TBC ve Mantar enf. dışlanmalı ) Yamalı dağılım nedniyle negatif biopsi tamamen hastalığı ekarte etmez. Lenfanjioleiomyomatozis : En sık 1. ve 5. yıllar arasında tekrarlar. Natürü gereği pnömotoraks, şilotoraks, assit, hemorajik renal anjiomyolipomlar.. Bronkoalveolar ca : % 50 nükseder. Transplantasyon sonrası tekrarlasa bile 5 yıllık survey ~ % 40
106. Üst lop fibrozisi : Sebebi tam bilinmemekle birlikte kronik rejeksiyonun nadir bir şekli olduğu düşünülür. 18 ile 72. aylar arasında gözlenir. ( Ort. 42. ay ) Hastada progresif dispne gelişir. Solunum fonksiyon testlerinde miks obstrüktif ve restriktif tip patern izlenir. Patolojik olarak nonspesifik inflamasyon ve fibrozis izlenir. HRCT bulgusu : İnterlobüler septum kalınlaşması, kaba retiküler opasiteler, periferal buzlu cam görünümü, fibrozis, traksiyon bronşektazileri, bal peteği görünümü, volüm kaybı
107. Post transplant biopsi nodül : Yanında yada çevresinde halo şeklinde buzlu cam alanının olduğu 2 – 15 mm. çaplı solid yada kaviter nodül. HRCT’ de biopsiden 4 hf sonra izlenebilir.
