A 23-year-old male presented with cough, hemoptysis, and left-sided chest pain for 2-3 months. Imaging showed left mediastinal and lung masses. Biopsy of an expectorated cast revealed malignant cells consistent with mixed cellularity Hodgkin's lymphoma. Further workup found widespread lymphadenopathy and additional lung and liver lesions. Biopsy of a lung lesion found alpha-fetoprotein positive cells. Differential diagnoses include mixed cellularity Hodgkin's lymphoma and hepatocellular carcinoma. Further testing is needed to determine the final diagnosis and guide treatment.
