1) Pulmonary edema is defined as an accumulation of fluid and solutes in the extravascular pulmonary spaces, which can quickly lead to acute respiratory insufficiency due to flooding of the alveoli.
2) In lesional pulmonary edema, the causal mechanism is an anatomical alteration of the alveolar-capillary membrane, which increases its permeability coefficient.
3) Several factors can cause lesional pulmonary edema by directly damaging the alveolar-capillary membrane, including infections, toxins, burns, trauma, and circulatory support procedures. This results in a severe impairment of gas exchange and hypoxemia.

The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma.
STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.

3. Définition
• L’oedème pulmonaire est défini comme une
accumulation de fluides et de solutés dans les
espaces extravasculaires pulmonaires.
• Une des conséquences principales de l’oedème
pulmonaire est l’inondation des alvéoles
pulmonaires, susceptible d’entraîner très
rapidement une IRA.
• Dans l’oedème lésionnel, le mécanisme causal
est représenté par une altération anatomique de
la membrane alvéolo-capillaire.
4. Physiopathologie
A. La membrane alvéolo-capillaire:
Constituée de 3 éléments principaux :
• L’endothélium capillaire.
• L’espace interstitiel alvéolo-capillaire.
• La barrière épithéliale.
Les échanges liquidiens s’effectuent entre le
capillaire et le secteur interstitiel drainé par
le réseau lymphatique pulmonaire.
5. Le flux net de liquide sortant du capillaire obéit à l’équation
de Starling :
Q = a [K(Pcp + π int) – ( Pint + π pl)]
Q = flux net de liquide sortant du capillaire vers l'interstitium
a = coefficient de proportionnalité
K = coefficient de perméabilité
Pcp = pression capillaire pulmonaire
π int = pression oncotique du liquide interstitiel
Pint = pression interstitielle
π pl = pression oncotique plasmatique

7. • À l’état basal, le flux net de liquide se fait du capillaire
vers l’interstitium.
• Il n’y a pas d’oedème car le drainage interstitiel est
simultanément assuré par la circulation lymphatique.
• Une accumulation extravasculaire de liquide entraîne
une augmentation de débit lymphatique, lorsque les
possibilités de drainage lymphatique sont dépassées,
l'oedème pulmonaire apparaît.

8. Ainsi, d'après l'équation, l'apparition d'un oedème
pulmonaire peut être lié à l'un des 6 mécanismes
suivants (parfois associés) :

9. Plusieurs mécanismes compensateurs s’opposent à
l’inondation alvéolaire :
• L’accroissement de Pint.
• La diminution de π int liée à la dilution des protéines
interstitielles.
• L’accroissement du Q lymph.
Lorsque ces mécanismes sont débordés survient l’oedème
alvéolaire, peut-être favorisé par des modifications de la
perméabilité de l’épithélium alvéolaire

10. B. OEdème pulmonaire lésionnel:
Est lié à l’augmentation du coefficient de
perméabilité par l’altération anatomique de la
membrane alvéolo-capillaire (qui est normale ou
basse à l’état normal).
Le liquide d'oedème a une très forte teneur en
protides proche de celle du plasma.
L'oedème pulmonaire lésionnel est responsable
d'insuffisance respiratoire aigue avec évolution
ultérieure possible vers la fibrose interstitielle :
SDRA.
11. Étiologies
Très nombreuses, Essentiellement :
• Infectieuses: grippe, septicémies, choc
septique.
• Toxiques: inhalation de gaz toxiques,
ventilation en oxygène pur, inhalation du
liquide gastrique (syndrome de Mendelson).
• Brûlures respiratoires
• Traumatisme thoracique
• Noyade
12. Rarement :
• Embolies pulmonaires, amniotiques, ou
graisseuses.
• Décours de la circulation extracorporelle.
En cas d'étiologies infectieuses ou toxiques c'est
l'atteinte directe de la membrane alvéolo-capillaire
qui est responsable de l'oedème pulmonaire. Dans
les autres étiologies, le mécanisme précis
demeure inconnu.
13. Évolution
Se fait en 3 phases :
• Début brutal avec oedème pulmonaire
asphyxique.
• 2ème phase: lésions inflammatoires, oedème,
constitution de membranes hyalines.
• 3ème phase: fibrose pulmonaire chronique.
La conséquence est la survenue d'un trouble majeur des
échanges gazeux avec hypoxémie réfractaire par effet
shunt à l'étage pulmonaire.
La ventilation est nulle et la perfusion est maintenue.
14. Diagnostic clinique
• Il s'agit d'une URGENCE MÉDICALE
imposant un diagnostic rapide et une
thérapeutique efficace, la mort au cours de
l'oedème pulmonaire étant possible.
• Le début est brutal, nocturne, précédé de
grésillement laryngé et de toux irritative.
15. Le tableau clinique est d’une IRA associant :
• Tachypnée avec orthopnée et sueurs
• Expectoration mousseuse
• Anxiété et agitation
• Cyanose périphérique
• Râles crépitants à l’auscultation pulmonaire
Rechercher une douleur thoracique ou un
trouble du rythme (IDM ou, FA), une poussée
HTA, un écart de traitement ou de régime (huitres
par apport excessif de sel)
16. Signes cliniques de gravité:
• Tachypnée > 30/min, bradypnée
• Cyanose, marbrures
• Troubles de la conscience
• PA < 110 mmHg
• Rapidité de la détérioration clinique
18. Examens complémentaires
• La radiographie du thorax: réalisée au lit du patient , en
position ½ assise, montre la présence d'opacités alvéolo-interstitielles
diffuses, bilatérales, symétriques, donnant
un aspect floconneux.
• Les gaz du sang: une hypoxémie sévère, ↓ PaO2,
↓ SaO2.
• Echographie cardiaque : dilatation des cavités gauches,
trouble de la cinétique ventriculaire.
• Cathétérisme droit par sonde de Swan-Ganz si
l'échographie n'est pas contributive ou en cas d'évolution
défavorable.
• Enzymes cardiaques (CPK, Tropnonine), enzymes
hépatiques, urée et créatininémie.

20. Traitement
Traitement de la DR:
• Ventilation en pression expiratoire positive
intermittente, L'utilisation d'une Pression d'Expiration
Positive (PEP) permet d'augmenter la capacité résiduelle
fonctionnelle (CRF) en recrutant des alvéoles
pulmonaires collabées non fonctionnelles. Deux modes
ventilatoires utilisent la PEP : la ventilation mécanique
avec PEP (VM-PEP) et la ventilation spontanée avec PEP
(VS-PEP) qui nécessite un état de conscience parfait du
patient.
• Maintien d'un état circulatoire correct, évitant la surcharge
• Prévention, diagnostic précoce et traitement des
surinfections pulmonaires.
