Testis torsion can occur in undescended testes due to the abnormal positioning of the testis which increases risk. It is not uncommon, accounting for 23% of cases of torsion. However, delayed diagnosis leads to a low salvage rate of only 10% for cryptorchid testes with torsion compared to 56% for isolated torsion. Abdominal or groin pain in children with an undescended testis warrants investigation including ultrasound or exploratory surgery to rule out torsion due to the increased risk and low chance of testis survival if not addressed promptly. Proper diagnosis is important to distinguish torsion from other causes of pain like incarcerated hernia.
