This document is an application for companies to remit Utah state withholding tax payments via electronic funds transfer (EFT). By completing and signing the form, the company agrees to initiate tax payments by transferring funds electronically from their bank account to the tax commission's account. The company must follow procedures for transfer format and content specified by the commission. Either party can terminate the agreement by providing 10 days written notice. The authorized signature will approve all future payments until changed.
