This document is a Notice of Change form from the Utah State Tax Commission. It allows taxpayers to report changes to existing tax accounts for corporate tax, withholding tax, sales tax, or mineral production withholding tax. The form has four sections: 1) general taxpayer information; 2) changes to the main tax account like closing the account or changing an address; 3) changes to individual sales tax outlets; and 4) an authorized signature line. The completed form should be returned to the Utah State Tax Commission to process any requested changes.
