εξωνοσοκομειακά στον κόσμο

κατάργηση, ενώ θα πρέπει να δημιουργηθούν/ εξοπλιστούν νέες δομές

• Η αποκλειστική απασχόληση οδηγεί τους ιατρούς σε συρρίκνωση
των εισοδημάτων τους και σε αυξημένη δυσαρέσκεια, ειδικά αν
δεν μπορεί να συνδυαστεί με μια ικανοποιητική αποζημίωση…
• Ο κίνδυνος να αλιεύει ασθενείς ο γιατρός και να τους μεταφέρει
στο ιδιωτικό του ιατρείο δυνατόν να εκλείψει απλά με την
απαγόρευση στον οικογενειακό ιατρό να παρέχει υπηρεσίες
ιδιωτικά σε ασθενείς της λίστας του – εφαρμόζεται πιλοτικά στην
Μ. Βρετανία από έτους.
Ταυτόχρονα η δυνατότητα του ασθενούς να αλλάξει οικογενειακό
ιατρό, αν δεν είναι ικανοποιημένος από αυτόν, δημιουργεί κίνητρο
στο γιατρό να παρέχει ποιοτική φροντίδα στους ασθενείς της
λίστας του.

33. Pay for Performance (P4P)

stimulate good performance and separately creates

• The main goal of P4P is to improve patient outcomes

34. P4P
• Designing a fair and effective P4P program is a complex undertaking. This
complexity and the limited effectiveness thus far cast serious doubt on whether
P4P can be cost effective.
• Performance payments themselves, data collection and validation, payment
calculation involve significant transaction costs.
• Evaluations of P4P programs should assess the impact on quality but also include
comprehensive cost analyses. However, a recent review identified only nine

70. Referral system- Gatekeeping…
One method of achieving coordinated care is for access to a specialist to be available
only by referral from the patient’s GP, the so-called “gatekeeper” function.
The role of providing first-contact care is more developed in countries with a
gatekeeping system
• 1. No gatekeeping system in place.
Patients have direct access to most physicians
(Austria, Belgium, Cyprus, Germany, Luxembourg, Switzerland, Turkey)
• 2. No formal gatekeeping system in place, but there are incentives.

73. Integration and coordination
• The lack of multidisciplinary collaboration and teamwork
is not only identified as a gap in health policy but also as a
weakness in the process of service delivery.

πρόβλημα υγείας του, με τον ΕΟΠΥΥ να πληρώνει τις επιλογές του, που πολλές
φορές είναι λανθασμένες…
• Ασθενείς που χρειάζονται εξειδικευμένη φροντίδα αντιμετωπίζουν εμπόδια στην
πρόσβαση (αναμονή, out of pocket)- περιοχές της χώρας χωρίς κανέναν
συμβεβλημένο ειδικό
• Τα εμπόδια στην πρόσβαση στην ΠΦΥ, αλλά και στην εξειδικευμένη
εξωνοσοκομειακή φροντίδα οδηγούν στο φαινόμενο της αντίστροφης
υποκατάστασης- επιβάρυνση των νοσοκομείων
Η προσδοκία από την Πφυ είναι να αυξήσει την αποδοτικότητα του συστήματος υγείας και την ανταποκρισμότητα στις αναγκες του πληθυσμου
