11. Public resources should provide the core of

Revenue-raising mechanisms should be defined based on the ability to
pay and be progressive.
Out-of-pocket payments must be reduced to levels where they are no
longer a financial barrier to accessing needed care, impoverish
households, or push households deeper into poverty.

13. All countries need to both invest more and invest better in
PHC
by designing their health financing arrangements—
mobilising additional pooled public funding, allocating and
protecting sufficient funds for PHC,
and incentivising providers to maintain the health of the
populations they serve—in ways that place people at the
centre and by addressing inequities first.

Establishing the right financing arrangements can drive improvements in how PHC is delivered and
equip the system to respond effectively to evolving population health needs

that is provided free at the point of service use
• resources for PHC are allocated equitably across levels of service delivery
and geographic areas, and are protected so that sufficient resources reach
frontline primary health-care service providers and patients and prioritise
the poorest and most vulnerable people.
• primary health-care provider payment mechanisms support the allocation
of resources based on people’s health needs, create incentive
environments that promote PHC that is people centred, foster continuity
and quality of care, and remain flexible enough to support rapidly changing
service delivery models. To achieve these goals, a so-called blended
provider payment mechanism with capitation at its core is the best
approach to paying for PHC.

πολύπλοκες μικτές μεθόδους για να επιτύχουν ένα
περισσότερο επιθυμητό μίγμα κινήτρων, αποφεύγοντας μερικά

Λειτουργούν στην Πορτογαλία παράλληλα:
-κρατικές δομές με μισθωτούς ΟΙ (UCSP)- κατάλοιπα από το παρελθόν και

και την ενημέρωση του ΑΗΦΥ
• Οι προσωπικοί γιατροί που θα συμβληθούν με το πρόγραμμα
(ιδιώτες συμεββλημένοι και γιατροί του ΕΣΥ) θα λάβουν ξεχωριστή
αποζημίωση ύψους 20 ευρώ ανά ασθενή.

Βέβαια δεν είναι όλοι οι ασθενείς ίδιοι
Υπάρχουν εύκολοι και δύσκολοι που απαιτούν προσπάθεια και κόστος
Με τα δημογραφικά στοιχεία και το κλινικό προφίλ ένας ασθενής δυνατόν να έχει 4-πλασιο κίνδυνο από το μέσο κίνδυνο
Υπάρχει site όπου φαίνονται οι επιδοσεις των πορτογαλων σε διαφορους δεικτες
Υπάρχουν δυο ειδη κέντρων υγείας που τρέχουν παράλληλα, τα uscp τα παραδοσιακά με μισθωτούς γιατρούς και τα usf, τα σύγχρονα με χρήση αποζημίωσης βάση απόδοσης, τα αποτελέσματα τους στους δείκτες απόδοσης διαφέρουν παρασάγγας, όπως στο ποσοστό γυναικών που έχουν υποβληθεί σε τεστ παπ 31% με 62%
Σε μαστογραφία – 41% με 59%
Στο ποσοστό ρυθμισμένων υπερτασικών 37 με 65%
Στο ποσοστο ρυθμισμενων διαβητικών 41% με 70%
Και μάλιστα με χαμηλότερη φαρμακευτική δαπάνη, αφού και αυτή αποτελεί στόχο απόδοσης!
