5. What is SelfCare Pharmacy?
• Innovative pharmacy practice that involves managing co-morbidities
and stable long term conditions, using therapeutic and
psychological tools, as well as aids to support independent living.
• Brings together public health, health care, social care, and self-care
(including self-management). It focuses on empowering the patient
to take control of their own health condition and living
independently in their local community.

6. What are the tools of SelfCare Pharmacy?
• The SelfCare Assessment form assesses a patient’s health, wellbeing, personal
and social factors that impact upon health.
• Pharmacists are trained in health coaching to transfer responsibility for care to
patients from the pharmacist. They are present to facilitate a prioritised
behaviour change to manage long term conditions and living independently.
• Patient SelfCare Plan is a written record, completed by the patient with the
support of the pharmacist. It puts the person, their needs and choices at the
centre of the process. It focuses on goal setting and outcomes that support the
person to achieve optimal health and wellbeing.
• Integrated working and communication (including team working, and working
with Health & Social Care professionals and voluntary groups)
