This risk assessment form evaluates potential hazards at a photoshoot location. It identifies slipping on a wet surface, broken glass on the ground, and falling tree branches as hazards. Controls currently in place include checking weather forecasts, regularly cleaning the area, and trimming back trees. The form recommends only using the area when dry to reduce slipping risks, staying away from any glass, and avoiding the area during bad weather to reduce risks from falling branches. It provides a risk rating scale to evaluate hazard severity, likelihood of harm, and overall risk score to determine what action needs to be taken.
Risk assessment PA.20
This risk assessment form documents potential hazards at a college location for an editing activity. Four main hazards are identified: (1) tripping hazards from bags, chairs, and wires that could injure students or teachers; (2) low hanging lights that could hit taller individuals; (3) eye strain from computer screens that could affect those prone to headaches; and (4) water spills near electronics that could damage equipment or cause a fire. Existing controls are identified to mitigate these risks, such as keeping areas clear and drinks away from computers. Further actions proposed include adding more space between tables to prevent crowding, and raising any low hanging light wires.

Further actions identified were moving any sharp objects, bringing protective gloves, and wearing stronger shoes. The risk was rated as medium based on potential for temporary disability from cuts.
Risk assesment stairs
This risk assessment form documents potential hazards for a photoshoot at the Lymm Hotel stairs. Hazards identified include slippery stairs, cars on the nearby road, falling branches from trees, and members of the public interfering. Controls already in place are being careful on the stairs if wet, walking on the pavement away from the road, keeping equipment concealed, and checking for hazards before shooting. Further actions planned are not going up wet stairs and changing locations if any hazards are found.

Tripping over the tripod and knocking it into someone Burning hands on lights 5 3 The camera operator could trip over the tripod causing it to fall and hit another person Someone operating the light in the room may accidentally touch the light causing a burn Stand clear of the tripod at all times Don’t touch any of the light except for the switch and allow the lights to cool down Take extra care when moving around the tripod Move lights when just turned on to ensure they aren’t too hot, allow them to cool down before moving again 1.
