What is referral system?
• When the health worker or nurse has insufficient
resources to manage a patient's condition at one
level in hospital or centre she is not able to manage
patient in her centre.
• In such situation, she needs help from another
centre, which is differently resourced with facilities
or services, it can properly assist the treatment of
patient.
• The insufficient resources may be drugs,
equipments or skills of staff to manage
condition of serious patient.
• In such conditions, patient is transferred to
other centre or hospital, where such
resources are available. This is called
referral of patient.
Purposes of referral system:
1. To seek expert opinion regarding the
patient.
2. To seek additional or different services
for patient then available services.
3. To seek admission facility and better
management of patient
4. To get proper diagnostic and therapeutic
tools for patient
• A good referral system is an essential
component of health care system.
• As more and more paramedical workers are
employed in the health industry, it becomes
necessary to establish a sound referral system.
• Referral system is also known as Triage
system.

• When should we refer a patient?
• The question arises in mind of health
worker or nurse very often, that which
short of patient are to be referred?
• So according to triage system, patients
are classified in to three categories.
• Category :1. Fatal conditions and life can't
be saved even with treatment.
• Category:2. Serious conditions and life can
be saved but only with immediate treatment.
• Category:3. Minor conditions so life is not
threatened and referral can be safely delayed
or managed.
• Referral Slip: All referral should be always
accompanied by a referral slip. Referral slip
contains the following information about patient.
1. Patient's name, address and identification number
2. Present complaints
3. Treatment given if any
4. Reasons for referral
5. Name and designation of person referring patient
6. Date and time of referral
7. Name of PHC (Primary Health Centre ) making
