Challenge: Patient Follow-Up
INTRODUCTIONS
Jonathan Meade - 16 years experience working in Information Technology;
   System Administrator, Systems Analyst, Software Developer, Project
   Manager, Department Director and Vice President of Information
   Technology. Mr. Meade earned his Master’s in Information Technology.


Chad Matthews - 17 years experience working in Information Technology with 6
   years working in Healthcare; Software Engineer, Systems Analyst, Software
   Engineer Team Lead, Project Manager and Executive Technology Director.
   Mr. Matthews earned his Bachelor’s in Business Administration and is holds
   certifications in Project Management Professional (PMP) and Six Sigma
   Green Belt.

James Schnitzius - 15 years experience working in Information Technology with
   5 years working in Healthcare. Development during his career has been
   used for successful healthcare organizations and by large corporations’ world
   wide to manage strategic email solutions. In his career he has worked on a
   wide variety of applications as an expert in C# and .Net development.
OVERVIEW
During the month of May, doctors, nurses, and informaticists submitted
  IT challenges facing some of Indiana’s leading healthcare
  organizations, including IU Health, Indiana Health Information
  Technology, St. Vincent’s Health, Indiana State Department of
  Health, Community Health, Marion County Public Health
  Department and the Franciscan Alliance.

Over 430 votes were cast on submitted IT challenges to narrow to 3
  challenges. Our team selected to work on a challenge called
  Patient Follow-Up.

The goal: Provide a system for providers/entities to electronically refer
  patients to one another that includes overview documentation to
  assist in patient introduction. 7/13/12 start date with round 1
  submissions due on 7/27/12.

JUDGING CRITERIA
   Likelihood to grow a scalable business that helps Indiana HIT
    sector.
   Impact on better health, better healthcare, and cost of care.
   The introduction of new experts to HIT.
   Willingness of judge to work with team to try and pilot the solution in
    their organization.

Chad Matthews - 17 years experience working in Information Technology with 6 years working in Healthcare; Software Engineer, Systems Analyst, Software Engineer Team Lead, Project Manager and Executive Technology Director. Mr. Matthews earned his Bachelor’s in Business Administration and is holds certifications in Project Management Professional (PMP) and Six Sigma Green Belt. James Schnitzius - 15 years experience working in Information Technology with 5 years working in Healthcare. Development during his career has been used for successful healthcare organizations and by large corporations’ world wide to manage strategic email solutions. In his career he has worked on a wide variety of applications as an expert in C# and .Net development.

OVERVIEW During the month of May, doctors, nurses, and informaticists submitted IT challenges facing some of Indiana’s leading healthcare organizations, including IU Health, Indiana Health Information Technology, St. Vincent’s Health, Indiana State Department of Health, Community Health, Marion County Public Health Department and the Franciscan Alliance. Over 430 votes were cast on submitted IT challenges to narrow to 3 challenges. Our team selected to work on a challenge called Patient Follow-Up. The goal: Provide a system for providers/entities to electronically refer patients to one another that includes overview documentation to assist in patient introduction. 7/13/12 start date with round 1 submissions due on 7/27/12.
