Quinolones were discovered in 1962 and work by inhibiting bacterial DNA gyrase and topoisomerase IV. They are well absorbed orally and concentrated in urine and bile. Common side effects include nausea, headaches, and tendon rupture. While generally effective against gram-negative bacteria, concerns include increasing bacterial resistance and adverse effects like QT prolongation. Newer generations have expanded gram-positive and atypical pathogen coverage but resistance remains a concern.
