Promoting Comfort and Pain Management on an Adult
Acute Medicine Unit
Prior to the start of this QI project:
• Develop a Comfort Menu (CM) listing
non-pharmacological interventions
that patients could use to aid in pain
control by November 1, 2014
• Evaluate the unit nurses perceptions
regarding pain management and
provide in-services regarding quality
pain management by November 30,
2014
The success of this QI project will be
measured by patients responses to questions
related to pain/comfort on the unit discharge
cards and an improvement (5%) in Press
Ganey scores “How well your pain was
controlled” .
Nurses on the unit were given the “Knowledge & Attitudes Survey Regarding Pain” to evaluate their perceptions
regarding pain management. The results from the survey (n=25) were that 68% answered the questions correctly
and 72% felt their ability to care for patients with chronic pain was good/very good. Education was provided to
nursing staff regarding quality pain management to dispel any myths about managing patients with chronic pain and
complex medical diagnoses.
In November, the Comfort Menu (CM), listing available non-pharmacological items to promote comfort, was given to
patients on admission and the admitting nurse was responsible for explaining the use and purpose of the menu to
the patient.
Audits were performed to ensure compliance of distributing and explaining the Comfort Menu to patients. We found
that although patients stated they received the CM 91.3% of the time, they did not always receive education on its
use. Emails were sent to nurses to remind them of the importance of managing patients pain.
We also collected feedback from patients to determine if the CM was beneficial. Survey question results were: Was
the Comfort Menu a valuable resource (n=81)? Yes=91% No=2% and I have/will use the Comfort Menu (n=83)?
Yes=90% N0=4%
Patient discharge card questionnaire responses January 2015- April 2015 (n=105): “Were
you included in the management of your pain/comfort plan of care while you were hospitalized?”
98% responded yes and 2% responded no and “Was your pain/comfort managed to meet your
needs?” yes= 96% yes and no=4%
Ongoing audits are being conducted to ensure
consistent distribution of the Comfort Menu and
that nurses and providing education on its use.
The long term goal of this project is continuous
staff engagement on improving pain
management and comfort of patients.
Although the Press Ganey data is highly
variable, the use of the Comfort Menu, staff
education, and increased awareness may assist
with improving future patient satisfaction scores
related to pain.
The keys to success to the project were:
• The process was led by staff
• Listening to the patient/family voice
• Leadership support
• Partnering with the Pain Committee

In September 2014 Press Ganey patient
satisfaction pain scores on 6B, an Internal
Medicine Unit, were well below University of
Michigan Health System (UMHS) standards. In
August 2014 the unit data was 82.8 and in
September it was 81.4 and the Hospitals goal
was to achieve 88.8 or higher.
A team of nurses on the unit conducted a
quality improvement (QI) project to address
comfort and pain management with patients on
the unit by implementing a Comfort Menu
listing patient and nurse focused strategies.
The purpose of the project was to improve our
patients’ pain experience during their
hospitalization and potentially improve patient
pain satisfaction scores attained after discharge
