Attendees are invited to complete a brief anonymous survey after the webinar to evaluate the usefulness and teaching method; completing the survey allows the organizers to publish aggregate results. The survey is optional and will not impact the webinar participation.

1. Project CARE 4 Epilepsy
Thank you for joining our educational webinar. This webinar is one of a
series intended to provide education about epilepsy. These educational
webinars are given as a part of Project CARE 4 Epilepsy, funded by the
Health Resources Services Administration (HRSA), Grant #H98MC26260.
You are invited to participate in a research study. The research has been
approved by the Cleveland Clinic’s Institutional Review Board. The study is
comprised of a brief survey. The survey questions are designed to help us
know the usefulness of this type of programming and to evaluate the
method of teaching.
It is anticipated that the survey will take only a few minutes to complete.
The results of the survey information will be made public as aggregate,
group information and not on an individual basis. No information will be
presented that could identify you.
If you choose to complete the survey, you are giving us permission to use
your information. There will be no compensation for completing the survey.
However we are hopeful that you will complete the survey, since your
response may help us demonstrate the usefulness of projects like this.
Please know that your participation in this research survey is voluntary and
will not affect your participation within the educational webinar.
