This presentation was given to the national Public Health Stakeholder Forum for England on 21 January 2015. It covers my take on tobacco control priorities and work we need to do
This document discusses the changing landscape of tobacco control and the role of e-cigarettes. It notes that traditional smoking cessation methods are becoming less effective and that the population is segmenting into different tobacco and nicotine product users. The document argues that e-cigarettes can play a key role in tobacco control by providing a less harmful alternative. It contends that embracing e-cigarettes could help reduce smoking rates and prevent smoking-related deaths and disease, while not normalizing smoking. The document suggests that local public health services should support e-cigarette use as part of a comprehensive tobacco control strategy.
This document summarizes a presentation about public health in Hertfordshire. It outlines some of the major public health challenges in the county, including health inequalities and non-communicable diseases. It discusses how public health responsibilities have shifted to local governments and the new opportunities this presents. It emphasizes taking a lifecourse approach to health from conception to death and using proportionate universalism. The presentation discusses building partnerships across organizations to address the social and economic determinants of health.
Health and Social Care Devolution in Greater ManchesterCarl Peachey
Greater Manchester has a long history of collaboration between local authorities and health services. In 2014, it reached a devolution agreement with the UK government to take control of an estimated £6 billion annual health and social care budget by 2016. This would allow Greater Manchester to integrate services, shift care closer to homes and communities, and address major challenges like improving population health and closing health inequalities gaps. Key early achievements of the devolution include commitments to 7-day primary care access, decisions on the Healthier Together hospital reconfiguration, and a new public health leadership model.

11.9 million people in the UK lack essential digital skills needed for online health tools. Good Things Foundation addressed this through two phases of their Widening Digital Participation program (2013-2016 and 2017-2020). Phase 1 trained 221,941 people and engaged 157,391 more through signposting and events. Phase 2 included "pathfinder" projects targeting specific groups that found increased health screening, reduced GP visits, and improved well-being. Based on these, Good Things launched 27 pilot Digital Health Hubs providing a community space for digital skills training and health information searching, helping over 50% of users feel more confident and informed about managing their health.

Expo is the most significant annual health and social care event in the calendar, uniting more NHS and care leaders, commissioners, clinicians, voluntary sector partners, innovators and media than any other health and care event.
Expo 15 returned to Manchester and was hosted once again by NHS England. Around 5000 people a day from health and care, the voluntary sector, local government, and industry joined together at Manchester Central Convention Centre for two packed days of speakers, workshops, exhibitions and professional development.
This year, Expo was more relevant and engaging than ever before, happening within the first 100 days of the new Government, and almost 12 months after the publication of the NHS Five Year Forward View. It was also a great opportunity to check on and learn from the progress of Greater Manchester as the area prepares to take over a £6 billion devolved health and social care budget, pledging to integrate hospital, community, primary and social care and vastly improve health and well-being.

The Stolen Years: the case for action to reduce smoking prevalence among thos...UKFacultyPublicHealth
The document discusses the findings and recommendations of "The Stolen Years" report on reducing smoking among those with mental health conditions. The report found that smoking has a major negative impact on the health and life expectancy of those with mental illness. While attitudes are changing, many mental health staff still view quitting as incompatible with treatment and significant resources are spent facilitating smoking. The report sets an ambition for smoking rates among this group to fall below 5% by 2035, and outlines recommendations around leadership, empowering patients, staff support for quitting, and ensuring effective stop smoking services and support across all care settings. Next steps include disseminating the report and continued partnership work on research, guidance, and tools.

This document discusses e-cigarettes and their potential role in tobacco control. It notes that e-cigarettes have changed the landscape of tobacco control and could be an important tool for harm reduction. While more research is still needed, the current evidence suggests that the benefits of e-cigarettes for helping smokers quit may outweigh the potential harms, especially when compared to the proven harms of smoking tobacco. The document advocates for a public health approach that engages with e-cigarettes as a harm reduction strategy but continues monitoring the evidence for any risks.
The strategic plan outlines Public Health England's vision for the next four years to improve health outcomes and reduce health inequalities by 2020. Key priorities include tackling non-communicable diseases through prevention efforts like reducing tobacco and alcohol use, improving nutrition, and increasing physical activity. Public Health England also aims to address the wider social determinants of health and take a holistic, place-based approach through partnerships across sectors.

Prof Jim McManus discusses leadership models in public service and how they can fail or succeed across diverse communities. He reviews leadership approaches over the past 100 years, from the "great man" theory to more modern adaptive and transformational models. Effective leadership requires competence, strong culture, emotional intelligence, and a focus on justice. It also means fostering other leaders. Leadership should be inclusive and draw from diverse cultural worldviews rather than being based solely on white western men. Traditional virtues like justice, charity, fortitude, temperance and prudence can be recast through modern psychology to develop leaders.

This document discusses the impacts of poverty on health and wellbeing in the context of the COVID-19 pandemic and cost of living crisis. It notes increases in economic inactivity due to long-term illness, worsening life expectancy for some groups, and rising mental health issues like depression and anxiety. Addressing poverty requires a systems approach that considers socioeconomic factors like income, employment and social support, rather than just focusing on healthcare access. Key strategies discussed include addressing stigma, improving psychological flexibility, and taking a multi-pronged "Swiss cheese" approach similar to COVID-19 responses.

low birth weight presentation. Low birth weight (LBW) infant is defined as the one whose birth weight is less than 2500g irrespective of their gestational age. Premature birth and low birth weight(LBW) is still a serious problem in newborn. Causing high morbidity and mortality rate worldwide. The nursing care provide to low birth weight babies is crucial in promoting their overall health and development. Through careful assessment, diagnosis,, planning, and evaluation plays a vital role in ensuring these vulnerable infants receive the specialize care they need. In India every third of the infant weight less than 2500g.
Birth period, socioeconomical status, nutritional and intrauterine environment are the factors influencing low birth weight

Time-lapse embryo monitoring is an advanced imaging technique used in IVF to continuously observe embryo development. It captures high-resolution images at regular intervals, allowing embryologists to select the most viable embryos for transfer based on detailed growth patterns. This technology enhances embryo selection, potentially increasing pregnancy success rates.
Are you looking for a long-lasting solution to your missing tooth?
Dental implants are the most common type of method for replacing the missing tooth. Unlike dentures or bridges, implants are surgically placed in the jawbone. In layman’s terms, a dental implant is similar to the natural root of the tooth. It offers a stable foundation for the artificial tooth giving it the look, feel, and function similar to the natural tooth.
Cell Therapy Expansion and Challenges in Autoimmune DiseaseHealth Advances
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.

PHE suggested game changers
1. Behavioural science in the digital age, allowing us to reach people we
haven’t been able to reach before and providing personalised support on a
mass scale.
2. Place-based approaches, developing local solutions that draw on all the
assets of an area, integrate public services and build resilience in
communities.
3. NHS preventative services implemented at scale, working with NHS
England.
4. Transparency, so that everyone can access information on performance or
need and evidence on what works, to make better decisions. This will
include a partnership with the Chartered Institute of Public Finance and
Accountancy to focus on the return on investment from public health
interventions.
5. The contribution of employers to improving mental and physical health.
6. Using the concept of wellness (and developing measures of wellness) to
give a broader view of health.
4. www.hertsdirect.org
My take on PHE suggested game
changers
1. Behavioural science in the digital age, - focus on national tools which
can be disseminated locally
2. Place-based approaches, developing local solutions - this is our job,
you need to be clear what your role is
3. NHS preventative services implemented at scale, working with NHS
England – need for greater clarity about who does what
4. information on performance or need and evidence on what works – yes
5. The contribution of employers – you do national, we do local
6. Using the concept of wellness (and developing measures of wellness) to
give a broader view of health – do we even know what this means?
What about resilience and coping?

• Responsible for delivery of each of these
priorities working with TC Alliance partners
• Reports to Public Health and Localism Panel
and to HWb Board
• Project management approach to delivering
ambitions
14. www.hertsdirect.org
Work underway now
• Refresh ambitions
• Director of Public Health Report 13-14 focus on ‘Early
Avoidable Death’ and prioritises smoking-related
behaviour –
• New NHS contribution plan in development on back of
NHS Statement of Support
– “Three big wins for the NHS” System Leaders sign up
– Work with CCGs and JCT to include Tobacco Control and
Smoking Cessation in NHS targets (mental health, NHS
Hospital Trusts and Community Provider)
• Include Tobacco Control in PH communication and
marketing plan 2014-2015
• Develop harm reduction guidelines - to include e-
cigarettes guidance
15. www.hertsdirect.org
E cigarettes are a game changer in
tobacco control
• We have thought, read and reflected long and
hard on this
• The evidence points to the benefits being
greater than the harms
• We will keep evidence under review
• We will treat e cigarettes as a tool for public
health gain not a threat unless credible scientific
evidence says otherwise
• Most vapers care about their health, we cannot
ignore this important trend
16. www.hertsdirect.org
E cigarettes 2
• Gateway theories and fears on e cigarettes are
not supported by the evidence so far
• Yes lets protect children proportionately
• Is public health at the forefront of making the
trend to vaping the healthiest it can be, and if
there is evidence of harm, reducing or
protecting the public from it?
• Or are we in denial?
17. www.hertsdirect.org
Harm Reduction
• Our new harm reduction guidelines are
approved and working
• They include a range of harm reduction
strategies including working with e cigarettes,
cutting down to quit etc
• Our watchword is effectiveness and cost-
effectiveness. If it works for our population and
is safe, what legitimate reason do we have not
to engage with it?
