Title: Sense of Smell
Presenter: Dr. Faiza, Assistant Professor of Physiology
Qualifications:
MBBS (Best Graduate, AIMC Lahore)
FCPS Physiology
ICMT, CHPE, DHPE (STMU)
MPH (GC University, Faisalabad)
MBA (Virtual University of Pakistan)
Learning Objectives:
Describe the primary categories of smells and the concept of odor blindness.
Explain the structure and location of the olfactory membrane and mucosa, including the types and roles of cells involved in olfaction.
Describe the pathway and mechanisms of olfactory signal transmission from the olfactory receptors to the brain.
Illustrate the biochemical cascade triggered by odorant binding to olfactory receptors, including the role of G-proteins and second messengers in generating an action potential.
Identify different types of olfactory disorders such as anosmia, hyposmia, hyperosmia, and dysosmia, including their potential causes.
Key Topics:
Olfactory Genes:
3% of the human genome accounts for olfactory genes.
400 genes for odorant receptors.
Olfactory Membrane:
Located in the superior part of the nasal cavity.
Medially: Folds downward along the superior septum.
Laterally: Folds over the superior turbinate and upper surface of the middle turbinate.
Total surface area: 5-10 square centimeters.
Olfactory Mucosa:
Olfactory Cells: Bipolar nerve cells derived from the CNS (100 million), with 4-25 olfactory cilia per cell.
Sustentacular Cells: Produce mucus and maintain ionic and molecular environment.
Basal Cells: Replace worn-out olfactory cells with an average lifespan of 1-2 months.
Bowman’s Gland: Secretes mucus.
Stimulation of Olfactory Cells:
Odorant dissolves in mucus and attaches to receptors on olfactory cilia.
Involves a cascade effect through G-proteins and second messengers, leading to depolarization and action potential generation in the olfactory nerve.
Quality of a Good Odorant:
Small (3-20 Carbon atoms), volatile, water-soluble, and lipid-soluble.
Facilitated by odorant-binding proteins in mucus.
Membrane Potential and Action Potential:
Resting membrane potential: -55mV.
Action potential frequency in the olfactory nerve increases with odorant strength.
Adaptation Towards the Sense of Smell:
Rapid adaptation within the first second, with further slow adaptation.
Psychological adaptation greater than receptor adaptation, involving feedback inhibition from the central nervous system.
Primary Sensations of Smell:
Camphoraceous, Musky, Floral, Pepperminty, Ethereal, Pungent, Putrid.
Odor Detection Threshold:
Examples: Hydrogen sulfide (0.0005 ppm), Methyl-mercaptan (0.002 ppm).
Some toxic substances are odorless at lethal concentrations.
Characteristics of Smell:
Odor blindness for single substances due to lack of appropriate receptor protein.
Behavioral and emotional influences of smell.
Transmission of Olfactory Signals:
From olfactory cells to glomeruli in the olfactory bulb, involving lateral inhibition.
Primitive, less old, and new olfactory systems with different path

First of all, I’d like to inform you that Thailand is currently in a period of change in many ways including Thailand's economic structure that is entering the transition. It is developing a new model called "Value-Based Economy" which is a reform of the economy, focusing on the development of sustainable, prosperous and sustainable economy. The model consists of government’s policy of medical hub to drive the New Engine of Growth in term of New S-Curve in the new industry. This is an important mechanism for the expansion of the existing industry that can increase revenue for the country.
For national policy, the Thai government has been announced the policy of Medical Hub since 2004
At this moment, the government policy is also promoting health tourism policy that the product can make more income to Thailand
In 2015, The Ministry of Public Health (Department of Health Service Support) in cooperation with the Ministry of Tourism and Sports has appointed Medical and Wellness Tourism Promotion Board in order to push Thailand to become an international medical hub and come up with the Strategic Plans of Developing Thailand as an International Medical Hub (2017–2026) in the 10-year period (No. 3) which is comprised of 4 categories: (1) Medical Service Hub (2) Wellness Hub (3) Academic Hub and (4) Product Hub. The strategic plan was approved by the cabinet on September 13, 2019.
The plan was launched in the form of a Quick Win scheme in support of the government's policy to generate revenue for the country, raise the standard of international medical services as well as facilitate foreigners. The goals are the Top-Ten Healthcare Service Competition in the world and increasing income from Healthcare Services and Products.
We have launched the quick win project through the National Medical Hub Committee.
The Goals are for promoting foreigners coming to access more medical services, adding the value of health service business in Thailand and creating competitiveness in healthcare services.
Thank you for your attention.
For the competitiveness, This chart shows rough analysis of competitiveness among ASEAN countries between expertise/technology on vertical axis and price on horizontal axis.
And area of circle represents the amount of patients that can be treated.
hai international hospitals can maintain high quality expertise and technology at the price lower than the other countries.
When we see this information , the medical hub of other countries in Asia is various, in Singapore has focus on providing advanced medical services. Malaysia has focus on Muslim countries to attract Islamic patients. India is cheaper and focusing on health insurance group. For South Korea has emphasize on long-term vacation and cosmetic surgery. On the other hand, Thailand has been implemented medical hub policy continuously, and is ready with quality medical facilities, technology, medical staffs for services.
For national policy, the Thai government has been announced the policy of Medical Hub since 2004
