2. Σκοπός: Στόχος είναι να μελετηθεί η αποτελεσματικότητα της τοπικής αγωγής με κρέμα
ιβερμεκτίνης 1% στις βλεφαρίδες σε ασθενείς με χρόνια βλεφαρίτιδα από Demodex.
Μέθοδος: Μελετήθηκαν 12 οφθαλμοί 6 ασθενών στους οποίους
ανευρέθηκαν παράσιτα Demodex κατά την παρατήρηση με
μικροσκόπιο σε τουλάχιστον 2 βλεφαρίδες από κάθε βλέφαρο.
Οι ασθενείς εφάρμοζαν με προσοχή λεπτή στρώση κρέμας στη βάση
των βλεφαρίδων και στο δέρμα των βλεφάρων για 15 λεπτά 1 φορά
την εβδομάδα, συνολικά για 6 εβδομάδες. Παράλληλα φρόντιζαν για
την καθημερινή υγιεινή των βλεφάρων χρησιμοποιώντας αφρό
καθαρισμού με tea tree oil 1% μία φορά την ημέρα.
Αξιολογήθηκε το ερωτηματολόγιο SPEED (Standard Patient Evaluation of Eye Dryness) όπου οι
ασθενείς αξιολογούν τα συμπτώματά τους ανάλογα με την ένταση και την συχνότητά τους, καθώς
αξιολογήθηκαν και κλινικά σημεία όπως η ύπαρξη κυλινδρικών φολίδων γύρω από τη βάση των
βλεφαρίδων, ο χρόνος διάσπασης δακρυϊκής στιβάδας (TBUT), και η υπεραιμία του επιπεφυκότα και
του βλεφαρικού χείλους. Οι ασθενείς επανεξετάστηκαν την 3η
και την 6η
εβδομάδα.
3. Αποτελέσματα: Στις επανεξετάσεις των ασθενών κατά την 3η και 6η εβδομάδα, παρατηρήθηκε σημαντική
βελτίωση σε όλες τις παραμέτρους. Ο μέσος όρος του SPEED score βελτιώθηκε και ήταν 4.83±1.84 την
τελευταία ημέρα της θεραπείας από 12.67±5.05 που ήταν την πρώτη ημέρα (p value=0.002), και ο μέσος
όρος TBUT ήταν 8.33±1.03 s από 4.67±1.63 s που καταγράφηκε την πρώτη ημέρα (p value=0.012). Κατά
την εξέταση στη σχισμοειδή λυχνία παρατηρήθηκε πως είχαν μειωθεί σημαντικά οι κυλινδρικές φολίδες
στη βάση των βλεφαρίδων και η υπεραιμία του επιπεφυκότα και του βλεφαρικού χείλους είχαν
υποχωρήσει. Οι ασθενείς ανέφεραν πως είχαν ανακουφιστεί σε μεγάλο βαθμό από τα συμπτώματα της
βλεφαρίτιδας.
Συμπεράσματα: Όπως φαίνεται και από μεγαλύτερες μελέτες η ιβερμεκτίνη λόγω της αντιπαρασιτικής
της δράσης έχει σημαντικό ρόλο στην ανακούφιση των συμπτωμάτων των ασθενών με χρόνια
βλεφαρίτιδα από Demodex στους οποίους η χρήση μόνο ενός αφρού καθαρισμού δεν ήταν επαρκής.
Συνεπώς η χορήγησή της συνιστάται για την απομάκρυνση των παρασίτων Demodex.
