            • What is a patient registry?
                    In it’s simplest form, a registry is a list of all
                      patients in a physician’s practice who share
                      some characteristic, such as gender, age
                      group, or disease state. By tracking like
                      characteristics, physicians can better
                      organize patient’s care.

• Building registries take time
   • Start simple
   • Decide on what care you will track
   • Build on that care as you go along
   • Add a work flow in your office to identify who is going to
   enter the data
   • How are you going to use that data
   •Who will be responsible for extracting it from the registry
   • Allow yourself a reasonable amount of time before you put
   your data to use
