The document discusses potential issues with using MTBF/MTTF as the primary reliability metric for the defense and aerospace industries. It argues that MTBF/MTTF provides an incomplete view of reliability across the entire product lifecycle and can result in overly optimistic assessments. The document proposes using an alternative metric called Bx/Lx, which specifies the life point where no more than a certain percentage (like 10%) of failures have occurred. This provides a more comprehensive view of reliability focused on early failures. Overall, the document advocates updating reliability metrics and practices to better reflect physical failure mechanisms.
Operational excellence through lean & six sigmaHarsh Upadhyay
Operational excellence focuses on continuous improvement through lean, six sigma and lean six sigma approaches. It aims to optimize processes by focusing on customer needs and empowering employees. Caterpillar deployed lean six sigma across 27 business units to regain industry leadership. PASCO trained employees in lean six sigma to drive a culture change and focus on high potential markets and product innovation. ScottishPower used lean six sigma to improve customer service and sales operations to increase its market share.
The world has changed dramatically since LEAN and Six Sigma were popularized in the early 1990′s. Globalization, product proliferation, information technology, intense competition, and an activist regulatory environment have contributed to a rapid rise in complexity. As a result, many companies are finding that LEAN and Six Sigma aren’t delivering the results they expected. In this presentation, delivered by Chris Seifert at APICS 2013, we discuss a new approach that a select few companies are utilizing to achieve Operational Excellence in the face of complexity.
This document discusses an integral agile view of team health presented by Lyssa Adkins and Michael K. Spayd at AgileNZ 2015. It introduces an integral agile transformation framework with four quadrants - leadership and engagement, organizational culture and relationships, competencies products and technologies, and organizational architecture and structure. The framework is used to examine team health from an integral perspective focusing on the individual, team, environment and business agility. Key skills for developing team health including coaching, teaching, facilitation and using analysis tools are also discussed.
This document discusses the Kepner-Tregoe approach to decision making, which involves analyzing problems through considering timing, trends, and impacts. It outlines using Pareto analysis to prioritize problems based on their frequency and financial impacts. The document provides an example decision analysis where two potential sites for a new office are evaluated based on weighted factors like access to transportation, costs, and air quality to determine the best alternative.
Agile IT Operatinos - Getting to Daily ReleasesLeadingAgile
Getting to Daily Releases with Agile IT Operations. Devin Hedge, Enterprise Transformation Consultant talks to a group at Triagile about the Six Key Areas to focus on when attempting to transform IT Operations with Lean and Agile principles. The talk covers Service Engineering, IT Operations, and the Tier 1 Support/NOC organizations. Kanban, Service Management (ITSM), and what it means to have a DevOps orientation.
The document discusses operational excellence (OPEX) in the pharmaceutical industry. It traces the evolution of OPEX from earlier continuous improvement approaches beginning in the 1940s through its adoption and increasing implementation in pharma since the early 2000s. Key drivers for OPEX in pharma included financial pressures, FDA expectations for improved manufacturing processes, and the need to enhance competitiveness. By 2013, over half of pharma companies had implemented OPEX in at least some manufacturing plants. The document outlines best practices for sustainable OPEX implementation, including effective management systems, project governance, communication, and people focus.
The document discusses using A3 problem solving and kaizen (continuous improvement) methods to drive organizational change. It describes implementing kaizen memos to celebrate small improvements. Problems were analyzed using A3 thinking, with targets set and countermeasures identified and tracked. Leadership was turned "upside down" by having managers solve problems using coaching and A3 thinking. This drove significant improvements like reducing rework lead times from 14.6 to 5 days. The approach spread laterally through communities of practice and helped transform organizations.
The document discusses the role of the Release Train Engineer (RTE) in SAFe. The RTE facilitates important SAFe events like PI Planning, ART/Domain Syncs, system demos, and inspect and adapt meetings. The RTE also coaches the Scrum Masters to help ensure built-in quality, alignment, transparency, and effective program execution. The primary responsibilities of the RTE are to enable high quality delivery and continuous improvement.
Failure mode and effects analysis (FMEA) is a method to identify potential failures, determine their causes and effects, prioritize risks, and identify actions to address high-priority risks. An FMEA involves assembling a cross-functional team to analyze a process, product or service by identifying functions, potential failure modes and effects, causes, controls, severity, occurrence, detection ratings and risk priority numbers to prioritize improvement actions. FMEAs are used throughout a product or service lifecycle to prevent and reduce failures and risks.

An Assessment Model Study for Lean and Agile (Leagile) Index by Using Fuzzy AHPDr. Lutfi Apiliogullari
This document describes a study that develops an assessment model to evaluate companies on their level of implementing lean and agile principles and strategies. The study uses fuzzy analytic hierarchy process (fuzzy AHP) and decision making trial and evaluation laboratory (DEMATEL) methods to determine the important lean and agile criteria and their relationships. Lean and agile criteria are identified from literature and expert opinions. Fuzzy AHP is used to assign weights to the criteria. The model is applied to a company to calculate their initial lean/agile index. Improvements are then made and the index is recalculated to test the model. The goal is to help companies assess their situation regarding lean and agile implementation and identify areas for improvement.
The document discusses cost reduction strategies in the manufacturing industry. It identifies six main areas to focus on: marketing, new product introduction, strategic sourcing, manufacturing management, logistics network, and plan design source make deliver phases/functions. For each area, it provides actions that can be taken such as concurrent engineering, just-in-time purchasing, focusing on eliminating waste, and using lean tools like kaizen to increase productivity and quality. The overall strategies presented are aimed at reducing costs through greater efficiency, optimization of operations and processes, and strengthening supply chain relationships.
This document outlines strategies for cost reduction and an Industry 4.0 roadmap. It discusses operational excellence through techniques like Lean, Agile, and Six Sigma to improve productivity, quality, and efficiency. It also covers strategic sourcing best practices like selecting suppliers analytically rather than heuristically and adopting just-in-time purchasing. The document outlines production strategies for different product types and a hybrid model. It discusses operational efficiency methods including total productive maintenance (TPM), total quality management (TQM), standards, and skills development. Finally, it proposes a multi-phase roadmap to Industry 4.0 through standards, integration, advanced technologies, and change management.
The OpEx | Check-Up is an analysis platform developed through doctoral research that evaluates 6 factors and 21 sub-factors related to lean and agile manufacturing principles. It analyzes a company's current performance through 140 questions. The platform provides general evaluation, factor analysis, sub-factor analysis, and detailed analysis of specific areas like work place organization, JIT production, operational availability, product development, supply chain management, and management & organization.
The document outlines 7 principles for becoming a world class manufacturer: (1) each customer has different expectations, (2) forecasts are always wrong, (3) one size does not fit all, (4) supply chains compete rather than individual firms, and (5) communication is the lifeblood of organizations. It emphasizes that customers have varying expectations and that forecasts are unreliable, so manufacturers should adopt flexible strategies tailored to each customer rather than one-size-fits-all approaches.
The document discusses lean transformation and success. It outlines the goals of companies as revenue, profit, and cash flow. It then discusses lean concepts like Heijunka, Jidoka, TPM, flow, poka-yoke and various other lean tools. It shows how implementing these lean concepts through a pull system and standardization can increase profit from 3% to 10% by reducing costs like direct material costs, supply chain costs, and transformation costs. Finally, it discusses the business philosophy of continuous improvement through challenging oneself, genchi gembutsu, kaizen and teamwork to meet customer expectations.
