Excellence in Operations For Hospital Operations Group No 4Dr Rahul Deshpande
Rockland Hospitals aim for excellence in operations through quality management. They seek to comply with quality standards, continually improve health and safety, and enhance staff competence. Their vision is to deliver high quality medical services through a team of caring professionals. They measure quality using the five dimensions of service quality: reliability, responsiveness, assurance, empathy, and tangibles. For each dimension, they have identified specific quality standards and targets across different stages of inpatient and outpatient care. This includes standards for patient registration, diagnostics, surgery, post-care, billing, and more. The goal is to achieve excellence by meeting these quality measures.
The document discusses the healthcare industry in India. It notes that India's healthcare system is undergoing a transformation and growth over the next decade will be closely tied to reforms. The industry is expected to substantially evolve to help India achieve its long-term healthcare vision. It then provides an overview of the healthcare scenario in India including statistics on expenditure and infrastructure. It also discusses St. John's Medical College Hospital including its services, processes, pricing strategies, and quality initiatives to improve service delivery.

Outpatient Surgery Centers Market in India l In-depth Analysis l By Mirdul AminMirdul Amin Sarkar
The Outpatient surgery; which also known as ambulatory surgery, day case, or day surgery, is a kind of surgery procedure which require the patient to stay in the hospital/healthcare institution for less than 24 hours. The outpatient surgery does not require an overnight hospital stay
The term “outpatient” arises from the fact that surgery patients may go home and do not need an overnight hospital bed. The purpose of outpatient surgery is to keep hospital costs down, as well as saving the patient time that would otherwise be wasted in the hospital
Outpatient surgery has grown in popularity due to the rise in outpatient surgery centers and improved technology
With shorter medical procedure duration and fewer complications it makes sense to let patients go home sooner. About 55% of all surgical procedures are done on an outpatient basis
The outpatient surgery allows the patient to return home on the same day after a surgical procedure is performed, could be a novel platform for the Indian healthcare industry to grow
In coming years, the healthcare industry would have a number of players in the sector
The outpatient surgery platform/centers increase the accessibility and affordability of surgery to the common patient and the sector has huge potential in India

Organizational Culture, Good or Bad?
IGPA Building a Culture of Quality Ajaz Hussain_5 Sept 2015_Rferences minAjaz Hussain
Improving Confidence in Quality of Medicines . We make two products – medicine and evidence (documents) but many forget this and do not pay attention to documentation.
Level of attention to documentation is a “canary in a coal mine”
Breaches are irrational –”System 1 thinking” and cognitive biases.
Culture of Quality is familiar to all of us – a framework proposed
Quality Metrics – great idea – very much needed; but we are not yet ready for an FDA Guidance.
We must first address our collective blind spots; be confident that process validation truly ensures complexity is sufficiently reduced and that outcomes are predictable.

Claudio Foglini -- Senior Manager - Scalaris Economic Crime Intelligence AG
The document discusses the impact of the global recession from 2007-2010 on the automotive industry. It analyzes declines in global car sales, particularly in the US and European markets. Ford is presented as a case study of how a major automaker survived the recession through leadership changes, marketing strategies like the Ford Fiesta campaign, financial moves like loans and asset sales, and cost cutting. The conclusions drawn are that economic downturns negatively impact durable goods industries and that companies must be agile and adaptive to withstand recessions.
Greencore operates across four divisions: food-to-go, ready meals, grocery, and US business. Its food-to-go segment is the star, as it has high market share and strong growth in the UK sandwich market. Ready meals and grocery are cash cows that generate stable revenue despite limited growth in their markets. Greencore's US business is currently a question mark due to its low market share, but it has potential to become a star with Greencore's investments and the overall growth of the US convenience food market. Overall, Greencore is considered a star for its strong financial performance and strategic focus on growing its food-to-go segment.
The document discusses various types of retaining walls and their failure modes. It describes gravity, semi-gravity, cantilever, counterfort, and buttress retaining walls. The five modes of failure are identified as sliding, overturning, bearing capacity, shallow shear, and deep shear failures. Factors of safety are provided for each failure mode. Two case studies of retaining wall collapses are also summarized.
This document provides an overview of quality management systems and Six Sigma. It discusses basics of quality management including definitions of quality, dimensions of quality, and the scope of pharmaceutical quality management systems. It then introduces Total Quality Management (TQM) and its principles of continuous improvement. Six Sigma is defined as a statistical approach to process improvement, and its DMAIC methodology is explained. The key principles of Six Sigma are outlined as focusing on customer requirements, using data to identify process variation, improving processes to eliminate variation, involving multidisciplinary teams, and maintaining flexibility.
Six Sigma® is a business strategy,Focusing On Continuous Improvement: Understanding Customer Needs,Analyzing Business Processes, and Utilizing Appropriate Performance Measures And Statistical Methodology. The central idea behind Six Sigma is that if you can measure how many "defects" you have in a process.
You can systematically figure out how to eliminate them and get as close to "zero defects" as possible.

There are eight important dimensional qualities that can serve a huge part in the development of a business. Some of these dimensions are equally reinforcing and push business to do better and move to the next level. However, product service or good can rank high in one of the dimensions and rank low in another dimension. For medical Needs business to give better results they should focus on building in these dimensions, (Chvala & Johnson, 2017).
Performance: Medical Needs should ensure that they provide all the medical devices that the doctors prescribe to any patient. They should plan in such a way that all products are available in the required time.
Features: Medical Needs should have products or services that supplement their core functioning. Medical Needs can provide a free weighing machine and free BMI testing.
Reliability: Medical Needs should work on providing products on the stipulated time and the right quantities, with this, they will receive the benefit of reliability which every customer is looking for.
Conformity: Just as Medical needs stated regarding a provision of vendors in all areas and ensuring that all medical products are available to the patients, they should focus mainly on that.
Durability: Medical Needs should focus mainly on providing durable products and services. They should try not to fail customers at any point, (Garvin, 1987).
Serviceability: This business should mainly focus on the needs of the customers. They should offer fast services, courteous services, competent work, and original medical products.
Aesthetics: The outlook of each product is so important, so it is very important for Medical Needs to ensure that their products have a better outlook and an attractive customer service.
Perception: Customer’s perception is crucial for any business. Medical Needs should listen to their customer’s plea and perception towards their services and work towards bettering the services according to the customer’s perception, (Ross, 2017).
To achieve quality gains, the managers need to perceive things in a different outlook. For any upcoming business, it will be necessary to perform these eight dimensions to ensure that their business picks up and moves to the next level. Medical Needs have a classified goal which is going to be achieved along implementation of the eight-dimensional model.

Plan
What is your plan for evaluation of the strategies using performance improvement data and tracers? What tracers will you use? Include necessary detail to deliver key points and requirements, such as specific data collection methods, timeframes for evaluation, and intended re-evaluation.
Tracer method is a unique technique used by the healthcare organizations, to obtain a real time picture of quality performance from point of entry to discharge. A key part of The Joint Commission’s on-site survey process is the tracer methodology (The Joint Commission, 2017).. Some traditional tracer tools can be used for quality and safety improvement. The focus of these tools is on ….. and the plan for the evaluation of this initiative for fall prevention will use tracers in the following manner….
OR
To evaluate the identified measure is the 30 day readmission rate for patients, data twill be racked by system tracers which will be completed monthly by the Assistant Director of Nursing.
Plan Evaluation
How effective and sustainable is your plan? In other words, evaluate the effectiveness and the ease of use, timeliness, and efficiency of your plan for the progress and success of your initiative.
The plan to prevent falls is effective and sustainable with the involvement and collaboration of all team members by implementing the following strategies… The initiative will be evaluated by the following methods, post implementation…….
OR
Every three months this data will be compiled and analyzed to determine what actions were effective and ineffective. The complete study will take place over a one year period with the desired result of an 15% or below hospital readmission rate.
Use of Tracers
Individual tracers make the most sense to utilize for this proposal because these tracers are designed to “trace” the care experiences that a patient had during hospitalization. For example: in case of fall prevention, these tracers help to track the patient’s experience regarding safety, satisfaction of personal needs, hygiene, compliance of staff during care….. System tracers can be utilized as well, for example….
OR
System tracers provide information by tracking where in an organizational process breakdowns occur or exist and are a valuable tool in identifying where changes needs to occur. ...
The document discusses improving nurse scheduling in health systems using Lean principles. It describes how Lean and Six Sigma can help rethink care delivery processes and optimize nurse scheduling. The objectives are to understand the current scheduling process, identify areas for improvement, and implement changes to reduce costs. Some key areas discussed are readiness assessments, targeting excess costs, demand patterns in the industry, measuring and analyzing scheduling processes, practices, and technology to identify gaps and recommend solutions for improving efficiency and productivity.
Six Sigma is a structured process for defining, measuring, and controlling opportunities for improvement in repetitive processes. It can significantly reduce defects in healthcare by focusing on operational efficiency and translating patient needs into defined tasks. While Six Sigma is misunderstood in some healthcare organizations, it does not sacrifice clinical quality or cut costs at the expense of patients. Key elements for successful Six Sigma implementation include focusing on patient requirements, designing processes to meet them, measuring performance, involving staff, and using a systematic problem-solving approach with senior management support. Healthcare systems often look outside for experts to initially launch Six Sigma programs and improve integration of new technologies.

LAND USE LAND COVER AND NDVI OF MIRZAPUR DISTRICT, UPRAHUL
This Dissertation explores the particular circumstances of Mirzapur, a region located in the
core of India. Mirzapur, with its varied terrains and abundant biodiversity, offers an optimal
environment for investigating the changes in vegetation cover dynamics. Our study utilizes
advanced technologies such as GIS (Geographic Information Systems) and Remote sensing to
analyze the transformations that have taken place over the course of a decade.
The complex relationship between human activities and the environment has been the focus
of extensive research and worry. As the global community grapples with swift urbanization,
population expansion, and economic progress, the effects on natural ecosystems are becoming
more evident. A crucial element of this impact is the alteration of vegetation cover, which plays a
significant role in maintaining the ecological equilibrium of our planet.Land serves as the foundation for all human activities and provides the necessary materials for
these activities. As the most crucial natural resource, its utilization by humans results in different
'Land uses,' which are determined by both human activities and the physical characteristics of the
land.
The utilization of land is impacted by human needs and environmental factors. In countries
like India, rapid population growth and the emphasis on extensive resource exploitation can lead
to significant land degradation, adversely affecting the region's land cover.
Therefore, human intervention has significantly influenced land use patterns over many
centuries, evolving its structure over time and space. In the present era, these changes have
accelerated due to factors such as agriculture and urbanization. Information regarding land use and
cover is essential for various planning and management tasks related to the Earth's surface,
providing crucial environmental data for scientific, resource management, policy purposes, and
diverse human activities.
Accurate understanding of land use and cover is imperative for the development planning
of any area. Consequently, a wide range of professionals, including earth system scientists, land
and water managers, and urban planners, are interested in obtaining data on land use and cover
changes, conversion trends, and other related patterns. The spatial dimensions of land use and
cover support policymakers and scientists in making well-informed decisions, as alterations in
these patterns indicate shifts in economic and social conditions. Monitoring such changes with the
help of Advanced technologies like Remote Sensing and Geographic Information Systems is
crucial for coordinated efforts across different administrative levels. Advanced technologies like
Remote Sensing and Geographic Information Systems

Changes in vegetation cover refer to variations in the distribution, composition, and overall
structure of plant communities across different temporal and spatial scales. These changes can

4 “Six Sigma In Healthcare
Published: Jun 12 2004, 00:00 IST
Written by Mr. Gopal Kulkarni, Master Black Belt - Six Sigma and Founder & Managing Director,
Synagoge Knowledge Services
Six Sigma is a powerful problem solving technique that helps in reducing variation in any process by
using powerful statistical tools for analysis. Six sigma has been successfully used by various organizations
as an enabler for business strategies leading to huge additions in their yearly turnover and at the same
time improving customer satisfaction by quantum leaps. This article describes how this methodology can
be used in hospitals to enhance customer care activities and reduce wastage to improve the bottom line.
Healthcare In India
A huge percentage of the expenditure in the healthcare segment is borne by the government sector. A
large chunk of money spend on healthcare by private organizations is limited to urban areas. There is a
growth in investment from private organizations in the tertiary healthcare sector. They provide for the
multi-specialty hospitals and super specialty hospitals. A large part of the government funds go into
primary healthcare and the rest goes into the upkeep of large hospitals.
3. When we consider the large hospitals (private or government) there is increasing pressure on the
organization to better their performance in customer care and at the same time suffer huge price
pressures because of the increasing influence of insurance in healthcare. Rapidly changing technologies,
escalating manpower costs, increased regulations and consumer activism leads to further pressures to
reduce costs and enhance productivity and ROI.
How does six sigma help? Let us take an example of a patient undergoing a diagnostic examination. The
process steps for the diagnostic examination would be: Let us look at this process from two angles: One
from the customer’s (patient) point of view and another from the hospitals’ point of view.
From a patient’s angle his need would be that of a defect free report in the shortest possible time. He
would also expect reasonable charges for the service, comfortable environment and a courteous staff.
From the hospitals angle, the need would be to maximize the number of patients attended to within an
available time period so as to minimize the cost of examination and ensure customer loyalty.
Given these two needs you will find that the goals of both the hospital and the patient are one and the
same. If the hospital is able to minimize the time for turnaround, it can maximize the number of
examinations and utilize its diagnostic equipment to the fullest.
This is where six sigma can help. It can reduce the variation in the time taken for a particular process.
For example, let us try to reduce the variation in the time taken for a diagnostic scan. The time taken is
defined as the time period from when the patient enters the hospital / clinic to the time he gets the report.
When data of this time is collected over a period of time, it can be represented by a normal distribution
curve as shown below. As the curve becomes flatter, the variation is higher. Let us assume that the mean
time is 4 hours and standard deviation (a measure of variation) 1 hour.
Going by the characteristics of the normal curve it would mean that 99.73% of the patients get their
reports between one and seven hours. If the standard deviation is reduced by half it would mean that the
same process can give an output within 2.5 to 5.5 hrs.
How does one go about reducing the variation or even the mean? The output (time taken for scanning)
can be represented in a mathematical form:
Y is the output and the X’s are the variables affecting the output. The power of six sigma lies in
identifying those X variables that have the maximum impact on the variation and also those X’s that have
the maximum impac on the mean time. Using tools like hypothesis testing, simulation design of
experiments etc, we can arrive at the critical X’s. When we control these X’s, we can reach our goal of
minimizing the time taken for scanning.
What benefits does the hospital derive from this?
• Reduced output time, leading to better utilization of time
• Higher productivity, savings on manpower cost per patient
• Improved cycle time, leading to customer satisfaction and more loyalty
We can replace the diagnostic time by any other parameter like: Reduced pharma inventories, Reduction
in discharge time, Price realization, Cost/bed, Bed utilization, Plugging revenue leakage, Asset utilization
and Reducing patient queues.
Improvements
Six Sigma methodology can still be applied to improve these parameters. The robustness and foolproof
data collection needed for the application of Six sigma in services is achieved by the increased use of
4. information technology. Although, the solution itself is derived from the six sigma process the use of IT in
the method ensures consistency and reduction in variation.
In conclusion it can be said that the absence of a physical product as in the healthcare sector does not
stop or limit the usage of this excellent tool that can streamline your business and increase the turnover.
All that is required would be rigorous data collection and data based decision making.”
