Mycarematters is an online platform that allows users to upload non-medical information about their needs and preferences to provide healthcare professionals a more complete picture of the individual. This helps enable person-centered care. The platform is currently undergoing trials in UK hospitals and aims to launch fully in 2016, allowing any UK resident to have their information accessed quickly in case of hospitalization or medical appointments. The goal is to improve care quality by sharing important context about patients' lives beyond their medical conditions.

2. Why do we need
Mycarematters?
It is increasingly
understood that
treating the whole
person leads to better
outcomes, which is
why the CQC wants to
see evidence of
person-centred care…
?

3. …and you can’t deliver
person-centred care
without knowing
about the person.

4. Hospitals are starting to
use tools to share
information about a
person, but it can be
challenging to collect
the data in the first
place, if a patient is
unable to communicate.

5. Patients or their carers might have
completed a Care Charts Pocket Chart or a
This is Me document but it can get left
behind, mislaid or filed.

6. Friends and family may have the answers,
as might care home staff if the person
usually lives in a care home, but there are
often delays before these people can be
contacted, time for mistakes to be made,
damage to be done.

7. So it seems obvious that
we need an online
record of people’s non-
medical needs and
preferences, that
hospital staff can
quickly and easily
access from the minute
the person is admitted
to the ward…

8. Mycarematters offers exactly
that: a quick and easy way to
find out about a patient’s
preferred food and drink, their
usual level of mobility and other
information that will allow staff
to engage with them, and ensure
they are treated in a person-
centred manner, without delay.

9. Information is uploaded
and owned by the person
or their advocate, so they
are in complete control
over what they share with
healthcare staff.

10. What stage are we at?
We’ve on our way!
Trials are scheduled to
begin in April, working
with three local
community hospitals…

11. By May we’ll be inviting people to
give it a go and give us their
feedback…

12. We’ll undoubtedly need to make changes,
refine and go on to do more testing…

13. And at some point during 2016 we
shall GO LIVE!!!

14. That means that any
person, living anywhere
in the UK, will be able to
upload their non-
medical information on
to Mycarematters. (A
carer, friend, family
member or care home
manager can do so on
their behalf.)

15. So they can then be
confident that, if they
have to go to hospital or
visit their GP or clinic of
any kind, all healthcare
professionals will have
access to their needs
and preferences, even if
communicating is a
challenge.

16. Staff will be able to use
a laptop, tablet or
phone with internet
access to tap in the
person’s unique code,
and view or print off
their needs and
preferences.

17. We’ll be using
encryption and the
latest security
technology to keep
people’s information
safe, and only account-
holders (or their
advocates) will be able
to amend or update
their information.
