1. MUMPS (EPIDEMIC
PAROTITIS)
• ACUTE CONTAGIOUS VIRAL INFECTION
• UNILATERAL OR BILATERAL SWELLING OF THE SALIVARY GLANDS USUALLY THE
PAROTID
• SUBMAILARY AND STIBLIIGUAL GLANDS ARE-OCCASIONALLY INVOVED.
• BUT SELDOM WITHOUT PAROTIDVOLVEMENT ALSO
• MENINGES, PANCREAS AND GONADS MAY ALSO BE INVOLVED
• ALITHOUGH IT USUALLY A DISEASE OF CHILDHOOD MUYPS MAY ALSO AFFECT
ADULTS SUCH CASES COMPLICATIONS DEVELOP
• INCUBATION PERIOD: 2 TO 3 WEEKS
2. PATHOGENESIS
• Once transmited through droplet nuclei orsaliva or fomites,
• it Starts replicating in the respiratory epi-thelium.
• It spreads to local lymph nodes
• Subsequently develops viremia
• affected area shows perivascular and interstitial mOnonuclear ceall infilterates with edema
• Necrosis of acinar and epithelial duct cells are seen In salivary glands and in the germinal epithelium Of
seminiferous tubules.
3. CLINICAL FEATURES
The disease usually preceded by theonset of
• Headache chills moderate fever, vomiting pain below the ear.
• These symptoms are followed by a firm,some what rubbery or elastic swelling of the salivary glands
• Frequently elevating the ear which usually lasts about one week
• produces pain upon mastication
• bilateral parotid in70% of the cases
• Pain and tenderness may be severe during the rapid phase of parotid enlargement
• swelling reaches maximum in 3 days
• Remain in peak for 2 to 3 days then gradually subsides
• The submandibular gland may also involve separately or in conjunction with parotid gland
• sub lingual rare
• presternal edema in some cases papila of opening of parotid will be reddened and puffy
4. DIAGNOSIS
• Virus Can be isolatcd rom saliva and throat swabs two davs before or 7 days after the onset
• And from CSF
• Confirmation:complement fixation test
• :ELISA
• :Heamagglutination inhibition
• Serum amylase elevated in both parotits and pancreatitis.
5. COMPLICATIONS
• Other organs of the body may be affected
• These include the testes, ovaries pancreas, mammary glands, and occasionally the prostate,epididymis, and
heart.
• When mumps involves the adult male,orchitis is a great danger
• This orchitis is usually unilateral, but occasionallycomplete sterility results.
• An acute pancreattis often causes an elevationin serum lipase.
• Serum amylase is also elevated but this is regardless of pancreatic involvement
• Meningoencephalitis ,deafness, mastoiditis are also occasional
• rare complicatiens like nerve deafness facial parabasis cerebral ataxia encephalitıs have aalso been reported.
• When occurs in the 1st trimester.ot pregnancy it results in abortion
