Internuclear ophthalmoplegia is caused by lesions in the medial longitudinal fasciculus located in the midbrain or upper pons. It is characterized by impaired adduction of one eye and rhythmic nystagmus of the abducting eye when attempting lateral gaze. The side of the lesion corresponds to the affected eye's adduction, not the side with nystagmus.
