Knowledge is power. Despite extensive investments in digital health technology, navigating the health system online is challenging for most citizens. Also for eHealth, the “Inverse Care Law” proposed by Hart in 1971, seems to apply. Availability of good medical or social care services and tools online, varies inversely with the need of the population. The low adoption of eHealth services, and persistent disparities in health triggers a call for multidisciplinary action. Barriers and challenges are not to be underestimated. Culture, education, skills, costs, perceptions of power and role, are essential for multidisciplinary action. This comes together in digital health literacy, which ought to become an integral part to navigate any health system. Patients living with an implanted device or coping with persistent, chronic disease such as diabetes, as well as citizens engaged in self-care, caring for an elderly relative, a neighbor, or their child with illness or deteriorating health, need a digital health compass. The panel will engage the audience to elaborate on a vision for this personal, digital health compass and drive advancement in health informatics and digital health standards. The transformative power of health data fueled by targeted digital health literacy interventions can be leveraged by open, massive, and individualized delivery. This way, digital health literate, confident patients and citizens join health professionals, researchers and policy makers to address age-related health and wellness changes to shape the emerging precision medicine and population health initiatives. Kate Bukowski ProCare Health Limited (Friday, 3.00, Innovation in Practice 3) Explores two domains which, on the surface, may seem very different – the US policy directing metrics to measure HIT adoption across the nation and the implementation and utilisation of a practice management system auditing tool (Dr Info) within a region of New Zealand’s general practices. Yet, when looked at under a different view, there are similarities which drive the use of HIT to improve results that engage patients in self-management of their health and clinicians in population health management. Both can improve the delivery of healthcare and provide value for money but both also need support and systems that foster innovation at a patient and practice level. This will continue to be an issue as the health workforce ages and becomes smaller. It is therefore crucially important how we utilise and train our workforce, that they have HIT systems that support their work with the approach that through showing results and improving results, clinicians will engage and want to use the technology to improve their practice. At the same time patients will need support to self-manage their conditions through the utilisation of patient portals and other innovative HIT initiatives.

On average, healthcare collects data about patients three times per year from which providers are expected to optimize diagnoses, treatments, predict health risks and cultivate long-term care plans. Where’s the data about patients’ health from the other 362 days per year? McKinsey ranks industries based on their Digital Quotient (DQ), which is derived from a cross product of three areas: Data Assets x Data Skills x Data Utilization. Healthcare ranks lower than all industries except mining. It’s time for healthcare to raise its digital quotient, however, it’s a delicate balance. The current “data-driven” strategy in healthcare is a train wreck, sucking the life out of clinicians’ sense of mastery, autonomy, and purpose. Healthcare’s digital strategy has largely ignored the digitization of patients’ state of health, but that’s changing, and the change will be revolutionary. Driven by bio-integrated sensors and affordable genomics, in the next five years, many patients will possess more data and AI-driven insights about their diagnosis and treatment options than healthcare systems, turning the existing dialogue with care providers on its head. It’s going to happen. Let’s make it happen the right way.

April 3, 2019 Digital innovation is transforming health care, and the amount of digital health care data being generated will likely have increasing research utility over time. Despite the seemingly logical and inevitable application of health care data from deceased persons for research and health care both now and in the future, the issue of how best to manage posthumous medical records is currently unclear, including elements of resource governance, issues of law, and infrastructural challenges. This presentation explored current issues surrounding how to manage the medical records of the dead, integrating evidence from the field of body donation to inform and guide the discussion on the utilisation of posthumous medical information. It also delivered results from a year-long study on posthumous health care data utility that explored the views of the general population on the use of posthumous medical records, which showed a centrally collated and government-governed resource of posthumous health care data was almost universally supported, with varying caveats around how such a resource should be utilized. The Digital Health @ Harvard series features speakers from Harvard as well as collaborators and colleagues from other institutions who research the intersection between health and digital technology. The series is cosponsored by the Berkman Klein Center for Internet & Society at Harvard University and the Petrie-Flom Center for Health Law Policy, Biotechnology, and Bioethics at Harvard Law School. The goal of the series is to discuss ongoing research in this research area, share new developments, identify opportunities for collaboration, and explore the digital health ecosystem more generally. Thoracotomy is the surgical procedure with an incision made to access the pleural space and the contents of the thoracic cavity. Given the structures to be accessed with in the cavity, different incisions have been established to easy the procedure and these incisions qualify the types of thoracotomies to be studied hereunder. PRE OP OPTIMIZATION FOR SPECIFIC FACTORS The outcome of surgical procedures is not measured only by clinical end points but also shorter stays and lower costs. Patients’ discharge is delayed commonly due to inadequate pain relief, infection, arrhythmias, prolonged air leak and debility. Many complications that occur from thoracic operations can be anticipated. An aggressive preoperative work up mitigates morbidity and shortens convalescence. APPROACH CONSIDERATIONS There are about three principles that can guides the choice of the thoracotomy incision to be used I. Adequate exposure must be achieved. The choice of incision is aided by a thorough understanding of the surface anatomy and a comprehensive review of the radiographic images that are obtained preoperatively. II. Chest-wall function and appearance should be preserved to the extent possible. This principle include non-spreading video-assisted thoracoscopic surgery (VATS) procedures, muscle-sparing techniques, avoidance of excessive rib retraction, and rib preservation when possible. III. The third principle is that closure must be meticulous and appropriate. Strict layered closure is the rule for thoracic surgical incisions. Every effort should be made to approximate the individual divided chest-wall muscles in appropriate layers; otherwise, a significant delay in the recovery of range of motion (ROM) may result. Care must be taken to avoid over approximating the ribs and to prevent an override; this will help minimize postoperative pain. POST-OPERATIVE CARE AND MONITORING
