This document outlines the certification criteria and accomplishments of an individual seeking Master Black Belt certification. It includes 7 criteria such as demonstrating leadership, mastery of Six Sigma methodology, building a continuous improvement infrastructure, and training others. For each criterion, the minimum requirements are listed along with the individual's specific accomplishments, such as mentoring a certain number of projects, participating in networks, and completing Black Belt and MBB training over a year prior to certification. Overall, the individual has exceeded the requirements across all criteria based on their leadership experiences and Six Sigma training history.
