This document contains a questionnaire about brand awareness of Kandamkulathy Vaidyasala, an Ayurvedic medicine company. The 14-question survey asks consumers about which Ayurvedic brands come to mind, their familiarity with and purchase history of Kandamkulathy products, and how they would rate the brand's quality and likelihood of future purchases. It also collects suggestions and complaints. The questionnaire aims to collect data for a study on Kandamkulathy's brand awareness among Ayurvedic consumers.
