- 15. Training • Employers mustprovide a training program for each employee using ladders and stairways. The program must enable each employee to recognize hazards related to ladders and stairways and to use proper procedures to minimize these hazards. For example, employers must ensure that each employee is trained by a competent person in the following areas, as applicable: • The nature of fall hazards in the work area; • The correct procedures for erecting, maintaining, and disassembling the fall protection systems to be used; • The proper construction, use, placement, and care in handling of all stairways and ladders; and • The maximum intended load-carrying capacities of ladders used. • In addition, retraining must be provided for each employee, as necessary, so that the employee maintains the understanding and knowledge acquired through compliance with the standard. 1926.1060(a) and (b)

Ladder Safety -Prevention • Climbing and descending: – Check to see if ladder is secure. – Have both hands free to hold on to the ladder, or at least one hand. – Face the ladder when climbing or descending – Keep the area around the top and bottom of ladders clear. – Only one person on a ladder!

- 62. Unstable surfaces • Slip-resistantfeet must not be used as a substitute for the care in placing, lashing, or holding a ladder upon a slippery surface. 1926.1053 (b)(7)

Height Requirement • Aladder (or stairway) must be provided at all work points of access where there is a break in elevation of 19 inches (48.2 centimeters) or more except if a suitable ramp, runway, embankment, or personnel hoist is provided to give safe access to all elevations. 1926.1051(a)
- 64. CPSC Safety Tips •Use a ladder that is the proper length for the job. Proper length is a minimum of 3 feet extending over the roofline or working surface. The three top rungs of a straight, single or extension ladder should not be stood on. • All metal ladders should have slip-resistant feet. • Never leave a raised ladder unattended. • Follow use instruction labels on ladders • Make sure the weight your ladder is supporting does not exceed its maximum load rating (user plus materials). • The ground under the ladder should be level and firm. Large flat wooden boards braced under the ladder can level a ladder on uneven ground or soft ground. A good practice is to have a helper hold the bottom of the ladder. • Do not place a ladder in front of a door that is not locked, blocked or guarded. • Keep your body centered between the rails of the ladder at all times. Do not lean too far to the side while working. • Do not step on the top step, bucket shelf or attempt to climb or stand on the rear section of a stepladder.

An accident occurred on November 19, 2010 in the main electrical room of the ENS building on the UT Campus, Austin, TX. The OSHA Austin area office opened the accident investigation the same day. The worker was a temporary worker for Able Body Labor assigned to Western States Fire Protection Company. Western States Fire Protection Co. provided both the equipment and the supervision. A laborer was painting a sprinkler pipe while standing approximately on the forth step of a fiberglass step ladder. This would mean that his head fell approx. 10 feet (3m) onto the concrete floor. He died later in the hospital from head injuries. No one saw the accident nor the victim on the ladder immediately before the accident. The ladder was displaced and damaged as a result of the accident. Work was being done to get the space ready for new tenants. The work included construction of a new handicapped building entrance, construction of a new foyer, and construction of a common bathroom area. Contacts # 3, 6 and 8 were working on the common bathroom area. Each employee was working independently on different sections of the bathroom. The bathroom was framed in at this point, with some sheet rock already installed. They were continuing installation of sheetrock. Contact # 3 was working near the outside windows (west), Contact # 8 was working in the opposite side of the bathroom (east), on the upper, more intricate pieces of the wall, and Contact # 6 was working on the top of a utility closet in the bathroom (central section of bathroom). Each contact could see the other one working. Contact # 8 was utilizing a step ladder to reach the higher portions of the bathroom. The heights were greater than 10 FT. The step ladder being used was a Husky (Model 01-41200-00) 12 FT fiberglass step ladder. The step ladder was propped up against the wall near the corner so contact # 8 could reach the upper corner to finish the pieces surrounding the existing structural steel of the building. Contact # 8 was not utilizing the step ladder in accordance with the manufacturer's recommendations (located on a sticker on the ladder). The recommendations include making sure the ladder is fully open, spreaders secure, pail-shelf in position. It also states to place it on a firm level surface with a secure footing. Contact # 8 remained in the common bathroom area, working, while Contacts # 3, 5, 6 and 7 took their lunch breaks at approximately the same time, between 11:00 AM and 12:00 PM. Shortly after Contacts # 3, 5, 6 and 7 returned from break, Contact # 2 returned from running errands. Contact # 2 questioned the employees as to the whereabouts of Contact # 8. There were two auto dealership employees washing exterior overhead glass garage door windows outside the dealership's service area, working at times from 6 foot step ladders. One of the two employees finished and went back inside to put his ladder,towel and window spray cleaner bottle away. During this time a manager inside the dealership service area observed the second employee reaming outside walking slumped and wobbly away from the area last seen on his ladder some approximate 20 feet to a showroom entryway door. The manager lost visual contact as the worker crossed behind a building structure. A second in store sales manager stated he shortly after this time herd a noise as if a bird hit the showroom window (20 feet away from the ladder),he turned quickly to see the remaining outside window cleaner laying on the ground next to the showroom entry door. The employee was unconscious and never regained consciousness dieing some 9 days later of a reported traumatic brain injury. The dealership officials mentioned a previous episode of the worker becoming light headed and 911 being called months ago, suspecting in this case the worker had some type of seizure and just fell as he stared to enter the door. EMS medical reports indicate the mechanism of the head injury was from fall from one to five feet. Hospital records later indicated the worker fell from the second or third rung of a step ladder while descending. There were no eye witnesses to the event and the employee involved in the incident never regained consciousness to describe his accident so all accounts of the incident come from second hand observations before or after the incident, conjecture of persons after the accident and medical analysis of the workers head injury and most predictable cause based on surrounding work practices and conditions. The company started work at 6AM on 9/14/10. One employee who was a journeyman sheet metal mechanic was assigned to work in a hallway on the 5th floor of the Childrens Tower being constructed at Johns Hopkins Hospital. The employee was working from an 8 foot fiberglass step ladder. It is unknown what step he was standing on. The ceiling grid was 9 feet above the floor. His work area was approximately two feet above the ceiling grid. The employee apparently placed his tools up near the work area above the ceiling grid. The ladder was set up on the concrete floor which was level and free of debris. The ladder was facing east in the hallway and was set up next to the wall which was made of drywall. There was no foot traffic by the ladder and the ladder was in excellent condition with no defects and good tread on the bottom of the legs. At about 7:40AM the employee fell from the ladder up against the wall with his feet and body knocking the ladder down to the floor. There was no indication that the ladder hit the opposite wall. There was no yell or scream from the employee. The employee landed at the foot of the ladder face up with both legs up on the ladders locking arms. No one saw the incident occur. Two employees who were working nearby inside other rooms apparently heard the ladder hitting the floor and came out to see what the noise was and found the employee. He was breathing but unresponsive and blood was coming from his left ear with a small amount of blood on the floor. Rescue personnel arrived and the employee was treated and then transported to the emergency room at Johns Hopkins Hospital. He was pronounced brain dead at 7PM on 9/14/10 and died between 11:30AM and 12:00PM on 9/15/10. He was an organ donor and the family did not want an autopsy performed. The victim had a history of heart related problems including having stints placed in his heart. He also suffered from emphysema. There is no indication that there was anything wrong with the ladder or that it was set up unsafely. Apparently the employee had some sort of health issue occur that caused him to fall from the ladder to the concrete floor. - #28 Ladders come when several warnings that users should be trained on before use. Many falls occur when a worker leans past the side rails to reach further. The ladder then shifts or falls causing the worker to fall. On Tuesday, March 2, 2010 at approximately 1647 hours, a technician (EE#1) was in the process of installing an internet cable drop at a customer home. EE#1 was about to attach a "P" hook at the corner of the house and was placing his 28' extension ladder against the house, but it was too long and placed it at an angle from the house and extending it onto the driveway adjacent to the home. The ladder was placed at a pitch that was more than the 4 to 1 ratio of the working height of the ladder. While the EE#1 was on the fourth rung down from the top, the feet of the ladder slipped out causing the ladder and EE#1 to fall down. Employee #2 suffered severe head trauma from an un-witnessed fall from an extension ladder or garage roof at 4351 Mensha Place, San Diego. Employee #2 had been working as a painter at this location. Employee #2 received emergency medical treatment from ALS Unit 24 and was transported to Scripps Memorial Hospital where he passed away. Alex Painting is an unlicensed Painting Contractor. Alex Painting has no business license, CSLB license or Workers Compensation Insurance. The employer did not report the accident to the Division in violation of T8 CCR 342(a). The Division opened an investigation with the employer on 09/28/10. At the time of the accident Employee #2 was working as a painter and is reported by Employee #1 to have been working with him on the roof over the garage. The garage roof is a first story roof with a perimeter measured to be 8 feet 8 inches above the driveway. Just previous to the accident Employee #1 left Employee #2 on the roof to use his phone. Employee #1 stated that after leaving Employee #2 he went to the side of the house, out of site from employee #2, for approximately 25 minutes. While on the phone Employee #1 stated he heard a noise that he described to be that of a ladder falling. Upon investigation Employee #1 found Employee #2 with injuries on the driveway. He suffered much bodily damage, (concussions, broken ribs, and internal). It was reported he had suffered a heart attack by the crew. The autopsy showed differently. The crew was hispanic from Guatamola and there was some confusion as to the First Aid response. The crew stated they did not call 911 because they thought the phone would ring in the place they had bought them (Florida, Louisianna, Texas. They also took the time to remove the shirt of the victim, clean him up, and put on another shirt before loading him into PU for transport. They had set and secured one section of exterior wall and began to set another. The employee placed a extension ladder against the interior side of the exterior wall that was already set and secured to the deck and wall section to the right. They had also placed a 2 inch X 4 inch X 12 foot brace that was attached in the center of the wall section that the ladder was leaned up against. The employee climbed 13 feet up the ladder and began to secure the second two story section of exterior wall to the first using a pneumatic nail gun. As he was doing so the section of exterior wall that he was leaned against with the extension ladder fell 23 feet to the ground as he rode the ladder down and landed on the wall, ladder, and ground. As a result the employee suffered massive internal chest injuries including 4 holes to the heart and head trauma. The victim was transported by ambulance to Montgomery County General Hospital by ambulance and later transferred to Medstar in DC for further treatment. The Medstar physician pronounced the employee deceased as a result of the injuries sustained from the fall at 10:56 pm on 05/13/10. Employee #1 rotated the ladder 180 degrees while standing at the edge of the roof. The fly (top) section of the extension ladder began to slide uncontrolled down the base section of the ladder. The fly section continued to slide until one ladder lock penetrated a ladder rung (8th from the bottom). At that time, the ladder stopped sliding immediately. Employee #1 fell from the ladder landing on the back of his head/neck. Employee #1 was pronounced dead at the local emergency room. The roof was approximately twenty-two feet above the ground. The employee was found by a visitor of another property that was adjacent to where the employee was working. The telescopic handler was being used to support a metal rack which was supporting the drum assembly of the commercial roll-up door. It was determined that the metal rack which was supporting the drum assembly had not been approved by the manufacturer of the powered industrial truck. The hand tools, which were all serviceable, were used by the employee to assembly other component of the roll-up door. The ladders, both of which were serviceable, measured two feet in width and were placed approximately centered between the outer wall and the edge of the concrete platforms. The employees were working from the ladder using the hand tools to finish the installation of the roll-up door. After gathering details, documents and employer/employee interviews, it was determined the victim sustained the fatal injury while performing assigned work activities involving the installation of the roll-up at the new international terminal and concourse. The victim was working with another Air Control DJ, LLC. employee installing sheet metal on the frame of the exhaust hood over one of the robotic cells. The victim and the other employee were working off of aluminum extension ladders that were leaning against the exhaust hood frame. The edge of the frame was 11 feet high. The victim and the other employee were drilling and screwing down the sheet metal to the frame of the exhaust hood. The victim's co- worker ran out of screws and stopped work, went down his ladder and walked approximately 60 feet to where the screws were in a box, picked up some screws and walked back to his ladder. When the employee returned to his ladder he noticed that the victim's ladder was slid back but he didn't see the victim. The employee looked down on the floor and found the victim laying face down with his head bleeding. The employee ran to one of the nearby Saehaesung employees and told them that there had been an accident and that an ambulance was needed. An ambulance was called and arrived on scene and transported the victim to a local hospital in Andalusia. The victim was later transferred to a Pensacola Florida hospital for further treatment where he later passed away. The employer failed to provide a training program for each employee who used ladders that enabled each employee to recognize hazards related to ladders. An employee used a 16 foot high fixed ladder to access the building's roof top at the work site. The work to be accomplished was the repair of an electrical short on the roof's exhaust system (mounted blower motor). This work area was 30 feet from the roof's edge, and not a fall concern. The employee did not carry a tool box up the fixed ladder, he only took up the tools required, wire strippers, screw driver and pliers. There were no eye witnesses to the accident, therefore it is unknown where the employee fell from, the ladder or the roof and what he was doing at the time of the accident. Since the work was completed, it is thought the victim fell from the ladder while descending to the ground. The victim fell onto an above ground water line and broke the line off below the surface of the ground. The sound of the water splashing back onto the asphalt alerted a neighbor who contacted the manager of the restaurant where the accident occurred. This neighbor contacted 911. The building manager said the victim was lying face down with his head turned to one side and response to the manager's questions was poor. Local Police Department was contacted at 1623 hours, dispatched at 1628 hours, arrived on the accident scene at 1633 hours, and cleared the site at 1700 hours. The paramedics arrived on scene and transported the victim to the local trauma center. The injuries sustained by the victim suggests that he fell backwards onto the two inch diameter gate frame of the chain link fence, bending the top of the gate. The victim then fell onto the a water pipe facing downward. The victim's head and back sustained injuries. The victim's spinal cord and aorta were severed and the victim received several broken ribs. The employees were approximately 95ft above ground level, working from a scaffold on the interior of the Spheroid's shaft. The crew of employees were fitting the seam on the newly added section. With the new section secured, Employee #1 climbed a fixed ladder to access additional tools needed to complete the job. In order to reach the tools, Employee #1 had to climb approximately 8ft up the fixed ladder, through a manhole hatch, and onto the "Top Platform." The three other crew members stated that Employee #1 opened the hatch and began to climb through it. Before Employee #1 could reach the "Top Platform" he lost his footing and fell. The attachment appears to be an end loader type attachment. Both employees had gained access to the bucket of the end loader by a 20 foot extension ladder. Both employees were using the bucket as a work platform. Employee's were working between 10 and 12 foot above the ground. Employee #1 was attempting to transfer from the bucket to the extension ladder. According to employee #2 the extension ladder did not have the rung locked assembly for the fly section of the ladder locked. When employee #1 attempted to transfer the fly or top section of the extension ladder slid down approximately 2 rungs. The ladder started to tip and employee fell 10 - 12 foot to the ground. Employee #2 was still in the end loader of the tractor when this occurred. Employee #2 was stranded. Employee #2 yelled, screamed and whistled to get someone attention. However he did not get anyone attention. Employee #2 finally shook the ladder and got the lock assembly latched on to a rung of the extension ladder. Employee #2 descended the ladder saw that employee #1 was not moving. Employee #2 ran to another employee and sated to call rescue that employee #1 was severely injured. This incident occurred just out of two small towns. Both towns sent rescue. Also city and county police departments arrived on site. Mercy Life Flight arrived also. However, Employee #1 was transferred to Belmond Hospital in Belmond Iowa to stabilize his condition. After Employee #1 was stabilized he was transferred to Mercy Hospital in Mason City, Iowa. On 1 October 2010, at or about 1155 hours, E1 was at the site climbing the manufactured attachable scaffold ladder on the 25' high bricklayer's scaffold located in Area 1 of Building A (Wrestling Room). This is the normal procedure for an employee to gain access to the working level of the scaffold. After E1 had climbed ~25' to the fifth tier of the scaffold (working level), E1 went to climb off of the ladder and onto the scaffold. According to W1, while holding onto the third rung from the top of the ladder with his left hand, E1 placed his right hand on the midrail of the scaffold and was presumably going to enter between the top rail and the midrail (the standard practice at the site). EE #1 and EE #2 accessed the area using a fixed ladder going from ground to a platform over the pipe rack. The line was not shut down and remained on-stream during the repair work. This would require the use of an engineered clamp to be installed and a sealant pumped into the clamp to seal the leak. At the time of the incident the line contained 16.78% of H2S, which equates to approx. 160,000 ppm of H2S. Both employees were wearing tight fitted airline respirators that were attached to a 16 pack of cylinders containing grade D breathing air. Approximately 1.5 hours into the pipe repair operation, EE #1 attempted to remove himself from the area when he collapsed on the platform. First responders were there within 3-4 minutes. EE #1 died of an overexposure to hydrogen sulfide. The weather did not play a role in this accident. The well pit is a brick-lined, hand dug well dating back 100-120 years, originally powered by a wooden windmill but adapted to a deep well pump system. The old pump had not been used in 10-12 years and was rusted in place inside the well casing. Victim #1 accessed the well pit with an extension ladder and poured up to 3.75 gallons of muriatic acid into the casing to dissolve the rust, exited the pit and waited several hours before entering the pit again to attempt to remove the pump. Victim#1 became overcome by vapors that had built up inside the well and collapsed. Victim#2 was working at the top of the well, entered the well to assist Victim#1 and was also overcome and collapsed. The homeowner called 911. Within minutes a 32-year veteran firefighter of the Liberty Township Volunteer Fire Department (VFD) arrived in his personal vehicle and became Victim#3 when he entered the well, attempted to climb back out after noticing the vapors and then collapsed after being overcome falling off the ladder back into the well pit. Shortly after, another Firefighter (6-year veteran) with the Liberty Township VFD arrived in his personal vehicle and became Victim#4 as he entered the well to assist Victim#3 and also collapsed. Next on scene were 4 more firefighters with Liberty VFD who suited up with self contained breathing apparatus (SCBA), rigged up ropes, and began removal of the victims. During removal of Victim #4, the first out, Muncie Fire Department arrived and assisted with the rescue providing a harness to remove the other victims. Delaware County EMS arrived and transported all victims to Ball Memorial Hospital, where Victim #1 died. Victim#2 was in a medically-induced coma and was found to show no brain activity and taken off of life support on June 8, 2010. Victim#3 received 15 staples to close wounds on his head from falling off the ladder into the well when he attempted to self-rescue and was released from the hospital June1, 2010. Victim#4 was also placed in a medically-induced coma suffering from respiratory failure and was on a ventilator for approximately 4 days. Victim#4 was released from the hospital expected to make a full recovery. After consulting with Delaware County Health Department and IDEM, Liberty Volunteer Fire Department put 1,800 gallons of water into the well pit to flush it clean and displace any residual acid. It was confirmed through interviews of Victim#1 and #2's family members and Victim#1's employer that Victim#1 was a licensed plumber doing work independently as a favor for a friend. No compensation was to be given for work performed at this residence. Contact 3 stated that when he finished helping the customer/mechanic look for the rivets he went to the 100 LL aviation gasoline truck to talk to the victim. Contact 3 stated he couldn't find the victim on the tarmac around the 100 LL aviation gasoline pump or in the cab of the truck, he then climbed the fixed ladder on the side of the truck and found the victim lying on top of the truck with his head inside the inspection/quick ventilation hatch for the 1200 gallon tank. Contact 3 removed the victim's head from the quick ventilation hatch and checked the victim for a pulse. Contact 3 stated the victim had no pulse and he and contact 2 waited for emergency assistance. The victim was pronounced dead at the scene by the paramedics. The Medical Examiner from the Anoka County Coroners Office indicated asphyxiation as the preliminary cause of death. The cause of death as indicated by the Medical Examiner from the Anoka County Coroners Office was "Inhalation of aviation gasoline". The training videos used by the employer to train the victim included the procedure of looking into the ventilation hatch as a means of manually checking the overfill gauge in the top of the tank. There was no mention in the training videos of the tank of the 100 LL aviation gasoline truck being a permit-required confined space. The company is in the business of manufacturing organic fertilizers and other related products. On April 1, 2010, at approximately 2:30 p.m., Worker #1 and Worker #2 (leadman) were preparing product involving the use of fermented molasses as one of the ingredients. The workers were having problems moving the molasses from tank #2 to tank #1. The owner of the company was out of state but was in telephonic communication with Worker #2 regarding the problems with the molasses. According to the employer he told Worker #2 to add water to the molasses to bring it to a level where the container contents could be transferred to tank #1. Due to the low volume of product in tank #2, Worker #1 entered and was descending a rope ladder into the storage tank, which contained fermented molasses, to attempt to push the molasses through the piping into tank #1. When descending the ladder worker #1 fell or slipped off the ladder falling into the bottom of the tank. Worker #2, heard Worker #1 fall and went on top of the tank to see what happened. Worker #2 called for assistance via 911, and subsequently entered the tank to attempt rescue when he collapsed falling down into the tank. Worker #3, heard Worker #2 drop into the tank and went to the top of the tank and began to enter it through the top, but stopped because he was not sure how he would get workers #1 and #2 out through the manhole. Worker #3 climbed down off of the tanks and found a electric saw and cut a hole in the bottom side of the tank. Worker #3 then entered the tank and while trying to pull one of the victims out was overcome and passed out, falling to the outside of the tank. Upon arrival, the Lawrence Fire Department and EMS began treatment of Worker #3 and transported him to hospital where he was treated and released. The Lawrence Fire Department extracted workers #1 and #2 from the tank and attempted to revive them. Workers #1 was taken to hospital and was pronounced dead, and Worker #2 pronounced dead at the scene. The molasses tanks #1 and #2 measured approximately 14 feet in height, 120 inches in diameter, with domed tops, and single manhole measuring 21 inches in diameter. A rope ladder was used to enter/egress the interior of the tanks and was either attached to the top of the tank or the ceiling bracing of the structure. Access to the tanks from the outside was by ladder to the top of tank #1 the workers would then jump across to tank #2. There is no catwalk and insufficient space to access the top of tank #2 with a ladder. Oxygen levels taken during the course of the investigation in the molasses storage tank #2 was measured and found to be 9.9while carbon dioxide levels were found to be between 56,977 and 97,932 parts per million.The employer did not have any equipment or devices to monitor the atmosphere in the tanks, and used a box fan to ventilate for an hour or two before entering into product tanks. Employees harvest avocados on privately owned ranches from Carpinteria to Goleta using aluminum extension ladders, picking poles and clippers. The employee was just finishing up his second season working for Andrade Farm Labor as an avocado picker. The accident occurred on R. E. Hall Ranch, an avocado ranch located at 4189 Foothill Road in Carpinteria, CA. At the time of the accident, the employee had just placed his 32 foot aluminum extension ladder up against an upper branch of an avocado tree and then placed his left hand and right foot on the ladder to ascend the ladder. Without this support brace the power line sagged down to the level where V1 was working. This power line was not in contact with the pole or any other lines. It was in fact approximately 2.5 feet away from the pole. V1 was doing his work while in between the power line and his ladder. At some point, with his back in contact with or close proximity to the exposed and energized power line, he also made contact with a second path for which current could flow. Csho was briefed by Chief Moore of the condition and waited for Csho. Upon arrival there were representatives of the news media, Milford Fire and Police Departments and UI. Later to join was the mediocolegal death investigator (MDI). Chief Moore stated that the deceased was Mr. Victor Larranaga, EE2 of 17 Linen Ave Bridgeport, CT. The other employee is Hevardo Macquez who was transported to Bridgeport Hospital for burn treatment and injuries and was released on June 3, 2010. The decrease's identify was later confirmed by the MDI, Alfredo Camargo. Chief Moore explained from eye witnesses' reports that the 2 employees had been power washing the 3 story home all day. The employees were soaking wet. It was explained that the employees had tied a 40 ft aluminum ladder to 2 other small aluminum extension ladders ("daisy chain") to extend the ladder approximately 40ft. The employees were working on the second tier of the home trying to access the upper most area in the peak of the triangle. EE1 was on the ladder and came down while EE2 took the ladder to move it and it is believed that EE2 lost his hold and the ladder went back hitting the primary conductive power line which was approximately 13, 800 Volts. The MFD's report number is 10-03312. The home owner contacted Premier Maintenance Inc to perform power washing of all eaves, some caulking of windows, and to touch up with varnish on eaves as needed. The building is a 2.5 story 3700-3800 sq ft home. The 2 employees came out to power wash the house with several metal (aluminum) ladders and while elevated (over 18 feet from the ground below) on upper level of the house, the employee was moving a "daisy chained" ladder when the ladder swayed back and hit a 13,800 Volt power line. The first employee tried to help the first and was knocked to the ground and was taken to the hospital for treatment at the burn unit. The second employee was electrocuted. Employee #1 and Employee #2 observed a small amount of smoke trailing from the top of the cyclone on the dust collection system. The dust collection system, located outside of the facility, serviced woodworking machinery. The local fire department had been called and while waiting for the fire department to arrive, the employees decided to open the gates on the bottom of the hopper bin and empty the contents of the bin onto the ground. Employee #1 climbed the ladder accessing the platform where the hydraulic control for the gates was located. Employee #1 opened the gates a little ways first before opening the gates most of the way. The contents of the hopper bin fell to the ground and dust bellowed up around the hopper bin on both sides. The dust cloud ignited and Employee #1 was engulfed in the flash fire. Employee #1 ran/jumped from the platform to the concrete approximately 13 feet below. Employee #2 attempted to put the fire out on Employee #1 by patting him and rolling him on the ground. Employee #1 was conscious and coherent when he was transported by EMS to the hospital. Employee #1 was air evacuated to Via Christi St. Francis Hospital in Wichita, KS where he later died.
