World Health Organization’s Definition
The social determinants of health are the conditions in
which people are born, grow, live, work and age.
These circumstances are shaped by the distribution of
money, power and resources at global, national and
local levels. The social determinants of health are
mostly responsible for health inequities - the unfair
and avoidable differences in health status seen within
and between populations.

Evaluation Grants
• Two years of support to housing organizations to demonstrate the impact of
their supportive services on the health of their clients
• Each grantee focuses on a distinct population (elders, the chronically
homeless, low-income families) and delivers services in varied settings across
the Commonwealth
• Goal of analyses: identify greater synergies between the health care and
social services fields, with an eye toward supporting an advocacy and policy
agenda focused on payment options for social services
• Analyses are expected in late 2018/early 2019
