This document is an interview evaluation form used by human resources to assess candidates for job positions. It instructs interviewers to rate candidates on a scale of 1 to 5 based on their educational background, prior work experience, technical qualifications, communication skills, interest in the position, knowledge of the organization, teamwork skills, initiative, time management, and customer service abilities. Interviewers are asked to provide comments to justify each rating.
