• Prospective Advisers (Grade 8, 9, 10 and
12) must secure a copy of the FORM 1
(previous school year) of the class assigned
to him/her and gather the MLESF of their
students.
• Grade 7 and 11 advisers must gather the
MLESF of their prospective students
