This document outlines an individual learning monitoring plan for a Grade 3 student named John Erris Reyes. The plan identifies two areas for improvement: expanding Filipino vocabulary and mastering letter sounds and cvc words in English. For Filipino, the learner will be provided vocabulary lists and passages to read daily. Communication with parents will also occur regularly. For English, the learner will review consonant sounds then vowels sounds. Activities are outlined to practice cvc words by vowel family over 4 weeks. Progress will be monitored on October 21 for each subject area.
