This review article summarizes the current evidence on therapeutic hypothermia for neonatal hypoxic ischemic encephalopathy (HIE) from randomized controlled trials. The review identified 7 trials including 1214 newborns that compared therapeutic hypothermia to normothermia. The results showed that therapeutic hypothermia reduced the risk of death or major neurodevelopmental disability at 18 months and increased the rate of survival with normal neurological function. Both total body cooling and selective head cooling were effective in reducing these risks. The review concludes that hypothermia improves outcomes for newborns with moderate to severe HIE and should be considered as part of routine clinical care for these newborns.

Human Touch vs. Axillary Digital Thermometry for Detection of Neonatal Hypoth...Siddharth Agarwal
We examined the diagnostic accuracy of human touch (HT) method in assessing hypothermia against
axillary digital thermometry (ADT) by a trained non-medical field investigator (who supervised
activities of community health volunteers) in seven villages of Agra district, Uttar Pradesh, India. Body
temperature of 148 newborns born between March and August 2005 was measured at four points in time
for each enrolled newborn (within 48 h and on days 7, 30 and 60) by the field investigator under the
axilla using a digital thermometer and by HT method using standard methodology. Total observations
were 533. Hypothermia assessed by HT was in agreement with that assessed by ADT (<36.5
C) in 498
observations. Hypothermia assessed by HT showed a high diagnostic accuracy when compared against
ADT (kappa 0.65–0.81; sensitivity 74%; specificity 96.7%; positive predictive value 22; negative
predictive value 0.26). HT is a simple, quick, inexpensive and programmatically important method.
However, being a subjective assessment, its reliability depends on the investigator being adequately
trained and competent in making consistently accurate assessments. There is also a need to assess
whether with training and supervision even the less literate mothers, traditional birth attendants and
community health volunteers can accurately assess mild and moderate hypothermia before promoting
HT for early identification of neonatal risk in community-based programs.

To evaluate the diagnostic value of Copeptin as a novel biomarker in early diagnosis of Acute Myocardial Infarction. 56 patients with acute Myocardial Infarction (STEMI) and 25 healthy controls who were admitted to the Cardiology and Clinical Pathology Departments, national heart institute (NHI) from October 2015 to April 2016. The kit used a double-antibody sandwich enzyme-linked immune-sorbent assay (ELISA) to assay the level of Human Copeptin in samples. As regard copeptin, the median range of copeptin level was 242.5pg/ml in patient group and 75pg/ml in control group. The comparative study between the two groups shows a significant difference (p < 0.05) Conclusion: Copeptin is a reliable diagnostic tool in patients with AMI (STEMI) with sensitivity 85.7%, specificity 86.7%, PPV 96% and NPV 61.9%.

Over the past few decades, the prognosis of neonatal seizures has experienced considerable enhancement due to the improvement in neonatal and infant care. The mortality rate of neonatal seizures has fallen from 40% to 20%, and the relationship between electro encephalogram (EEG) and prognosis has become quite clear. The underlying cause of seizures is a major determinant of the outcome of the disease. For example, patients with secondary seizures and hypoxic-ischemic encephalopathy have only 50% chance of normal development and total recovery, while newborns with secondary seizures and subarachnoid hemorrhage or better hypocalcemia have higher chances of recovery. Searches were conducted by two independent researchers in international (PubMed, Web of Science, Scopus, and Google Scholar) and national (SID and Magiran) databases for related studies from the inception of the databases to September 2017 (without time limitation) in English and Persian languages. It is possible to achieve accurate diagnosis through checking the history before birth and performing a thorough physical examination in some rare cases. Depending on the case, tests or additional actions can be done. EEG is the primary means for diagnosis and may exhibit paroxysmal activity in the difference between seizures or may produce electrographic seizures in cases where seizure is hidden or latent. One of the most important points in the treatment of neonatal seizures is the diagnosis of underlying cause (such as hypoglycemia, meningitis, drug deprivation, and trauma) because such diagnosis facilitates different approaches to control neonatal seizures. Most experts agree to control all clinical and electrographic seizures. Some others focus merely on clinical seizures. Most centers prefer the first approach. An important point before starting an anticonvulsant drug is to decide if the patient needs intravenous and luteinizing treatment with an initial bolus dose, or it can be easy to start treatment with a prescription for a long-acting medication based on the severity of seizure, duration, and frequency.
