Base Service Fee


  The Base Service Fee includes all basic services before inclusion of
  Optional Services, which could be charged separately. To determine
  the fee, the following information is needed:

      Federal Tax ID #
      Years in Business
      Description of Operations
      The number of employees (or anticipated # by year end)
      The number of managers supervising the workforce
      Estimated annual employee turnover




One-Time Consultation Fee – Limited to Base Services

Monthly Service Fee (3 - 12 month agreement)
           ◊   0 - 10 employees          $500 Base Rate
           ◊   11 - 74 employees         $25 / FTE
           ◊   75 – 100 employees        $35 / FTE

Employee Records Fee


    o There is a $2.50 charge per employee record for all active
       employees to exam existing employment records for compliance.


Records Set-Up Fee (one-time fee)

    terminated* employees, includes materials and supplies
       ◊   Less than 100 employees - $50
       ◊   100-250 employees - $150
       ◊   Greater than 250 employees - $175
