This document outlines the 5 steps of the "STOP" observation process: 1. Observe work activities and the surrounding environment, looking for both safe and unsafe acts. 2. If an unsafe act or condition is observed, stop the job and discuss it with the workers involved to understand the risks. 3. Make recommendations with worker input on how to complete the job safely. 4. Complete a behavior observation card with details of the unsafe act and agreed corrective actions. 5. Be specific on the card about the observation, corrective actions, completion dates, and observers. Submit the card for review.

4. STEP ONE
Observe work as it is taking place. This includes:
• Observing people’s activity
• Observing people’s actions
• Observing the surrounding environment.
• Look for unsafe acts as well as good safety performance.
• Look for detail and an overall impression.
• Keep an open mind.

5. STEP TWO
• If an unsafe act or condition is observed, stop the job
immediately unless stopping the worker will create a
greater hazard; discuss the situation with the individuals
involved.
• Be understanding. Try to make the workers understand why
you had to stop the job by highlighting the risk they may
be exposed to.
• Ask them for suggestions on the safer way to do the job.

6. STEP THREE
• Make recommendations with the workers input on how to
correct the situation and complete the job safely.

Step Four
• Complete the appropriate behavior observation card.
Be sure to mark all the required boxes and provide a
brief summary of the unsafe act and corrective action.

7. STEP FIVE
• Be specific and check the appropriate category and box(s)
that apply.
• Give brief description of observation in comment section.
• List any corrective actions and dates they are to be
completed.
• Provide name of person(s) completing the observation.
• Submit card for review and processing.
