Depression has become a common mental disorder in our elderly population. This has caused a global concern for occur, geriatric patients, as depression often results in a significant burden for families as well as communities. Elderly people who suffer from depression may have an inferior baseline and record for medical assessments than those individuals without depression. Despite consistent evidence of the effectiveness of antidepressants for many with depression,

particularly those with more severe depression, remission rates are disappointingly low. An AHRQ-sponsored report found that only 46% of patients experienced remission from depression during 6 to 12 weeks of treatment with second-generation antidepressants. One major reason for this issue is non-adherence to medications and treatment plans. Studies have shown that patients' age, race and ethnicity are consistently associated with predictions of outcomes. (Rossom et al., 2016).
This case study involves a 69-year old man whose chief complaint is unremitting, chronic depression. After several years of medications and treatments, he feels hopeless for a recovery from his chronic depression. This assignments seeks to explore his family and social support systems, diagnostic testing, differential diagnosis and pharmacologic treatment options for this patient.

These questions are an important yet simple place to start when treating patients. Sleep disturbances plague much of the world's population and have shown to be a major indicator for mental health issues. Changes in sleep neurophysiology are often observed in depressive patients, and impaired sleep is, in many cases, the chief complaint of depression (Armitage, 2007). Depressed patients with sleep disturbance are likely to present more severe symptoms and difficulties in treatment. In addition, persistent insomnia is the most common residual symptom in depressed patients and is considered a vital predictor of depression relapse and may contribute to unpleasant clinical outcomes (Hinkelmann et al., 20120. Questions involving feelings of hopelessness and suicidal ideations with or without a plan relate to issues of patient safety. Across psychiatric disorders, hopelessness is associated with suicidal ideation and behavior. A meta-analysis of 166 longitudinal studies (sample size not reported) found that hopelessness was associated with an increased risk of ideation (Ribeiro, Huang, Fox, & Franklin, 2018).
Family and social support system
Family and social support systems are imperative for any patient in recovery. If the patient is agreeable to discussions with family members, then a discussion with his wife would be helpful. Researc.

Many people throughout the world experience some type of depression in their lives and it is one of the most common mental disorders. The current statistic show that depression is linked to genetic, environmental, biological and is also psychological. Depression can ben found with any age person. A small child or an adult may have to deal with the depression that is affecting them. Chemical imbalances in the brain is the leading cause for a person dealing with the depressive order. The neurotransmitter is the what we call the communicator between the brain and the limbic system. Researchers study the limbic system in the brain as this is where depression starts; especially for anxiety and stress. The 3 major neurotransmitters; serotonin, norepinephrine, and dopamine all have direct relations with a persons’ depression and anxiety.
Serotonin plays a crucial role in our brain. It is associated with many physical actions that we may portray. The actions associated with serotonin are mood altering, sleeping patterns, eating disorders, and aggression. If a persons’ serotonin levels decrease, they may experience these depressive symptoms. This can also make persons have a feeling of self-worth and suicidal feelings.
Another transmitter in the brain which is associated with the depressive disorder is dopamine. This is the part of the brain that deals with our motivation and how we gain the feeling of self-worth and self-pleasure. Early studies suggested that an existence of neurotransmitter norepinephrine deficiency in some certain areas of the brain resulted in depression. One main cause of depression is the reduction in the concentration of certain neurotransmitters in the brain, such as serotonin and dopamine. The decrease in the concentration of these neurotransmitters leads to disturbed neuronal signal processing which leads to alterations in the structure of the neuronal networks. These basic changes are accepted to be one of the fundamental purposes behind sorrow. The emergence of neuroimaging techniques, magnetic resonance imaging (MRI), positron emission tomography (PET) and functional fMRI, established the importance of the ‘neurocircuit of emotion’ which has been expanded to include other important brain areas and the prefrontal cortex (PFC). These brain sites and their connections, which have been widely studied, are responsible for maintaining emotional stability and their malfunction is considered central to the pathophysiology of depression (Palazidou, E., 2012).
Recent follow up studies also shows that there is a group of individuals with a depression disorder who exhibit low levels of the chemical norepinephrine. In autopsy studies, it has been shown that in comparison,.

a role in the manifestation of pain.

in the development of pain.

findings
The CNS pathway responsible for inhibition of pain
sensation includes projections from brainstem nuclei
to the spinal cord dorsal horn via the dorsolateral
funiculus (DLF).
More specifically, DLF fibers are comprised of
serotonergic projections from the raphe nuclei,
dopaminergic projections from the ventral
tegmental area (VTA), and noradrenergic

Dysfunction of the central noradrenergic system has
been hypothesized to play a role in the
pathophysiology of MDD, based upon evidence of
decreased norepinephrine metabolism, increased
activity of tyrosine hydroxylase, and decreased
density of norepinephrine transporter in the locus
coeruleus in depressed patients.
In addition, decreased neuronal counts in the locus
coeruleus, increased alpha-2 adrenergic receptor
density, and decreased alpha-1 adrenergic receptor
density have been found in the brains of depressed
suicide victims post-mortem.

are relevant targets for pharmacological
intervention.
Triple reuptake inhibitors under development,
which enhance neurotransmission of all three
systems, should be a potential next-generation

and a quicker therapeutic effect.

Below are different ratios of serotonin to norepinephrine reuptake inhibition. As you can
see, Effexor has the most lop-sided ratio. Newer drugs like Cymbalta and Pristiq slightly
bridged the gap, but still focus more on serotonin than norepinephrine. The latest SNRI
approved for depression called Fetzima favors the norepinephrine, but has almost an
equally balanced ratio.

Fetzima (Levomilnacipran)
Approved in 2013, this is an antidepressant that works as a serotonin-norepinephrine
reuptake inhibitor (SNRI). It contains the “levo” enantiomer of the drug milnacipran and has
similar effects. In comparison to other drugs, this drug provides a more balanced reuptake
of both serotonin and norepinephrine. What makes this drug different from other SNRIs is
that its ratio of reuptake inhibition is almost even (1:2).
66. Non-Antidepressant SNRIs
Sibutramine
Approved 1998, is an appetite suppressant drug that was initially prescribed
to help treat obesity. Due to the fact that it’s been associated with increased
risk of strokes and cardiovascular problems, it was pulled from the
market in 2010. This drug acted centrally as an SNRI with a distinct
mechanism of action, closely resembling amphetamines.
It differs from other appetite suppressants (e.g. amphetamines) because it
doesn’t force the release of neurotransmitters, it only prevents their
reuptake.
Savella (Milnacipran)
Approved 2009, this drug is utilized for the treatment of fibromyalgia. It has
also been approved to treat major depression in countries outside the
United States. This drug inhibits the reuptake of serotonin and
norepinephrine at a ratio of 1:3. The drug Fetzima (Levomilnacipran) which
was approved in 2013 in the United States is very similar to this drug and it
inhibits reuptake at a 1:2 ratio – which is slightly more balanced.
