2. What is surgery
• Surgery is an art. It is that branch of medicine
which is concerned with the repair, Removal or
replacement of the diseased tissue; in other
words, any cutting, suturing and repairing of
human tissues.
• The act of performing surgery may be called
a surgical procedure, operation, or simply
"surgery“.

• The veins around your anus tend to stretch under

10. 1. Primary hemorrhoids cause :
1. Standing position: of the human being is partly
responsible for piles because blood has to flow
against gravity.
2. Anatomical position: the veins passing through the
submucosa of the rectum, get constricted during the
act of defecation.
3. Familial or genetics: absence of valves or congenital
weakness of vein wall.
4. Constipation.: cause excessive straining. Thus
gravity, straining and irregular bowel habits are
important factors in the development of piles.

11. 2.secondary hemorrhoids causes:
A)Carcinoma of rectum: by blocking the veins, can
produce back pressure and can manifest as piles.
B)Portal hypertension: is an un common causes of piles
(rectal varices).
C)Pregnanacy : due to compression on superior rectal
veins or due to progesterone which relaxes smooth
muscles in the wall of th veins, can cause
haemorrhoids.

a) internal haemorrhoids: above the denate line covered
with mucous membrane.
b) external haemorrhoids: at anal verge, covered with
skin.
c) interno-external: both varieties together.

• Severe pain
• Inflammation
• A hard lump near your anus

16. • Internal hemorrhoids
• Internal hemorrhoids lie inside the rectum. You usually can't
see or feel them, and they rarely cause discomfort. But
straining or irritation when passing stool can cause:
• Painless bleeding occurse after defaecation. Splash in the pan

toilet tissue or in the toilet.

canal and usually require manual reduction.
• Grade IV : hemorrhoids are irreducible and
Parmentely prolapsed. And patient complaint pain
or discomfort.

• Other risk factors historically associated with the development of
hemorrhoids include the following:
• Lack of erect posture
• Familial tendency
• Higher socioeconomic status
• Chronic diarrhea
• Colon malignancy
• Hepatic disease

• Loss of rectal muscle tone
• Rectal surgery
• Episiotomy
• Anal intercourse
• Inflammatory bowel disease, including ulcerative colitis, and Crohn
disease

23. Conservative Management
• Eat high-fiber foods. Eat more fruits, vegetables and
whole grains. Doing so softens the stool and increases
its bulk, which will help you avoid the straining that can
worsen symptoms from existing hemorrhoids. Add
fiber to your diet slowly to avoid problems with gas.
• Use topical treatments. Apply an over-the-counter
hemorrhoid cream or hydrocortisone.
• Soak regularly in a warm bath or sitz bath. Soak your
anal area in plain warm water for 10 to 15 minutes two
to three times a day.
• Take oral pain relievers. You can use acetaminophen
(Tylenol, others), aspirin or ibuprofen (Advil, Motrin ,
others) temporarily to help relieve your discomfort.
