2. Creating the Form
1. Log in to your Google account.
2. Google search: Google Forms.
3. Click on the first result, then click “Go to
Google Forms”
4. Rename the Untitled form by double-clicking
on that text and replacing with the name of
your assessment. (Ex: Ancient Egypt Test)
3. Adding Your Questions
1. Add questions one at a time, selecting
question types from the “Add Item”
dropdown menu.
2. Review the video tutorial for question types,
and inserting page breaks and graphics.
4. Good Practices to Keep in Mind
Make student identifying information the first
thing that students enter. This makes it easier
for tracking purposes during grading.
Example: Question 1 (Text): Last Name
Question 2( Text): First Name
Question 3 (MC): Period Selection
5. Changing the Theme
1. In the toolbar above the questions, select
“Change Theme.”
2. This will allow you to choose from a variety
of graphic options to make it more user-
friendly. Please select one other than plain
white.
6. Previewing Your Assessment
1. Click “View Live Form” in the toolbar.
2. This is a preview of what others will see
when taking your test.
3. This URL address at the top is also the link
that you can share with others who are
taking your assessment.
7. Administering the Test
1. Click “Send Form” in the upper right-hand
corner.
2. Enter the email addresses of the people you
want to take your assessment.
For this assignment, put my email address that
you used for Task 1.
Note: You can have your class groupings pre-loaded so
you can send to all students in one click.

3. When giving the assessment to students,
you have two options. In addition to sending the
link via email, you can post it to your class site.
I prefer this second option so that I can upload
it right before the test and all students can
access without glitches.
9. Viewing the Data
1. Go to your Google Drive.
2. Find the document with your test name +
(Responses), and this is the spreadsheet of
your data so far.
