This document contains a checklist to assess factors that can affect a student's study habits. The checklist is divided into three sections: 1) Family Condition, 2) Technology, and 3) Class Schedule. For each section, students are asked to check off statements that apply to their situation and may hinder their ability to study effectively. Some examples of challenges listed include a noisy home environment, lack of access to technology, and an exhausting class schedule. The checklist aims to identify external barriers students face in developing strong study habits.
